Available in PDF, EPUB and Kindle. Book excerpt: Neurosurgical procedures are becoming more common and are taking place in the operating room and in interventional suites. Procedures that used to be performed only at major academic institutions are also being done in small community hospitals, and anesthesiologists in private practice are being asked to care for these patients. In many cases, treatment options are controversial or rapidly evolving. Close cooperation between the anesthesiologist and neurosurgeon is essential to achieve optimal outcomes and early recognition of any adverse events so appropriate therapy can be implemented. Fundamentals of Neuroanesthesia is a comprehensive guide to neuroanesthesia that discusses neurophysiology, neuroanatomy, and neurosurgical procedures and offers practical approaches and solutions to administering neuroanesthesia and providing perioperative care for neurosurgical patients. Chapters emphasize clinical management of neurosurgical problems that may be encountered in community practice as well as major academic medical centers. Highlighted key points, figures, algorithms, and management procedures supplement the text. This book is a must-have volume for general anesthesiologists, anesthesiology fellows, and subspecialists.

Book excerpt: There are relationships that exist between neuroanesthesia, neurosurgical procedures, individual patient pathology and the positioning of a patient for said procedure. A comprehensive examination of these relationships, their association with patient morbidity/mortality and how to approach these issues in an evidence-based manner has yet to become available. Positioning related injuries have been documented as major contributors to neurosurgical/neuroanesthesiology liability. This text examines these relationships. It provides considerations necessary to the correct positioning of a patient for a neurosurgical procedure for each individual patient and their individual pathology. In other words, this text will demonstrate how to construct the necessary surgical posture for the indicated neurosurgical procedure given the individual constraints of the patient within the environment of anesthesia and conforming to existing evidence-based practice guidelines. Sections will address physiological changes inherent in positioning in relation to anesthesia for neurosurgical procedures, assessment of patient for planned procedure, as well as considerations for managing problems associated with these relationships. Additional sections will examine the relationship between neurosurgical positioning and medical malpractice and the biomechanical science between positioning devices and neurosurgical procedures. Neurosurgery and its patient population are in a constant state of change. Providing the necessary considerations for the neurosurgical procedure planned under the anesthesia conditions planned in the position planned, often in the absence of multicase study literary support, without incurring additional morbidity is the goal of this text.

Book excerpt: The perioperative care of individuals with neurologic compromise is critically important, yet it is only one dimension of the rich relationship between anaesthesiology and the neurosciences. The mechanism of everyday therapeutic interventions such as anaesthesia and analgesia is exciting neuroscience in its own right. At the new frontier of outcomes studies lies the question of how the perioperative period might impact the brain. For example, questions related to anaesthetic neurotoxicity, delirium, and cognitive dysfunction pose critical challenges for the field. The Oxford Textbook in Neuroscience and Anaesthesiology addresses the exciting field of neuroanaesthesiology in a new and stimulating way. In twenty eight chapters, the neuroscientific basis of anaesthesiology, the full spectrum of clinical neuroanaesthesia, and the care of neurologic patients undergoing non-neurologic surgery are explored in one comprehensive textbook for the first time. The first section considers the neural mechanisms of general anaesthetics, cerebral physiology, the neurobiology of pain, and more. The second section explores the care of patients with neurologic disease in the operating room or intensive care unit. These clinical chapters systematically treat the perioperative considerations of both brain and spine surgery, and provide introductions to neurocritical care and pediatric neuroanaesthesia. The final section outlines the care of neurologic patients undergoing non-neurologic surgery. It examines key connections of neurology and anaesthesiology, examining how conditions such as dementia, stroke, or epilepsy interface with the perioperative period. Each chapter has been carefully crafted to be concise yet highly informative, reflecting the cutting edge of neuroscience and neuroanaesthesiology. This international textbook gathers the best available expertise of authors and leaders worldwide. Includes access to online-only content, including more than 20 cases and more than 90 questions that can be used in presentations and teaching sessions. Book excerpt: Ode of the Knight is a tale about a young man who suffers the loss of his father. Unable to cope with the trauma, he quickly realizes that he, the eldest son, can not fulfill his father's shoes. Embarking on a path of denial, his world and his loving family begin to fall apart before his very eyes. His shame in his failure plunges him into the depths of hell. Incapable of improving his situation he rejects life and everything of value. Soon the fifteen year old finds himself in predicaments far beyond his grasp of reality. He lives in his own world by his own rules. The child's obsession for punishment and self abasement is paramount. A series of poor decisions resulted in an involuntary meeting with a worthy adversary, King Edward; a ruthless and unforgiving challenger. A Knight of the Royal Black Order recognizes a subtle value in the young man and beseeches the King not to behead the child but to give him the boy's soul for one year. Against better judgement, the King agrees. If the child survives the Knight's imprisionment filled with trials, hardships, and adversity, the boy will be ordered to stand before the King and petition for his life. His exhibition of strength during his incarceration will determine his fate.
