Book excerpt: Pursuant to a congressional request, GAO provided information on: (1) certain recommendations the Blue Ribbon Panel on Sizing Department of Defense (DOD) Medical Treatment Facilities made; (2) the need for a replacement facility for the Brooke Army Medical Center; (3) staffing at the Wilford Hall Air Force Medical Center; (4) the possibility of closing Brooke and jointly staffing Wilford Hall with Army and Air Force personnel; and (5) the DOD decision not to replace a hospital at Malmstrom Air Force Base. GAO found that: (1) from 1983 to 1985, the average daily patient load and average length of stay increased at Brooke and decreased at Wilford Hall; (2) the Air Force's nursing staff projections for Wilford Hall are not consistent with its staffing guidelines; and (3) the Navy is developing a plan for joint Army and Air Force staffing of a teaching medical facility at Wilford Hall. GAO also found that: (1) the Panel recommended that DOD examine the issue of whether Brooke is essential to its medical readiness before it decides whether to close or replace Brooke; (2) if DOD closed Brooke, there would still be enough hospital beds in the area to provide care to DOD beneficiaries; and (3) closing Brooke would be more cost-effective than replacing it, since the care it provides could be provided by Wilford Hall and area civilian hospitals. In addition, GAO found that the Air Force reasonably decided to replace the hospital at Malmstrom with an ambulatory care clinic because: (1) the military mission at Malmstrom does not require direct support from a military hospital; and (2) there are adequate local civilian health care facilities.

Book excerpt: The Portsmouth Naval Medical Center is a teaching hospital that provides comprehensive health care services to active duty forces and, when space is available, provides medical services to other DOD beneficiaries (i.e., dependents of active duty members and retirees and their dependents) in the Norfolk, Virginia, area. When space is not available, beneficiaries receive health care in civilian hospitals and clinics under DOD health plans. Currently, Portsmouth provides medical care primarily in two hospitals (building 1 and building 215) and seven outpatient clinics located throughout the Norfolk area. The two buildings have a total of 348 inpatient beds and about 700,000 gross square feet of facility space. Building 1 is used primarily for psychiatric, pediatric, and obstetrics and gynecological services. Building 215 contains about 500,000 gross square feet of space on floors 1 through 15; floors 16 through 18 are mechanical support spaces and therefore are not available for occupancy. Building 215 is currently used to provide a range of inpatient and outpatient services. Appendix I shows the Portsmouth Naval Medical Center, as of August 1996.

The healthcare facility inventory is aging and requires extensive facility management, renovations and replacement construction to maintain the environment of a high quality of care. Recent developments in sustainability and evidence-based design (EBD) have created additional requirements for the design and construction of facilities. These areas of design emphasis, sustainability and EBD, are developing best practices according to the respective theory. Existing facilities in the MHS have been directed to undergo restoration and modernization by Department of Defense (DoD) civilian leadership. Governmental directives have mandated that these restorations and new construction complies with current building codes and that EBD design features be included in healthcare facility planning. The hospital building type has changed over history due to innovations in building technology, such as HVAC and steel frame construction, as well as healthcare initiatives, such as treatment of tuberculosis and the mentally ill. The design concepts of environmental sustainability and EBD are currently altering the hospital typology. Building professionals have found significant differences in facilities that are categorized as evidence-based and those noted as sustainable. The future of successful healthcare designs relies on a balance of these concepts. The hospital building type has one of the highest energy intensities out of all commercial building types. Hospitals have become more energy intense due to the evolution of the deep-plan hospital. The design of the building envelope is the most lasting feature affecting the energy use of a hospital, due to its service-life typically equaling the life of the facility. The building envelope design consists of the shape of the building, material selection, as well as its orientation. This research demonstrates the relationship between incorporating positive building occupant features, such as increased day lighting and views of nature, and efficient energy design choices. The use of energy simulation software and early design collaboration between multiple professional disciplines is recognized as critical to optimal design solutions.
