The author adhered tenaciously to this theory, involving the use of a small-diameter femoral head, throughout the epoch when the large-diame ter, metal-to-metal design dominated the field. During that considerable period general satisfaction with the early results rendered criticisms of the large-diameter head unwelcome. There was a formidable array of counter criticism: the small head would pierce a film of synovial fluid; the small head would wear the socket too rapidly; the small head would always have a high risk of dislocation; detachment of the trochanter, to achieve precise orientation for the small head, was unacceptable. But all these objections have now been largely overcome. Lubrication of high molecular weight polyethylene (HMWP) on metal is now accepted as being mainly by the boundary regime with thick fluid films playing no part. We now know that HMWP can indeed tolerate the very high stresses imposed by the small head and in tribological theory there may even be some advantage in high stress. Dislocation is now known not to be an automatic sequel to the small head.

Twenty years ago, Charnley first introduced bone cement to hip surgery, enabling total hip replacement to be practised on a larger scale. There is no question, however, that new approaches must be found to solving the problems of implant loosening, either by developing a new type of cement or by directly anchoring the implant without using ce ment. Much research has been done in this direction in recent years. The methods which have already been tested or are currently being tested were presented and discussed at a symposium on Cementless Fixation of Endoprostheses organized by the Orthopaedic Clinic of the University of Basel and held on 24-26 June 1982. Various possible ap proaches which might be realized in practice were discussed in a relaxed atmosphere. In addition to the biomechanical bases of cementless fixa tion of endoprostheses, the various models were discussed, with special regard to their biocompatibility, physical characteristics, design, clinical applicability, and previous clinical experience. This book contains the papers delivered at this symposium.

It is self-evident, too, in gastro enterology and pulmology that a functional analysis is based on readings of different electronic probes. In urology, however, we tend to rely on optical control by cystoscopy, static data in order to reach a functional interpretation that is based on this information alone. " Referring to the preface mentioned above, especially the upper uri nary tract is said to be assessed mainly from an anatomic-morpho logic point of view in the clinical routine, though in this case we are also mainly concerned with an active transport system. 1971, which the Twelve years after the urodynamic meeting in above-mentioned volume was based on, a second urodynamic meet ing took place in Aachen that was called and also dealt with "Re-: viewing the Aachen 1971 Meeting. " Its purpose was to review which of the statements that had been made more than a decade before were still valuable, what had to be added or revised, and which ex perimental tests and results could be clinically applied.
