Book excerpt: In 2005, I was arrested by agents from both the US Postal Service and the Food and Drug Administration for the importation of illegal human growth hormone and botulinum toxin (Botox) from China. At the time of my arrest, I was a thirty-seven year old Harvard graduate with medical and post-doctoral degrees. I attended one of the finest residency and fellowship training programs in the world at the University of California, San Francisco. I played two sports in college, earned awards at every level of education and training, had wonderful friends and a beautiful three-year-old daughter. Having grown up the son of a restaurant manager and a housewife, I had transcended the humble beginnings of a small Midwestern town to become the quintessential American Dream.Or so I thought. But with my arrest on felony importation charges, everything I had worked so hard for was swept away and the entire trajectory of my life was indelibly altered. I would embark on a three year battle not only for my medical license, but also for my freedom. This journey would lead to intense personal introspection, and in that process, I would discover with ugliness, there was also beauty, and with punishment, mercy. There are many reasons I have written this manuscript, with one of the most important being that I hoped my story would resonate with others who have gone through difficult circumstances as a consequence of a dark side of their personality. With this book, I hope to inspire others to accept and embrace the good and bad, while continually striving for improved self-understanding and acceptance.I have changed names primarily for legal purposes, but the facts are unchanged. Although the events described in the book occurred more than ten years ago, I think about them nearly every day. The shame and humiliation are ever-present. Any simple Google search of my name reveals the truth, and that truth has affected me over and over, despite the years, as it probably should. As the judge told me that day in a federal courtroom, "You have betrayed the public's trust." This is my confessional.

Available in PDF, EPUB and Kindle. Book excerpt: Unnecessary death rarely happens at the hands of doctors, but it does happen. Sometimes the cause is medical error. But sometimes the cause is politics. The issues underlying many medical catastrophes are numerous: a power struggle between providers, uncertainty over who’s in charge, hesitation to practice good medicine for fear of being fired, specialization run amok, part-time doctoring. Doctors often prefer to ignore the problems, but patient safety demands that they be aired. And so does the future of the medical profession. Beneath the politics lies confusion: Doctors no longer know who they are. They don’t know how much authority they should wield. They don’t know what distinguishes them from other healthcare professionals. They don’t what about being a doctor should make them proud. When doctors lack a firm sense of who they are, the whole of medicine lacks an essential core, giving rise to personal and professional politics—and catastrophes. Patients may be relying on a system that has veered off course. In dramatic and revealing stories of patients in the operating room and interactions with colleagues, Ronald W. Dworkin traces his path from medical school to anesthesiology residency to his early years in private practice, with the experiences of his father and grandfather, also doctors, hovering overhead, in his quest to answer the question: What is a Doctor? Sometimes funny, sometimes scary, sometimes poignant, the story of what it means to be a doctor in today’s medical setting comes to life, as Dworkin outlines the contours, the challenges and rewards, of modern medicine, and how it must be rescued in order to preserve the profession and protect patients from disasters.

In his first book, Confessions of a Surgeon: The Good, the Bad, and the Complicated he pushed open the operating room doors to give the public a startling view of what really went on inside the operating room. In Confessions of a Surgeon: A Deeper Cut, Dr. Ruggieri blows the operating room doors right off their hinges. It cuts deeper into a profession, even more mysterious then ever before. He candidly shares his thoughts on the patients that have impacted his life the most. He also exposes how surgeons (including himself) and the surgical profession have dramatically changed since the first time he nervously picked up a scalpel blade as a naïve surgical intern. He explores how these changes have helped and hurt patients. He also explores how these changes will continue to have a direct affect on anyone about to enter an operating room. Ultimately, Dr. Ruggieri’s passionate and candid account of his life inside a changing operating room will give his audience the power of transparency and truth.

Book excerpt: “A volume brimming with humanitarian lessons in medicine and life alike.” —Kirkus Reviews "A generous, compassionate book about what it is to be human and what it is to care. Paul Seward writes in language so clear and compelling you can see straight through it and into the beating heart beneath." —Kate Cole–Adams, author of Anesthesia Drawing on a career launched in the first days of the specialty of emergency medicine, Dr. Paul Seward takes the reader with him into the ER in his riveting memoir. Told in fast–paced, stand–alone chapters that recall unforgettable medical cases, Patient Care offers the fascination of medical mysteries, wrapped in the drama of living and dying. A snap judgment about a child nearly kills him, and a priest who may be having a heart attack refuses treatment. An asthmatic man develops air bubbles in his shoulders, and a pharmacist is haunted by a decision he makes. But the book goes beyond these stories. Each chapter explores ethical questions that remind us of the full humanity of patients, nurses, coroners, pharmacists, and, of course, doctors. How do they care for strangers in their moments of crisis? How do they care for themselves? Dr. Seward rejects doctor–as–God narratives to write frankly about moments of failure, and champions the role of his colleagues in health care. And, for all the moral dilemmas here, there is plenty of wit and humor, too. (See the patient who punches our doctor.) Readers of Patient Care will find themselves thinking along with Dr. Seward: “What is the right thing to do? What would I do?”

