Book excerpt: Objectifs: Mesurer la dose moyenne d'irradiation reçue au niveau du pouce, de l'index et du cristallin sur une période de trois mois. Conclure sur la dose efficace moyenne estimée pour un seul cliché radiographique intra-oral. Méthodes: L'étude s'est déroulée dans les services d'Odontologie Pédiatrique et de Soins Adaptés de l'Hôpital Universitaire de Rennes. Les praticiens ont été invités, pendant trois mois, à porter deux bagues dosimètres et un dosimètre cristallin à chaque fois qu'ils devaient tenir le capteur pour un examen radiographique. La dose totale enregistrée nous a permis de conclure sur la dose de rayonnement d'un seul cliché radiographique. Résultats: Les dosimètres ont été utilisés 109 fois pour des radiographies intra-orales au cours desquelles l'opérateur tenait le capteur radiographique avec ses doigts. Pour un seul cliché radiographique, la dose équivalente était de 0.75 mSv sur le pouce, 0.74 mSv sur l'index et en dessous du seuil de détection pour le cristallin. Le facteur de pondération du rayonnement X sur les tissus cutanés étant de 0.01, la dose efficace estimée pour une radiographie est donc de 0.0075 mSv au niveau des doigts. Conclusions: Nos valeurs sont cohérentes avec les résultats des études précédentes dans la littérature. Les étudiants dentistes ne doivent pas effectuer plus de 1 radiographie, en tenant le capteur avec les doigts, par semaine (en considérant 40 semaines de travail par an) pour éviter une exposition des mains supérieure à la limite de dose « population » de 50 mSv. Compte tenu de ces résultats, il apparaît que l'exposition des mains des étudiants et des dentistes travaillant principalement avec des enfants et des patients handicapés devrait faire l'objet d'une surveillance plus étroite.

Ce jour marqua le début d'une révolution dans le monde dentaire et médical mais certains pionniers de la radiologie en paieront un fort tribut, ne se protégeant pas suffisamment des effets de ce rayonnement. Nombreux furent ceux qui, moururent des effets de ce nouveau rayonnement, après avoir reçu une dose d'irradiation trop élevée. À cette époque, l'acquisition d'un appareil de radiologie dentaire relevait du « parcours du combattant ». En effet, les premiers appareillages de radiologie dentaire n'étaient pas encore disponibles en un seul bloc. Le dentiste devait, par ses propres moyens, se procurer les différents éléments de l'appareil sous forme de pièces détachées. Nous décrirons ainsi l'appareillage radiologique dentaire à ses débuts : sa construction et son évolution notamment avec le tube de Coolidge que nous utilisons encore aujourd'hui. En s'appuyant sur l'aspect historique, nous aborderons les différentes techniques de radiologie, que nous utilisons fréquemment en cabinet dentaire y compris la radiologie panoramique et la radiologie numérique. Notre but sera de découvrir par les expériences de nos prédécesseurs ce qui fait notre quotidien.
