Available in PDF, EPUB and Kindle. Book excerpt: The traditional end-points for clinical studies of lung diseases were based on functional parameters. Their value as surrogate markers for disease activity and progression has been increasingly questioned by scientists, carers, regulatory agencies and funding bodies. Novel tools and methods with regard to biomarkers and patient-reported outcomes have made these parameters emerge from their status as interesting secondary end-points and become potential primary outcomes for clinical trials. Nevertheless, their relevance and validity still needs to be proven. This issue of the European Respiratory Monograph describes the current status regarding end-points in all relevant areas of pulmonary medicine.

Book excerpt: Evidence-based Clinical Chinese Medicine: Vol. 1 Chronic Obstructive Pulmonary Disease provides a 'whole evidence' analysis of the Chinese medicine management of chronic obstructive pulmonary disease (COPD). Evidence from the classical Chinese medicine literature, contemporary clinical literature, and the outcomes of clinical trials and experimental studies are reviewed, analysed and synthesised. The data from all these sources are condensed to provide evidence-based statements which will inform clinical practice and guide future research. This book has been designed to be an easy reference at the point of care. During a patient consultation, Chinese medicine practitioners can refer to this book for guidance on which Chinese herbal medicine formulas, specific herbs, or acupuncture points, can best treat their patient, and be confident there is evidence which supports its use.Currently, Chinese medicine practitioners who develop a special interest in a particular health condition such as COPD have to consult a variety of sources to further their knowledge. Typically, they use the contemporary clinical literature to understand the theory, aetiology, pathogenesis and obtain expert opinions on the Chinese medicine management of COPD. They search the electronic literature to identify systematic reviews of clinical trials, if any exists, to obtain assessments of the current state of the clinical evidence for particular interventions. If they have the skills and resources, they may search the classical Chinese medicine literature for an historical perspective on treatments that have stood the test of time. This book provides all of this information for practitioners in one handy, easy to use reference. This allows practitioners to focus on their job of providing high quality healthcare, with the knowledge it is based on the best available evidence.

This book was released on 2011-08-26 with total page 200 pages. Available in PDF, EPUB and Kindle. Book excerpt: Chronic diseases are common and costly, yet they are also among the most preventable health problems. Comprehensive and accurate disease surveillance systems are needed to implement successful efforts which will reduce the burden of chronic diseases on the U.S. population. A number of sources of surveillance data-including population surveys, cohort studies, disease registries, administrative health data, and vital statistics-contribute critical information about chronic disease. But no central surveillance system provides the information needed to analyze how chronic disease impacts the U.S. population, to identify public health priorities, or to track the progress of preventive efforts. A Nationwide Framework for Surveillance of Cardiovascular and Chronic Lung Diseases outlines a conceptual framework for building a national chronic disease surveillance system focused primarily on cardiovascular and chronic lung diseases. This system should be capable of providing data on disparities in incidence and prevalence of the diseases by race, ethnicity, socioeconomic status, and geographic region, along with data on disease risk factors, clinical care delivery, and functional health outcomes. This coordinated surveillance system is needed to integrate and expand existing information across the multiple levels of decision making in order to generate actionable, timely knowledge for a range of stakeholders at the local, state or regional, and national levels. The recommendations presented in A Nationwide Framework for Surveillance of Cardiovascular and Chronic Lung Diseases focus on data collection, resource allocation, monitoring activities, and implementation. The report also recommends that systems evolve along with new knowledge about emerging risk factors, advancing technologies, and new understanding of the basis for disease. This report will inform decision-making among federal health agencies, especially the Department of Health and Human Services; public health and clinical practitioners; non-governmental organizations; and policy makers, among others.

First, the it focuses on both basic and clinical research on COPD, and the inflammatory mechanisms that function in these diseases. Second, it is unique with respect to scope of the discussion of the unusual characteristics of the immune response which occurs in these patients. Third, it includes knowledge being gained from translational research conducted through clinical trials at several Medical Schools in the United States. Not only is this research providing information about novel drugs and therapies, but it is also advancing our understanding of the genetics of these diseases. This work will illuminate the molecular basis for these diseases, and hopefully will permit us to individualize the therapies for these diseases.

Book excerpt: This book explains how analysis of the heterogeneity of chronic obstructive pulmonary disease (COPD) enhances understanding of the condition and leads to improved, personalized treatment. State of the art knowledge is presented on a range of issues related to the heterogeneity of COPD, such as phenotypes (clinical, physiologic, radiologic, etc.), genotypes, and the tools to be used for dissecting heterogeneity (CT, MRI, biomarkers, etc.). Especially modern radiologic imaging holds promise in this context, and its role is described in detail with the aid of numerous illustrations. The implications of the heterogeneity for personalized treatment are clearly identified, with description of an appropriate tailored treatment strategy for each subgroup of patients. Information is provided on both current and emerging strategies, including bronchoscopic lung volume reduction and approaches to the management of pulmonary hypertension and comorbidities. This book will be a great asset in clinical practice and research for all who have an interest in COPD, a leading cause of morbidity and mortality worldwide.

Defining patient involvement as patient participation in the HTA process and research into patient aspects, this book includes detailed explanations of approaches to participation and research, as well as case studies. Patient Involvement in HTA enables researchers, postgraduate students, HTA professionals and experts in the HTA community to study these complementary ways of taking account of patients’ knowledge, experiences, needs and preferences. Part I includes chapters discussing the ethical rationale, terminology, patient-based evidence, participation and patient input. Part II sets out methodology including: Qualitative Evidence Synthesis, Discrete Choice Experiments, Analytical Hierarchy Processes, Ethnographic Fieldwork, Deliberative Methods, Social Media Analysis, Patient-Reported Outcome Measures, patients as collaborative research partners and evaluation. Part III contains 15 case studies setting out current activities by HTA bodies on five continents, health technology developers and patient organisations. Each part includes discussion chapters from leading experts in patient involvement. A final chapter reflects on the need to clearly define the goals for patient involvement within the context of the HTA to identify the optimal approach. With cohesive contributions from more than 80 authors from a variety of disciplines around the globe, it is hoped this book will serve as a catalyst for collaboration to further develop patient involvement to improve HTA. "If you’re not involving patients, you're not doing HTA!" - Dr. Brian O’Rourke, President and CEO of CADTH, Chair of INAHTA

Available in PDF, EPUB and Kindle. Book excerpt: This book presents the current trends and state of the art solutions addressing various issues related to pulmonary disorders. Diagnostic and therapeutic challenges are tackled, starting with the noncommunicable diseases of sarcoidosis and granulomatosis with polyangiitis. Pulmonary involvement, practically unavoidable, runs an insidious course, and is often occulted by systemic symptoms. The establishment of a firm diagnosis, with a precision no one could oppugn, is difficult. Other issues pertain to quality of life, disease preventive measures, and the move toward personal health care in chronic sufferers from multiple conditions in later life. There is also an update on the prevalence and diagnostic and treatment challenges of extrapulmonary tuberculosis. In addition, the place of neuroproteomics in modern clinical practice is presented. The practical insights emphasize the role of science in advancing biomedical knowledge and care. The book is addressed to researchers and practitioners, and allied health care professionals engaged in effective patient care and therapy.
