Where safety and quality were once considered to be self-evident, these issues need to be made more explicit in modern surgery. Doctors and the general public alike are nowadays more aware of the variations in quality that exist among surgeons and institutions and of the unwarranted harmful effects surgery can have. Super- or sub-specialization seems one way to improve both its quality and safety. Surgical Gastroenterology or Gastrointestinal Surgery is one of these new superspecialties. The path towards this specialization is not easy and turf battles between General Surgery and Surgical Gastroenterology are still being fought. Professor Kaushik recounts the history and development of this superspecialty in India. He recounts that although the history of Surgical Gastroenterology in this country is short, its future is bright but, as has happened in other countries, its further division into organ-based superspecialties can be expected. ‘Practice makes one perfect’ is the motto that is probably true for the high-risk surgery which is still being performed in small institutions or by surgeons with only little expertise. The inverse correlation between volume and outcome expressed as mortality and morbidity has been shown all over the world. Wouters et al systematically analyze the literature covering this topic. They also show that concentration and regionalization do not automatically lead to an improved quality of surgery, since other issues for quality assurance are of equal importance, such as adherence to evidence-based guidelines, feedback, and monitoring. Despite all the inputs in training and instruction by experts, a uniformly high quality of surgery is hard to achieve in day-to-day gastrointestinal surgery. Sudhir Joseph and his colleagues show that a dedicated checklist can save lives and reduce postoperative morbidity in many countries with different systems and levels of healthcare. A similar beneficial effect was found in The Netherlands using a more comprehensive checklist that covers the whole period of the hospital stay. The new type of surgeon is aware of the principles of evidence-based medicine and he/she will challenge the dogmas of the ‘old’ surgery. Over the years our surgical masters have told us about the importance of good pre-operative and postoperative care for our patients. The bowel should be put at rest before operation and oral feeding restored only slowly after surgery starting with nothing by mouth to sips of clear water. Bed rest was essential for recovery. He introduced the ERAS (Enhanced Recovery after Surgery) strategy also named Fast Track Surgery. Mitchell et al provide ample evidence that all surgeons should adopt the ERAS strategy. Implementation of this will lead to better and more cost-effective operations.

Although it has fairly typical presenting clinical features in the form of an elderly patient with dysphagia; the subsequent investigations have to be fine-tuned to select which patient should undergo esophageal resection (a formidable procedure which carries a high mortality risk and is associated with serious complications), who should undergo neoadjuvant therapy and who should only be palliated with stents or chemoradiotherapy. In recent years this fine-tuning has become much more precise with many patients being saved from harmful explorations of the chest and abdomen only to encounter widespread disease and at the same time many who have large, locally invasive tumors which were thought to be unresectable being given neoadjuvant chemotherapy and subsequently being able, sometimes, to undergo curative esophagectomy. Possibly because of these improvements in diagnosis and multimodality treatment, the 5-year survival of patients with esophageal cancer that has been resected has improved from a dismal 5%, 15 years ago to a more respectable 25% now. In this book, authors, two thirds of whom are from the Memorial Sloan-Kettering Cancer Center (MSKCC) in New York, have evaluated the published evidence on different aspects of the management of patients with esophageal cancer as well as made recommendations from their own experience with the disease. Thus, we have chapters on diagnostic and therapeutic endoscopy, staging, perioperative therapy, operative treatment (transhiatal, radical, Ivor Lewis and the minimally invasive approaches) and postoperative complications. Finally there is a chapter discussing the epidemiology and difficulties in the management of patients with esophageal cancer in India where the disease is common and mainly affects poor people who cannot afford expensive treatment. The author mainly discusses the experience of the Tata Memorial Hospital in Mumbai where every year 1200 patients are seen with the disease and 180 undergo surgery. This is amongst the largest experience of the treatment of patients with esophageal cancer worldwide. All the subjects and authors have been brought together for this volume by the Editor, Manjit Singh Bains, who is a Professor of Clinical Cardiothoracic Surgery at the Cornell Weill Medical College in New York as well as an Attending Thoracic Surgeon at the MSKCC.

