Book excerpt: Approximately 697,000 U.S. service members were deployed to the Persian Gulf in 1990/1991 for Operations Desert Shield/Storm (ODS/S). The vast majority of troops returned from this large deployment healthy. In response to Gulf War veterans' concerns about the potential health effects of service in ODS/S, the Departments of Defense (DoD) and Veterans Affairs (VA) developed similar, clinical evaluation programs to provide them care and to understand the nature of their illnesses. The DoD Comprehensive Clinical Evaluation Program (CCEP) provides a systematic, in-depth medical evaluation for DoD beneficiaries (Persian Gulf War veterans now on active duty or retired; members of the full-time National Guard who are Persian Gulf veterans; Persian Gulf War veterans who are members of the Ready Reserve/Individual Ready Reserve/Standby Reserve/Reserve who are placed on orders by their units; and eligible family members of such personnel) who are experiencing illnesses that may be related to their service in the Persian Gulf. As of early December 1995, more than 27,000 individuals had enrolled in the program. Approximately 21,000 of these participants requested an examination of which 18,598 had completed the evaluation process and had the information about their health verified and entered into the CCEP database.

Book excerpt: This study examined the effects of the program change released in January 1995 on the efficiency of the inpatient Comprehensive Clinical Evaluation Program, Phases II and III, at Brooke Army Medical Center between October 1994 and February 1995. Attention was focused on lengths of stay, numbers of consults, and average waiting times for consults. A random sample of Post Gulf War veterans who had completed the program were chosen. Group 1 consisted of 21 patients admitted prior to the program change; Group 2, a sample of 15, were admitted after. Pearson r correlations were used to measure the correlation between variables within the groups. The correlation between length of stay and number of consults for Groups 1 and 2 were 21 and 57 percent, respectively. Analysis-of-variance was used to compare differences between group means. Mean length of stay was significantly shorter and numbers of consults significantly fewer in Group 2. These findings support the argument that medical protocols have an adverse impact on physicians' ability to manage patients on a case-by-case basis, and may cause an increase in lengths of stay, numbers of consults, and hospital costs. These data may help providers and administrators plan clinical protocols focused on case management.

Since their return from service in the Persian Gulf region during Operations Desert Shield and Desert Storm, some active-duty military personnel and veterans have reported a variety of health problems that they perceived to be associated with their service in that region. The DoD instituted the CCEP in June 1994 to evaluate and treat the health problems of these active-duty personnel. The DoD then asked the IOM committee to evaluate the protocol for the clinical evaluations and to comment on the interpretation of the CCEP results that have been obtained so far. In addition, the committee was asked to make recommendations relevant to the conduct of the clinical evaluations in the future and to the broader program of the DoD Persian Gulf health studies, if appropriate. The purpose of this report on the CCEP is to provide a comprehensive evaluation of the major issues that the committee has identified since its first meeting in October 1994.

Various types of research and health measurement are needed to address these diverse issues. The Institute of Medicine (IOM) was asked by the Department of Veterans Affairs (VA) and the Department of Defense (DoD) to undertake a study to identify important questions concerning the health of Gulf War veterans and then to design a study to answer those questions. The committee determined that it is of fundamental importance to ask how healthy are Gulf War veterans? Are they as healthy as others? What characteristics are associated with differences between the health of Gulf War veterans and the health of others? To address these questions, it will be necessary to measure not only the health status of those who served in the Gulf War, but also to compare Gulf War veterans with other groups. Further, one must continue to follow these groups through time to determine whether the groups differ in the way their health status is changing. As the committee began to develop a design that would address the fundamental questions identified, it realized that such a study could have important implications for understanding not only the health of Gulf War veterans, but also the health of veterans of other conflicts.
