Book excerpt: PEREGRINE is a 3D Monte Carlo dose calculation system designed to serve as a dose calculation engine for clinical radiation therapy treatment planning systems. Taking advantage of recent advances in low-cost computer hardware, modern multiprocessor architectures and optimized Monte Carlo transport algorithms, PEREGRINE performs mm-resolution Monte Carlo calculations in times that are reasonable for clinical use. PEREGRINE has been developed to simulate radiation therapy for several source types, including photons, electrons, neutrons and protons, for both teletherapy and brachytherapy. However the work described in this paper is limited to linear accelerator-based megavoltage photon therapy. Here we assess the accuracy, reliability, and added value of 3D Monte Carlo transport for photon therapy treatment planning. Comparisons with clinical measurements in homogeneous and heterogeneous phantoms demonstrate PEREGRINE's accuracy. Studies with variable tissue composition demonstrate the importance of material assignment on the overall dose distribution. Detailed analysis of Monte Carlo results provides new information for radiation research by expanding the set of observables.

Available in PDF, EPUB and Kindle. Book excerpt: During a radiotherapy treatment, skin of the patient is always exposed to the radiation; as a consequence late effects can be appear. Therefore, dose values in this region may be of interest for clinical evaluation and investigation of those undesirable effects. However, surface dose is not intuitive and is difficult to measure and calculate; the parameters, which modify it are not always the same and each one are dependent to the others. The aim of this project is to quantify absorbed dose in the build-up region and near to the skin approximately between 0 mm - 1 mm using two methods. One of them, an analytical calculation using Monte Carlo simulations; and the other an experimental approach using an extrapolation chamber. Monte Carlo simulations are performed with the PENELOPE package of subroutines for 6MV and 15MV photon beams. Experimental setup consists in the use of an extrapolation chamber (PTW model 23392), considered the most suitable instrument to measure absorbed dose in the build-up region, but its not commonly available in the hospitals because its use is not practical. The chamber was adapted for its use in clinical environments in a previous work, then for this work the measurement process is easier and results with high level of accuracy can be obtained. In this way, EC (extrapolation chamber) measurements served as the gold standard in our study to validate the Monte Carlo simulations and also with results reported by previous studies. As a result, we found that simulation outcomes are in good agreement with experimental data.

Book excerpt: For ten days at the end of September, 1987, a group of about 75 scientists from 21 different countries gathered in a restored monastery on a 750 meter high piece of rock jutting out of the Mediterranean Sea to discuss the simulation of the transport of electrons and photons using Monte Carlo techniques. When we first had the idea for this meeting, Ralph Nelson, who had organized a previous course at the "Ettore Majorana" Centre for Scientific Culture, suggested that Erice would be the ideal place for such a meeting. Nahum, Nelson and Rogers became Co-Directors of the Course, with the help of Alessandro Rindi, the Director of the School of Radiation Damage and Protection, and Professor Antonino Zichichi, Director of the "Ettore Majorana" Centre. The course was an outstanding success, both scientifically and socially, and those at the meeting will carry the marks of having attended, both intellectually and on a personal level where many friendships were made. The scientific content of the course was at a very high caliber, both because of the hard work done by all the lecturers in preparing their lectures (e. g. , complete copies of each lecture were available at the beginning of the course) and because of the high quality of the "students", many of whom were accomplished experts in the field. The outstanding facilities of the Centre contributed greatly to the success. This volume contains the formal record of the course lectures.