CHAPTER VIII For the nonce I will take leave of those tangled and difficult problems, which I am at a loss to solve, and in the face of which I must confess utter helplessness. I will now occupy myself with a question to which but one answer is possible, and that a perfectly straight one. It deals with gross and entirely conscious disregard for that consideration which is due to the human being. I approach the subject with regret, but it is impossible to pass it by. "A certain Dr Koch," we read in the Russian medical paper, Physician, "has published a pamphlet, entitled, ' Aerztliche Versuche an labenden Menschen' (' Medical Experiments on Living Man '), than which nothing were better calculated to further undermine the respect for, and confidence of the laity in, our profession. The author essays to prove that 'vivisection has long crossed the thresholds of our hospitals'--in other words, that experiments similar to those conducted upon the lower animals in the laboratory, are practised on living man in our infirmaries. As might have been expected, Koch's book was immediately seized upon by different feuilleton writers and newspapers chroniclers. It were highly desirable that our German colleagues should not leave a single one of Dr Koch's 'facts' without searching inquiry and explanation, as it is only possible by this means to nullify the effect of his book." (See The Physician, 1893, P-906.) 103 I have not read the above-mentioned pamphlet, and do not know how far the " facts" mentioned by Dr Koch merit the ironical inverted commas which the editor of the Physician had seen fit to place them in. But unfortunately there is much substantial truth even in the title of Dr Koch's booklet alone. In proof of the above it would be easy enough...

The writing was mostly for family, but at this time I have been encouraged to publish it. The most of the material is from memory, but augmented by letters written over a 35 year period, which my mother had saved. I was a letter writer from the time I left home at age 15 until my mothers death. I was not aware she had saved them all until they were discovered after her death in a closet. My mother and grandmother were instrumental in my success by their persistent prayers in my behalf. From my perspective, coincidence, does not answer many details of my story as well as divine intervention. Windows seemed to open in reasonable times when doors were closed. The reader can make up his or her own mind.

Book excerpt: The memoir of a high-functioning, law-abiding (well, mostly) sociopath and a roadmap—right from the source—for dealing with the sociopath in your life. “[A] gripping and important book . . . revelatory . . . quite the memorable roller coaster ride.”—The New York Times Book Review As M.E. Thomas says of her fellow sociopaths, “We are your neighbors, your coworkers, and quite possibly the people closest to you: lovers, family, friends. Our risk-seeking behavior and general fearlessness are thrilling, our glibness and charm alluring. Our often quick wit and outside-the-box thinking make us appear intelligent—even brilliant. We climb the corporate ladder faster than the rest, and appear to have limitless self-confidence. Who are we? We are highly successful, noncriminal sociopaths and we comprise 4 percent of the American population.” Confessions of a Sociopath—part confessional memoir, part primer for the curious—takes readers on a journey into the mind of a sociopath, revealing what makes them tick while debunking myths about sociopathy and offering a road map for dealing with the sociopaths in your life. M. E. Thomas draws from her own experiences as a diagnosed sociopath; her popular blog, Sociopathworld; and scientific literature to unveil for the very first time these men and women who are “hiding in plain sight.”

Book excerpt: A New York Times Editors’ Choice: “A mind-bending romp through a gender-fluid, eighteenth century London . . . a joyous mash-up of literary genres shot through with queer theory and awash in sex, crime, and revolution.” NAMED ONE OF THE BEST BOOKS OF THE YEAR BY The New Yorker • HuffPost • Kirkus Reviews • Finalist for the Lambda Literary Award • Shortlisted for the Center for Fiction First Novel Prize • “A dazzling tale of queer romance and resistance.”—Time Jack Sheppard and Edgeworth Bess were the most notorious thieves, jailbreakers, and lovers of eighteenth-century London. Yet no one knows the true story; their confessions have never been found. Until now. Reeling from heartbreak, a scholar named Dr. Voth discovers a long-lost manuscript—a gender-defying exposé of Jack and Bess’s adventures. Is Confessions of the Fox an authentic autobiography or a hoax? As Dr. Voth is drawn deeper into Jack and Bess’s tale of underworld resistance and gender transformation, it becomes clear that their fates are intertwined—and only a miracle will save them all. Writing with the narrative mastery of Sarah Waters and the playful imagination of Nabokov, Jordy Rosenberg is an audacious storyteller of extraordinary talent. Praise for Confessions of the Fox “A cunning metafiction of vulpine versatility . . . an action-adventure tale with postmodern flourishes; an academic comedy spliced with period erotica; an intimate meditation on belonging.”—Katy Waldman, The New Yorker “Confessions of the Fox is so goddamned good. Reading it was like an out-of-body experience. I want to run through the streets screaming about it. It should be in the personal canon of every queer and non-cis person. Read it.”—Carmen Maria Machado, National Book Award finalist for Her Body and Other Parties “A hat tip to Moby-Dick . . . a running footnote hall of mirrors to rival Borges . . . one of the most trenchant calls for progressive action that I have read in a very long time.”—The New York Times Book Review “An ambitious work of metafiction, a sexy queer love story . . . a bold first novel.”—Entertainment Weekly