Book excerpt: Liver Transplantation (LT), until recently the ‘forbidden fruit’, is the newest, the sexiest, the most controversial and arguably, the most technically challenging subspecialty of Surgical Gastroenterology to have mushroomed in India. In a journey spanning 12 years, 28 centers and 1500 liver transplants, we imagined there would be a gripping story to tell. It was only appropriate then, that we chose to inaugurate the series with an overview of Liver Transplantation. In the last 2 decades, the field has seen rapid progress with 1- and 5-year patient survival improving from 80% and 50% to 90% and 80%, respectively, owing to technical refinement, and better immunosuppressants, intensive care, and patient monitoring. Despite being a relatively new entrant into the field, India can now boast of at least some islands of excellence that have caught up with the "best in the west". This means the evolution of LT in these centers has been fast-forwarded up a steep curve. We have come a long way from the point of every LT being considered an experimental procedure with much media and public hype to one where it is accepted as a successful and durable panacea for all liver failure. The popularity of teams running successful programs in India has swung from being ostracized when the chips were down, to being celebrated and envied when the going became good. The focus has shifted from immediate to long-term survival, from surgical heroism to building multidisciplinary teams, from anecdotal to hard data and from media reports to scientific publications. ‘Fly-by night’ operations are on the decline as realization of their futility dawns on new centers and transplant teams. They are now happier to take the longer route of developing trained in-house teams. After spending a good part of the last decade honing the technique of living donor liver transplantation, we are becoming more sensitive to the morbidity and mortality risks to the living liver donor. We want to minimize the liver volume removed from the donor, and want to develop techniques of minimal access. More importantly, we want to train our guns back on pushing donation after brain or cardiac death. We have begun to maintain databases, are getting into audit mode and want a National registry. We want standardized treatment guidelines and training curricula. We want to take part in multicenter trials and further meaningful clinical and laboratory research. As a community, the liver transplant doctors of India are on a warpath of progress. We want it all and we want it now! The first step to the march into the future is to size up the past and the present. That is what we have attempted to do in this volume.

Book excerpt: Acute pancreatitis is defined as an acute inflammatory process of the pancreas that may also involve peripancreatic tissues and/or remote organ systems. It typically presents with sudden deep, boring pain that starts in the epigastrium and radiates to the back, which usually worsens on intake of food. It may be of varying severity ranging from mild to severe. Mild disease, if managed promptly and adequately resolves with minimal or no sequelae and seldom leads to extended morbidity or mortality. However Severe Acute Pancreatitis has a longer course of resolution, usually requires hospitalization and has a greater propensity to lead to adverse outcomes. As a thumb rule, it has greater morbidity and mortality rates as compared to Mild Acute Pancreatitis. Understandably, the management protocol for the severe acute pancreatitis is more invasive and emergent, and may require intensive care management. Hence understanding and assessment of the condition in the first 48 hours is a critical step in deciding the outcome. The various assessment criteria that are used to prognosticate and plan the management for this disease have been discussed in this clinical update. Previously, the condition was thought to be caused by infection, but now it is an established fact that it is an acute inflammation of the pancreatic gland with activation of the pancreatic enzymes within the gland leading to its autodigestion. Previously, acute pancreatitis was considered to be mostly associated with chronic alcohol abuse or acute bouts of binge drinking. With the availability of data from recent studies in India and the Asian subcontinent, Biliary disease is emerging as the commonest etiological agent. Extensive genetic analysis and mapping have helped put a cause to the erstwhile "idiopathic" cases. Newer diagnostic modalities and minimal invasive procedures have made the conventional surgeries nearly obsolete now. The rationale and use of antimicrobial treatment in the medical management of the condition has also undergone a change. This clinical update has been designed to update the readers on the important aspects of Acute Pancreatitis. The book has stressed upon various aspects of the condition like the etiology, pathogenesis, diagnosis and evaluation along with an insight into the management approach of the patient including the supportive and nutritional management. Overall, the book presents to the readers an excellent compilation of clinically applicable literature sourced from the most acclaimed physicians in the country.
