Book excerpt: "He who is faithfully analysing ... epi lepsy is doing far more than studying epilepsy" Hughlings Jackson Modifying this well-known statement by Jackson, one could say today: "He who is faithfully analysing anti-epileptic drugs is doing far more than studying anti-epileptic drugs". For these drugs not only serve to prevent epileptic fits and thus advance the treatment of epilepsy, they are also effective in the treatment of cardiac arrhythmias and trigeminal neuralgia. Furthermore, clinical pharmacologists consider anti-epi leptic drugs as model drugs in pharmacokinetics and pharmocodynamics, since reliable methods are available for their determination and their effects and side effects can be defined. The methods of estimating of drugs in body fluids provide a tool that enables us to throw light on many obscure relationships in pharmaceutical treatment. Now that we can study the pharmacokinetics and interaction of drugs in man, many hypotheses based on clinical experience alone may well be eliminated or corroborated. The grow ing body of knowledge will make us more careful about the administration of drugs in combination. Now that we can study how biological parameters interfere with drug action, we may perhaps proceed to the scientific analysis of many clinical observations that suggest the importance of such factors as age, sex, menstrual cycle, pregnancy, fever, diet, stress, sport, climate, and altitude.

Book excerpt: This volume provides up to date knowledge of the pharmacologies of the various antiepileptic drugs, especially those which have become available since 1984 when the previous edition (Handbook of Experimental Pharmacology, Volume 74: Antiepileptic Drugs, edited by H.-H. Frey and D. Janz) appeared. The data concerning the individual drugs are presented in a structured format. There are also chapters providing overviews of subjects such as the neurophysiological and biochemical mechanisms of action of the antiepileptic drugs, their testing in experimental animals, their interactions, their clinical use, and the new agents currently in the course of development.

These interactions can be beneficial or can cause harm. The aim of this book is to increase awareness of the possible impact of combination pharmacotherapies. Pharmacokinetic and pharmacodynamic interactions are discussed supported by clinical and experimental data. The book consists of five sections covering the general concepts and advantages of combination therapies, the principles of drug interactions, the mechanisms of interactions, drug interactions in specific populations or in patients with co-morbid health conditions, and concludes with a look at the future directions for this field of research. The book will be of interest to all who prescribe antiepileptics to epileptic and non-epileptic patients, including epileptologists, neurologists, neuro-pediatricians, psychiatrists and general practitioners.

Book excerpt: Epileptic disorders need treatment for many years or even for life, and this makes a thorough understanding of the pharmacokinetics and possible hazards and side effects of the drugs used in treatment mandatory. During recent decades our knowledge in this field has considerably increased, not least as a result of the development of specific and sensitive methods for the determination of anti epileptic agents in biological material. The clinical pharmacology of this group of drugs has been studied extensively and can today be regarded as well established. This does not necessarily mean that drug treatment of epilepsy is without problems. For example, it has recently been shown that one of the newer anti epileptic drugs, greeted with great enthusiasm by clinicians, may in rare instances induce serious damage to the liver and the pancreas, and seems even to have a certain teratogenic potential. Clinical problems should be understood as a challenge to the experimental pharmacologist, who should try to find explanations for the clinical hazards, and, if possible, show new ways in which better drugs might be developed. In recent years interest has focused on the importance of the inhibitory transmitter 'l'-aminobutyric acid (GABA) in the pathophysiology of epilepsy, and there have been a series of attempts to find useful antiepileptic drugs among substances interfering with GABA metabolism in the CNS.

For each drug, the authors list FDA-approved indications and summarize the evidence for efficacy and tolerability from clinical trials for epileptic, psychiatric, pain, and headache indications. Also included are recommendations from professional societies such as the American Academy of Neurology, American Psychiatric Association, and Association for the Study of Headache. Tables in the final chapter summarize these recommendations and indicate the authors' choice of drug in various clinical situations.

Book excerpt: The pharmacological fight against epilepsy began many centuries ago when Hippocrates discovered that the cause of epilepsy is natural, as opposed to supernatural and, as a consequence, must be treated with a natural remedy. Even though science has significantly progressed since that era, the challenge to find remedies for epilepsy is ever present. The aim of this particular volume is to offer an up-to-date review of the most recent advances in antiepileptic drug development, considered from various viewpoints: (i) general, by taking into account the size of refractory epilepsy and its related problems; (ii) experimental, by exploring the mechanisms of epileptogenesis and the possiblility of influencing it through drugs, and (iii) clinical, by describing the results obtained with compounds currently at an advanced stage of testing.

Book excerpt: This updated third edition of a successful book is a description of both pharmacokinetic and pharmacodynamic antiepileptic drug (AED) interactions, including details of the magnitude and mechanism of interactions, and also of drug combinations that are not associated with interactions and therefore can be coprescribed without undue concern. Presented in alphabetical order and by drug class, drug interactions that occur between AEDs and also between AEDs and non-AEDs are described in three sections: Drug interactions between AEDs; Drug interactions between AEDs and non-AED Drugs: Interactions affecting AEDs; Drug interactions between AEDs and non-AED Drugs: Interactions affected by AEDs. Antiepileptic Drug Interactions: A Clinical Guide, 3rd Edition with its clear, concise and unambiguous content will allow physicians and allied health professionals to make more rational choices when AED polytherapy regimens are indicated. There is always a choice and avoiding highly interacting drugs and choosing drug combinations that are minimally interacting or do not interact should be the goal in treating patients with epilepsy. This book provides all the necessary information so as to allow this goal to be achieved and, if necessary, to aid effective management of AED interactions.

Meanwhile, the practice of anesthesiology has expanded to the preoperative environment and numerous locations outside the OR. Anesthetic Pharmacology: Basic Principles and Clinical Practice, 2nd edition, is an outstanding therapeutic resource in anesthesia and critical care: Section 1 introduces the principles of drug action, Section 2 presents the molecular, cellular and integrated physiology of the target organ/functional system and Section 3 reviews the pharmacology and toxicology of anesthetic drugs. The new Section 4, Therapeutics of Clinical Practice, provides integrated and comparative pharmacology and the practical application of drugs in daily clinical practice. Edited by three highly acclaimed academic anesthetic pharmacologists, with contributions from an international team of experts, and illustrated in full colour, this is a sophisticated, user-friendly resource for all practitioners providing care in the perioperative period.
