In more than one way there is cause for celebration. Today marks the third lustrum of the annual international sympo sium on blood transfusion, organized by the Rode Kruis Bloedbank Groningen-Drenthe. In my opinion it has been a remarkable initiative of the Bloedbank, to start organizing a scientific conference, as it did, in 1976. It meant, among others, that in case of success the initiative would grow to be an annual item on the international congress calen der. It also meant that a possible third lustrum would coincide with the celebration of the 950th anniversary of the City of Groningen. I am happy to note that the initiative has been successful: over the past fourteen years the Rode Kruis Bloedbank Groningen-Drenthe has succeeded in organizing an annual symposium on blood transfusion, with a different theme each year, and with an average attendance of 250 participants from some 26 countries world-wide. The platform created with the special formula of the symposium, for science and industry, has been well balanced and beneficial to both. Mr. Chairman, ladies and gentlemen, I like to compliment the organizers with the success that they have attained. Allow me to men tion the name of just one person, in whom I like to thank everyone who has been involved in creating the annual Bloedbank-symposia: my warm congratulations to Dr. Smit Sibinga and his staff.

Book excerpt: Hypercoagulability and bleeding are problems physicians frequently encounter when treating intensive care patients. The coagulation mechanism is a tightly controlled system. Its balance is disturbed in conditions such as septicemia, trauma, shock, and others. The dis turbances may be local or disseminated and the therapeutic options available are limited and nonspecific: limited since in most cir cumstances they may not affect the underlying disease and are si milarly independent of the pathogenic mechanism; nonspecific since they cannot be specifically targeted to the problem of local or sys temic disturbance of coagulation. Furthermore, increased and de creased coagulability may be present at the same time. Activation of coagulation may lead to fibrin deposition and decreased hemostatic capacity, as seen in disseminated intravascular coagulation. New diagnostic approaches for acquired disorders of coagulation have been developed and form a new basis for therapeutic strategies. Despite this progress, many physicians are aware of the problem that, especially for intensive care patients, a treatment very often has to be selected before diagnostic procedures can be completed. We also know that for many diseases rare occurrence or the hetero genity of the patient population make it quite difficult to conduct clinical studies which prove that a therapeutic regimen is efficacious. Thus, much discussion about coagulation disorders in critically ill patients is still necessary. Even so, it is important to provide guidelines for handling these problems.

In this chapter, the readers are asked to make logical conclusions based on the given data and/or statistical results. Moreover, there is also a chapter on “Practical Calculations in Transfusion Medicine, Apheresis, and Hemostasis to help consolidate all the necessary formulas commonly used in daily practice for easy reference. These chapters are unique to our book and will not be found in any other currently on the market. All of the questions in this book were originally created by the authors of each chapter. Each question can either be standalone or part of a case scenario representing challenge cases in Transfusion Medicine, Apheresis, and Hemostasis. These questions often represent both rare and common clinical scenarios that the authors have seen during their clinical practice. Each question is then followed by 5 possible answers, with only one being correct (or the best answer). After the question, there is a conceptual explanation followed by a more factual explanation of the right and wrong answers. We gave the individual authors the freedom to choose how they explained the wrong answer choices. Some authors chose to be more direct (e.g. Answer A is incorrect because...), while other authors chose a more conversational style (e.g. Human resources (answer A) includes staffing, selection, orientation, training, and competency assessment of employees). This format is designed to help the student linking the conceptual and factual knowledge together to form a solid foundation for use in clinical practice. At the end of each chapter, there is a list of articles and textbooks that will prove useful to the motivated student who wishes to become an expert in the field. Another special feature to our textbook is the presence of a pre-test and post-test, which are provided to help the readers with self-assessment. As stated above, the main focus of this book is to help the readers preparing for the ABP certification exam in Transfusion Medicine. However, due to the interdisciplinary nature of the field of Transfusion Medicine, Apheresis, and Hemostasis, we believe that this book is also beneficial to and can be used by all clinicians involved in the management of complex transfusion, apheresis, and hemostasis issues, such as hematologists, anesthesiologists, surgeons, and critical care physicians.
