Available in PDF, EPUB and Kindle. Book excerpt: Chronic obstructive pulmonary disease (COPD) affects millions of people across the world. COPD is not only a major burden to patients but is also costly and results in billions of dollars of direct and indirect costs annually. In recent years and with advancement of science, the understanding of COPD has improved significantly. Fortunately, current management guidelines consider COPD a preventable and treatable condition, and recent studies clearly indicate that available pharmacological and non-pharmacological interventions may improve various clinical outcomes. COPD: A Guide to Diagnosis and Clinical Management offers an exciting, evidence-based assessment of the field and will be of significant interest to clinicians who care for patients with COPD, including primary care providers and specialists. Comprehensive and state-of-the-art, this title is authored by experts who took the task of developing a resource that focuses on the essential issues in caring for patients with COPD. The first four chapters of the book cover major points about the systemic nature of COPD, the clinical and physiological assessments, and the outcome measures and prognostic markers. In the following section, various pharmacologic and non-pharmacological management strategies are reviewed based on the available evidence. The final sections outline the non-pulmonary effects of COPD and their management. COPD: A Guide to Diagnosis and Clinical Management is a vital, evidence-based text that will prove invaluable for all clinicians who care for patients with this debilitating disorder.

Book excerpt: Chronic diseases are common and costly, yet they are also among the most preventable health problems. Comprehensive and accurate disease surveillance systems are needed to implement successful efforts which will reduce the burden of chronic diseases on the U.S. population. A number of sources of surveillance data--including population surveys, cohort studies, disease registries, administrative health data, and vital statistics--contribute critical information about chronic disease. But no central surveillance system provides the information needed to analyze how chronic disease impacts the U.S. population, to identify public health priorities, or to track the progress of preventive efforts. A Nationwide Framework for Surveillance of Cardiovascular and Chronic Lung Diseases outlines a conceptual framework for building a national chronic disease surveillance system focused primarily on cardiovascular and chronic lung diseases. This system should be capable of providing data on disparities in incidence and prevalence of the diseases by race, ethnicity, socioeconomic status, and geographic region, along with data on disease risk factors, clinical care delivery, and functional health outcomes. This coordinated surveillance system is needed to integrate and expand existing information across the multiple levels of decision making in order to generate actionable, timely knowledge for a range of stakeholders at the local, state or regional, and national levels. The recommendations presented in A Nationwide Framework for Surveillance of Cardiovascular and Chronic Lung Diseases focus on data collection, resource allocation, monitoring activities, and implementation. The report also recommends that systems evolve along with new knowledge about emerging risk factors, advancing technologies, and new understanding of the basis for disease. This report will inform decision-making among federal health agencies, especially the Department of Health and Human Services; public health and clinical practitioners; non-governmental organizations; and policy makers, among others.

Book excerpt: Chronic obstructive pulmonary disease (COPD) is a major cause of morbidity and mortality worldwide and is estimated to become the third most common cause of death over the next decade. The knowledge of COPD pathogenesis and the disease course has greatly improved this progression in understanding and continues to have significant implications in the management of this condition. Novel areas of interest in COPD pathogenesis include further development of animal models, a better understanding of the genetics and epigenetics, the role of the microbiome, and an increasing appreciation of the associated comorbidities. This book intends to provide the reader with a brief overview of these topics and also provide an in-depth review of the current nonpharmacological clinical approaches to managing patients with COPD.

Available in PDF, EPUB and Kindle. Book excerpt: This book provides teaching scripts for medical educators in internal medicine and coaches them in creating their own teaching scripts. Every year, thousands of attending internists are asked to train the next generation of physicians to master a growing body of knowledge. Formal teaching time has become increasingly limited due to rising clinical workload, medical documentation requirements, duty hour restrictions, and other time pressures. In addition, today’s physicians-in-training expect teaching sessions that deliver focused, evidence-based content that is integrated into clinical workflow. In keeping with both time pressures and trainee expectations, academic internists must be prepared to effectively and efficiently teach important diagnostic and management concepts. A teaching script is a methodical and structured plan that aids in effective teaching. The teaching scripts in this book anticipate learners’ misconceptions, highlight a limited number of teaching points, provide evidence to support the teaching points, use strategies to engage the learners, and provide a cognitive scaffold for teaching the topic that the teacher can refine over time. All divisions of internal medicine (e.g. cardiology, rheumatology, and gastroenterology) are covered and a section on undifferentiated symptom-based presentations (e.g. fatigue, fever, and unintentional weight loss) is included. This book provides well-constructed teaching scripts for commonly encountered clinical scenarios, is authored by experienced academic internists and allows the reader to either implement them directly or modify them for their own use. Each teaching script is designed to be taught in 10-15 minutes, but can be easily adjusted by the reader for longer or shorter talks. Teaching Scripts in Internal Medicine is an ideal tool for internal medicine attending physicians and trainees, as well as physician’s assistants, nurse practitioners, and all others who teach and learn internal medicine.

Book excerpt: The book endeavors to provide a stimulating and thought provoking scientific content to share and exchange new clinical studies and advancements in dealing with pulmonary diseases. The topics vary from clinical to translational research in respiratory diseases such as lung cancer, obstructive sleep apnea, chronic obstructive pulmonary disease, bacterial and fungal infections, lung lesions during febrile maladies, and others. An attempt has been made to show the intertwined relationship between the pulmonary system and other body systems such as kidney, cardiac, or hormonal functions. The ensuing interlocked morbidities, often exacerbating one another, require the coordination of various medical specialties to optimize the diagnostic and therapeutic processes. The knowledge sharing through publications of research and clinical experiences is indispensable to accelerate the innovation spectrum and to continue working on the therapeutic and preventive strategies in chronic pulmonary diseases. The book is addressed to pulmonologists, chest physicians, researchers, and healthcare professionals engaged in patient care.

Book excerpt: Since the 1970s, therapeutic nihilism has moved towards a more optimistic attitude regarding therapeutic alternatives in COPD. Research focused on inflammatory and physiological mechanisms has substantially increased during the last 10 years. This has led to an increased understanding of the pathophysiology of the disease, which has resulted in improved treatment. Thus, in parallel to smoking-cessation programmes, other treatment modalities have been shown to be successful. Physiotherapy and pharmacotherapy have been extensively studied and the knowledge regarding what these therapeutic approa.
