Si les troubles morphosyntaxiques ont suscité un grand intérêt de la part des chercheurs, l'étude des troubles phonologiques est très récente et l'on compte peu de travaux sur le français. A l'heure actuelle, aucun consensus n'a pu être obtenu quant à l'origine des troubles observés. Une hypothèse récente soutient que les enfants dysphasiques auraient un trouble d'accès à la complexité linguistique (Van der Lely, 2005). Elle permettrait d'expliquer pourquoi certains chercheurs s'accordent à considérer que les représentations phonologiques des enfants dysphasiques sont sous-spécifiées. Nous avons donc cherché à déterminer si la complexité phonologique influe sur les performances expressives des enfants dysphasiques francophones. Nous avons proposé une tâche de répétition de mots et de non-mots à neuf enfants, âgés de 7;3 ans à 10;8 ans (moyenne 8;9 ans, écart-type de 1:5), diagnostiqués dysphasiques à l'hôpital Clocheville de Tours. Le protocole a été élaboré de façon à tester les segments les plus complexes du français dans des positions syllabiques simples et complexes. Les résultats ont montré que les représentations phonologiques des enfants dysphasiques semblaient peu spécifiées à plusieurs niveaux : au niveau des positions syllabiques complexes, au niveau de certains segments associés aux positions syllabiques complexes ; au niveau de segments proches sur le plan articulatoire. Cette étude vient donc, d'une part, soutenir l'hypothèse d'un trouble d'accès à la complexité phonologique responsable d'une sous-spécification des représentations phonologiques. D'autre part, elle a permis de caractériser la représentation syllabique possible des enfants dysphasiques francophones. Ainsi, les résultats permettraient d'expliquer les phénomènes phonologiques observés dans le langage oral de ces enfants. Nous espérons que les informations recueillies dans cette étude pourront permettre de mieux cibler la prise en charge orthophonique des enfants dysphasiques et peut-être apporter des éléments de dépistage précoce.

Nous avons émis l'hypothèse selon laquelle l'utilisation des comptines dans la rééducation orthophonique de ces enfants pourrait avoir un impact positif tant sur le bégaiement que sur la phonologie. Nous avons donc utilisé cette médiation auprès d'enfants ayant de 5 à 8 ans, présentant les troubles précités sur une période de 5 à 6 mois. Afin de pouvoir observer l'impact de cette intervention, nous avons effectué une analyse de la phonologie en début et en fin d'étude ainsi que différents recueils de corpus de langage spontané. Les comptines nous ont permis de travailler et de jouer avec les sonorités de la langue, de façon ludique, portés par leurs rythmes et leurs mélodies. Les résultats nous ont permis d'observer un impact sur la phonologie. En ce qui concerne le bégaiement, le temps de prise en charge étant trop court, il est plus difficile de l'évaluer et de l'analyser. Il nous a néanmoins semblé que les comptines, loin de toute tension, offraient à l'enfant le plaisir de jouer avec les mots et les sonorités.

Book excerpt: Although speech-language pathologists are expected to be able to administer and interpret oral examinations, there are currently no screening tests available that provide careful administration instructions and data for intra-examiner and inter-examiner reliability. The Oral Speech Mechanism Screening Examination (OSMSE) is designed primarily for use by clinical speech-language pathologists. The examination could also serve, however, as a useful tool for oral myofunctional therapists, as well as physical therapists, dentists, and orthodontists interested specifically in speech. It is reliable, relatively easy and quick to administer, and appropriate for children and adults in either diagnostic or therapy settings. The OSMSE is intended to assess those anatomical structures and physiological functions that are most often considered to be potentially related to speech or language disorders. Structural and/or functional judgments included are organized on the OSMSE test form under categories of: lips, tongue, jaw, teeth, hard palate, soft palate, pharynx, breathing, and diadochokinesis. Results of a preliminary study with normal speaking subjects indicated that the OSMSE had satisfactory intra- and inter-examiner reliability after minimal training. The time required to administer the examination typically ranged from 5 to 10 minutes. (Author/GK)
