This new book, more than just a history of the UK’s supreme audit institution, examines the very definition of accountability through both an historic and an academic lens, critically exploring questions about the role of audit in a democracy and how well it is working. Holding Government to Account draws on several unique sources of evidence, including interviews with senior officials from the National Audit Office and the civil service, as well as senior parliamentarians with experience of the NAO’s relationships with government and legislature. These interviews are supplemented by an analysis of previously unpublished manuscript material in the National Archives, examination of NAO reports and parliamentary and other reports focused on accountability. The book begins with a history of the National Audit Office in the context of the UK’s wider history. It then offers an overview of the constitutional, political and human legacies of the Exchequer and Audit Department, followed by a close examination of the National Audit Office’s leadership and decision-making from inception in 1984 through to the present. The authors conclude with an exploration of the way in which the meaning of public sector audit has evolved over time, in accordance with its wider political, ideological and material context. In doing so, they demonstrate that any question about the National Audit Office’s future and organisation is really a question about what democracy and good government mean in a modern bureaucratic state. Book excerpt: The numbers of people seeking asylum in the UK peaked between 1999 and 2002, before almost halving in 2003. The high levels in applications led to a large backlog of cases awaiting an initial decision and subsequently an increased volume of appeals. The Immigration and Nationality Directorate spent £1.86 billion on its operations in 2002-03, including £1.07 billion in supporting asylum applicants. In the same year, the Immigration Appellate Authority spent £101 million on dealing with appeals from immigration and asylum cases. Following on from a NAO report on this topic (HCP 535, session 2003-04; ISBN 0102928495) published in June 2004, the Committees report examines the progress made in tackling the influx of asylum applications and in improving the timeliness and quality of decision-making. Findings include that the Home Office should balance additional administrative costs involved in increasing staff and infrastructure resourcing to meet surges in demand, against the extra costs arising if backlogs are allowed to accumulate; and fast-tracking procedures should be expanded, with a more demanding joint target set for the Directorate and Appellate Authority for time taken to process cases and reach a decision at appeal stage.

Recently they obtained authority to introduce the special measure of "reverse charge" for mobile phones and computer chips but this can only be a provisional solution. The problem can only be tackled at EU level and ultimately the European Union will have to agree a new legislative framework for administering VAT.

Book excerpt: The high speed railway linking London to the Channel Tunnel, known as High Speed 1, has now been fully open for almost five years and has had a good performance record. There have also been some costly mistakes. The Department originally expected London & Continental Railways Limited (LCR) to service the project debt from future revenues from Eurostar UK. However by the end of 1997 revenues were substantially below LCR's forecasts. Consequently, in 1998, the Department agreed to restructure the deal and guarantee most of LCR's debt. The taxpayer is now servicing and repaying the project debt of £4.8 billion. Passenger demand for international services on the line has been much lower than forecast and that is the root cause of the failure of the original deal. Planning assumptions failed to properly consider the impact on passenger numbers of the growth of low cost airlines and the competitive response of ferry companies. The Department still does not have plans in place to evaluate fully the impact of HS1. Total taxpayer support for the line, over a 60 year period to 2070, has an estimated present value of £10.2 billion. Benefits for passengers from shorter journey times over this period have an estimated present value of £7 billion. The basis of this cost/benefit analysis, however, is open to challenge. Also the Department will need to evaluate HS1's regeneration benefits and wider economic impacts worth many billions of pounds if the project is to demonstrate value for money. The Department must also learn the lessons as it develops its plans for HS2.

In 2001 the Department of Health anticipated that a growth in the NHS workforce would lead to a decline in the use of temporary staff but in spite of this intention temporary nurses still account for the same percentage of the nursing budget (around 9%). On the basis of a report by the Comptroller and Auditor General, the Committee has investigated the extent of the use of temporary nurses; whether the process is properly planned and managed; and whether there are safety and quality implications. One of the conclusions is that there has a lack of planning and it is only as a result of the problem with deficits that the NHS has taken a more co-ordinated approach

Book excerpt: In Britain, between 1980 and 1998, the number of people classified as obese tripled to 21per cent of women and 17 per cent of men. It is estimated that this costs the economy, as a whole, �2 billion and the NHS �0.5 billion in treatment. However the response of the NHS is patchy, with no national guidelines and only 28% of health authorities taking action to address the problem. There is little activity related to the management of obesity outside of general practice but only a small proportion of GPs follow a protocol. This report recommends that there should be strategies to reduce obesity and that the Department of Health should build on the plan in the National Service Framework on coronary heart disease and work with partners and professional bodies to clarify responsibilities. It should also work with the National Institute of Clinical excellence to disseminate information. The Department of Health should also lead a cross government strategy to promote the benefits of physical activity and there should be local targets to encourage cycling and walking. It should also work with the food industry to improve the balance of diet. Much of this work needs to be aimed at school children to promote a healthy lifestyle throughout life and guidance to schools on commercial sponsorship should be strengthened so that there is no conflict with messages on healthy eating.

Book excerpt: This report examines how lessons learnt from the introduction of ePassports will be incorporated into future projects; the cost of authenticating applicants' identities; passport fee trends; the measures being taken by the Identity and Passport Service (IPS) to hold down passport fees; and working with others to reduce costs and improve border security. An ePassport contains an electronic chip storing biographical data and a digital facial image of the passport holder. From 2009 new second generation ePassports will incorporate the fingerprints of the passport holder. Passport fees have risen ahead of inflation since September 2003 to fund ePassport technology and other projects intended to improve the security of the UK passport. From 2009 all passport applicants will have to attend in person to provide fingerprints for inclusion in second generation ePassports. The set-up cost of data collection, validation and storage necessary to introduce these changes will be substantial. During 2007 IPS has been introducing personal interviews at its 69 new interviewing offices for first time adult passport applicants. At least one of the 69 offices is intended to be within an hour's travel by public or private transport for 95% of the UK population (except in remote locations). But elderly and disabled people may still face difficulties in making the journey. With the introduction of second generation ePassports, all applicants will need to attend a local office to give their fingerprints. The long term durability of the chip embedded in the ePassport book is unproven.
