The clinical information system (or CIS) is an automated system with a long term database containing clinical information used for patient care. This definition excludes business systems (no clinical data), physiological monitoring systems (no long term database), and many research systems (not used in patient care). The theses of this book are (a) that CIS technology is mature, (b) that the CIS will have a major impact upon patient care and the health delivery system, and (c) that the number of commercial systems which now offer these potential benefits is very small. The objective of this book is to establish the above theses and thereby (a) inform both users and developers, (b) increase the demand for more sophisticated products, and finally, (c) provide marketplace incentives to advance the state of the art. The CIS is an application of computer technology for a specific class of problems. Its development requires a knowledge of the technology with an understanding of the application area. As with any tool-based application, the scope of the product will be limited by the capability of the tool. In the case of the CIS, reliable computers with comprehensive database facilities became com mercially available in the early 1970s. By the mid 1970s there was a maturation of the literature, and evaluations of 5-years' use began to appear. As will be shown, there have been surprisingly few new ideas introduced since the 1970s.

A registry database is a file (or files) derived from the registry. Although registries can serve many purposes, this guide focuses on registries created for one or more of the following purposes: to describe the natural history of disease, to determine clinical effectiveness or cost-effectiveness of health care products and services, to measure or monitor safety and harm, and/or to measure quality of care. Registries are classified according to how their populations are defined. For example, product registries include patients who have been exposed to biopharmaceutical products or medical devices. Health services registries consist of patients who have had a common procedure, clinical encounter, or hospitalization. Disease or condition registries are defined by patients having the same diagnosis, such as cystic fibrosis or heart failure. The User’s Guide was created by researchers affiliated with AHRQ’s Effective Health Care Program, particularly those who participated in AHRQ’s DEcIDE (Developing Evidence to Inform Decisions About Effectiveness) program. It should be possible for the data to be easily entered into the database-structure and afterwards also easily read, corrected and processed. Query languages, such as SQL (Structured Query Language) developed especially for these purposes, make databases easily accessible, also to researchers who may not be very well versed in computer programming. The cardiological/medical clinician and researcher of today is of necessity confronted more and more with computer-based data storage. For many practicing physicians, questions such as "What is informatics?" and "Why is informatics important in medicine?" are becoming commonplace. Further, once these basics are understood, many physicians seek more complete information about this new "basic science. " The goal of this book is to provide primary care physicians with a practical in troductory understanding of medical informatics, focusing on areas of importance in primary care. Additionally, we seek to present clinical contexts in which some of the various applications of medical informatics can be applied. The book begins with an overview of medical informatics, based on the inter action (interface) between the patient and the primary care physician. Next, we study how this interaction can be documented with electronic medical records, and how information on laboratory data and imaging, originating from other elec tronic sources, can be integrated into the electronic medical record. We then cover several areas that concern the content of the information used in primary care. Areas of focus include evidence-based medicine, decision support, knowledge re sources, and patient education. Finally, this book concludes with five chapters concerning practical aspects of primary care informatics: workflow, privacy and security, electronic billing, reporting and analysis, and telecommunications.
