Dans leur qualité́ d'aidants, ces derniers sont les premiers à faire face à ces troubles et le médecin traitant est très souvent le premier professionnel de santé chargé de dépister et de prendre en charge ces troubles. L'objectif de ce travail de thèse était de comparer la perception et l'état des connaissances des aidants et des médecins traitants sur les troubles neuro cognitifs des patients, à travers une étude comparative observationnelle entre deux populations différentes (Guadeloupe / France métropolitaine). Chacun des aidants des deux groupes ont été analysés, en prenant en compte leurs différences sociodémographiques et le fardeau de l'aidant afin de mettre en évidence ou pas une causalité direct ou indirecte sur la perception des troubles cognitifs de leurs proches. De même, nous avons interrogé dans la mesure du possible, les médecins traitants de chaque patient, afin de recueillir leur perception face à ses troubles. Soixante et onze questionnaires ont été recueillis pour l'ensemble des aidants des deux populations, dont cinquante trois en France métropolitaine et dix huit en Guadeloupe. Dix huit médecins traitants ont répondu à notre questionnaire par téléphone ou retour de mail. Selon notre étude, les troubles cognitifs sont perçus par 77,5 % des aidants, sans différence entre les deux groupes, avec une gravité de ses troubles cotées en moyenne à 2,65. Trente trois pour cent des médecins traitants ont identifié une atteinte cognitive chez leur patient, sans différence significative entre les deux groupes. La majorité des médecins traitants, soit 88,9% ne pense pas que ces troubles cognitifs soient liés à l'âge, et 50 % d'entre eux estiment qu'ils sont liés à une pathologie. Bien que l'effectif cible ne soit pas atteint aucune différence socio culturelle n'est retenue au sein de notre étude dans le cadre de la perception des troubles cognitifs, hormis que les troubles soient plus souvent attribués à l'âge en métropole quand ils sont reconnus. La stigmatisation des patients atteints de ses troubles, l'absence de thérapeutique et le manque de structure adaptée à l'accueil de ces patients mettent un frein à la reconnaissance de ces troubles. L'enjeu à venir pourrait donc être de sensibiliser les aidants principaux ainsi que les médecins traitants, dans le diagnostic et la prise en charge précoce avant le potentiel déclin cognitif majeur et la perte d'autonomie.

Selon les dernières données de la littérature, les démences touchent entre 6 et 8% des individus de plus de 65 ans. Elles sont le plus souvent neuro-dégénératives et la plus fréquente d’entre elles est la maladie d’Alzheimer. Bien qu’il n’existe aucun traitement médicamenteux des troubles cognitifs, une prise en charge précoce des patients atteints de troubles cognitifs, permet une meilleure prise en charge de ces derniers, en permettant de faciliter l’accès à de nombreuses interventions : pharmacologique ou non, éducation, discussion autour du projet de vie et la mise en place de directives anticipées. Les aidants naturels, c’est-à-dire les personnes pourvoyant de façon informelle et régulière aux besoins du malade, pourraient contribuer à un diagnostic plus précoce. Cependant le délai de reconnaissance des troubles cognitifs varie en fonction de l’aidant. Les facteurs influençant la reconnaissance des troubles cognitifs regroupent notamment : la reconnaissance des symptômes en tant que maladie, le niveau socio culturel de l’aidant, l’accès aux soins et à l’information, l’origine ethnique et la perception des troubles cognitifs. Il serait logique de penser que le lieu de vie modifie également cette perception. Cependant aucune étude n’a étudié la reconnaissance des troubles cognitifs par l’aidant en fonction des régions L’objectif principal de cette étude a pour but de comparer la prévalence de la reconnaissance des troubles cognitifs par l’aidant entre la Guadeloupe et la région parisienne. Les objectifs secondaires sont de savoir à quoi sont attribués les troubles cognitifs d’évaluer les facteurs influençant la reconnaissance, et d’évaluer le fardeau de l’aidant. Il s’agit d’une étude prospective, non interventionnelle, bi centrique. Au total, 71 aidants ont été recrutés pour l’étude. Le taux de reconnaissance des troubles cognitifs était de 77,5% (55), il n’y avait pas de différence significative entre les aidants de Guadeloupe et de Métropole, mais le nombre total de sujets à inclure n’a pas été atteint. Les aidants de Métropole avançaient comme principale origine des troubles cognitifs l’âge, contrairement aux aidants de Guadeloupe qui les considéraient plus souvent comme secondaires à une maladie. Les aidants de Guadeloupe considéraient les troubles cognitifs comme moins sévères alors que les patients de Guadeloupe étaient plus atteints. Les données de notre étude montrent une reconnaissance des troubles cognitifs encore insuffisante, une tolérance des troubles cognitifs plus importante en Guadeloupe, et une assimilation des troubles cognitifs a l’âge encore trop fréquent. L’ensemble de ces facteurs contribue à une mauvaise prise en charge des patients atteints de troubles cognitifs.

La reconnaissance des troubles neurocognitifs était appréciée par un questionnaire rempli par l’aidant principal. Le patient subissait des tests pour apprécier son état cognitif. Le médecin traitant était interrogé sur sa perception des troubles. Nous présentons des résultats préliminaires concernant 36 dyades patients-aidants. On ne retrouve pas de différence significative entre la reconnaissance des troubles neurocognitifs chez les aidants de Guadeloupe et de la région parisienne (72.2% contre 83.3% p=0.69). Il n’y avait pas de différence significative dans la reconnaissance des troubles entre les deux groupes malgré un taux de non diplômés plus élevé chez les aidants de Guadeloupe (38.9% contre 5.56%; p=0.05). Par ailleurs les aidants de la région parisienne ont plus souvent attribué les troubles cognitifs de leurs proches à l’âge (77.8% contre 27.8% ; p=0.008) or le vieillissement cognitif normal n’inclut pas de troubles neurocognitifs. Les faiblesses du travail sont le petit nombre d’inclus et le biais de sélection car toutes les dyades sont issues du milieu hospitalier. Les forces du travail résident dans son caractère inédit et dans les tendances inattendues qui se dégagent des résultats préliminaires. Au vu des résultats préliminaires, cette étude peut potentiellement changer notre regard sur la perception des troubles cognitifs au Antilles comme en France Hexagonale.
