Anyone with the least knowledge of present day urology will know that there are many very controversial aspects of this subject. Urology is not alone in this unsettled environment for there are similar debates in almost all other aspects of surgery. In addition to the rapid changes in technology, an important part of the explanation for these controversies is simply that more surgeons are prepared to admit that no area of their work is so established that it does not bear further scrutiny and assessment. Argument can be tedious but debate is healthy. This book aims to present material that is debatable: experienced practitioners of each topic explain why an opinion or preference can be sustained. The purist might wish to have these opinions resolved by a well-planned clinical trial, but experience of clinical trials shows that they do not always produce results that are easily translated into a positive change in clinical practice. Would the reader be persuaded more by a statistically significant difference or by an experienced clinician who has reached certain conclusions? Both are risky, but since surgical techniques are often not easily converted into appropriate studies or trials the Editors have obtained the best opinion on each controversy and innovation. The media adore controversy for its own sake. The medical profession accepts controversy but knows that few events in medicine are absolute and few procedures can be done only by one method.

Book excerpt: These proceedings emanate from the Second Prouts Neck Conference on prostate cancer held on October 17-19, 1986, the theme of which was treat ment, with focus on current issues and future research that is needed to answer critical questions related to optimal management of the various stages of prostate cancer. The objective was to reveal the most crucial problems impeding progress and to crystallize the combined multidisci plinary input generated by the conference into focused concepts or recommendations for presentation to the National Cancer Institute (NCI) , with the ultimate intent of targeting research to address the priority issues identified. In organizing the workshop, every effort was made to maintain a multidisciplinary balance among nationally renowned authorities on prostate cancer. Thus, leading surgeons, radiation and medical onco logists and biostatisticians were in equal presence. While there were spirited exchanges with careful scrutiny and critique of all data pre sented, there was a common belief that the challenge of prostate cancer would be best approached in this multidisciplinary Organ Systems-oriented fashion. During the course of the conference, it became apparent to all present that major nomenclature and procedural barriers have made it generally difficult, and frequently impossible, to compare results of clinical research.

Book excerpt: This interdisciplinary approach is reflected in the broad scope of this volume, which aims to provide an overview of current treatment strategies for bladder and prostate cancer. The first part of the book discusses the optimization of radiation techniques like 3D planning or IMRT and focuses on definitive radiotherapy with or without adjuvant or neoadjuvant hormonal manipulation. The quality of life after therapy is another important issue. Further chapters deal with radical cystectomy and organ preservation by radio-chemotherapy as well as the treatment of T1G3 cancer. In the last part, the emphasis lies on brachytherapy, which has become increasingly important for the treatment of prostate cancer. This book will be of great value to urologists and radiation oncologists as well as to all others involved in the care of patients with urologic cancer.

Available in PDF, EPUB and Kindle. Book excerpt: Penile implants have been an effective treatment for erectile dysfunction (ED) for more than 40 years. However, there is now a growing interest in this treatment option, as the number of procedures is expected to increase in the future. This book offers a reference guide for all those practitioners interested in understanding and/or performing this procedure. It will also benefit those in medical education who want to obtain specialized and high-quality knowledge regarding the complexities involved in penile implant surgery. Although the basic description of penile implant surgery is well documented in the literature, a detailed and comprehensive guide with a special focus on the most challenging and controversial aspects is lacking. The few books available on the topic provide a broad overview of prosthetic surgery in general, mainly discuss basic principles, and fail to provide a more detailed perspective. This comprehensive book discusses various aspects of penile implant surgery, ranging from indications to long-term complications. It addresses the most common and serious challenges regarding penile implants, which are rarely discussed elsewhere. The respective chapters were written by internationally respected experts on the controversies of penile prosthetic surgery. The book is intended for all those who plan to specialize in penile implant surgery, which include, but are not limited to, residents and fully trained urologists, as well as fellows and practitioners in sexual medicine and reconstructive urology.
