Book excerpt: Introduction : Le diabète gestationnel (DG), dont la prévalence en France est estimée à 6.4%, est un véritable problème de santé publique. Si les complications materno-fœtales (MF) et l’intérêt de son dépistage sont maintenant établies, le caractère ciblé ou systématique de son dépistage prête encore à controverse. L’objectif de cette étude était de comparer les complications MF chez les patientes porteuses d’un DG diagnostiqué par une HGPO 75g au 2e trimestre de grossesse, en fonction de la présence ou non de facteurs de risque (FDR). L’objectif secondaire était de réaliser une étude médico-économique sur le coût du dépistage et du suivi des patientes sans FDR. Matériel et méthode : Analyse rétrospective des patientes présentant un DG diagnostiqué sur une HGPO 75g réalisée de manière systématique au 2e trimestre de grossesse entre mai 2012 et février 2014 suivant les seuils des dernières recommandations. Les paramètres généraux, obstétricaux et néonataux étaient recueillis. Etaient comparées les complications MF entre les patientes avec ou sans FDR. Résultats : La prévalence du diabète gestationnel dans notre population est de 9.2%. 21.5% des patientes présentant un DG n’avaient pas de FDR. Le taux de césarienne dans notre population est de 22.2% (taux de césarienne sur le CHU à la même période : 21%). Il n’a pas été mis en évidence de différence significative en terme de complications MF entre les patientes avec ou sans FDR : césarienne 22.2 vs 31.9% (p=0.32), macrosomie ≥ 90ep 9.7 vs 6.5% (p=0.73), hypoglycémie néonatale 7.8 vs 0.8% (p=021), bien que les pourcentages soient plus élevés dans le groupe avec FDR. L’étude médico-économique retrouve un coût de 378 euros par patiente dépistée dans le groupe sans FDR. Discussion : Nos résultats semblent en faveur du dépistage systématique. Plus d’une patiente sur cinq ne présentait pas de facteur de risque, et le coût estimé du dépistage et du suivi paraît faible au regard des complication s évitées. Une étude médico-économique plus poussée et une comparaison avec des patientes non diabétiques nous parait souhaitable. Un suivi plus allégé des patientes sans facteur de risque pourrait être une piste à étudier
