Book excerpt: Alliance and client involvement are thought to be important therapy process factors in child psychotherapy; however, few studies have investigated them over the course of treatment. The present study examined change in alliance, client involvement, and the relationship between the two over time in an effectiveness study comparing cognitive behavioral therapy (CBT) and usual clinical care (UC) for child anxiety disorders. The sample included 40 clinically-referred children (57.50%, female, mean age = 10.81, SD = 2.11, 35.00% Caucasian, 32.50% Latino/Hispanic, 5.00% African-American, 7.50% mixed ethnicity, 20.00% not reported) and 39 therapists employed by community clinics. Two doctoral-level students comprised the coding teams for each measure and independently rated alliance and client involvement in all available recorded sessions. Unconditional multilevel growth models indicated alliance and client involvement did not significantly change over time. Findings suggest that when measured by observational coders, initial levels of alliance and client involvement remain relatively stable throughout different treatments for child anxiety in community settings. Existing therapy process models may require further specification based on treatment setting and method of measurement. In practice, these findings suggest strategies to bolster initial alliance and client involvement could help improve the impact and delivery of child anxiety treatment in community settings.

Book excerpt: This study examined the association between the therapeutic alliance and psychotherapy outcomes in 350 children and adolescents receiving outpatient therapy at a community mental health clinic. Therapeutic alliance and psychosocial distress were measured at intake 3 week, 2 month, 4 month and 6 month intervals. Participants aged 12 and older completed self report versions of the outcome and alliance measures and the parents of participants aged 4-17 completed the outcome measure. Therapists completed alliance measures for each participant. Analyses examined the relation between youth-rated therapeutic alliance and psychotherapy outcomes, premature termination, problem type, age of client, and clinician-rated alliance. Results indicated that early therapeutic alliance ratings were not related to premature termination from therapy. With the exception of the 3-week time point problem type was not found to be related to the formation of the alliance. A relationship between age of the client and the formation of a therapeutic alliance was true at the 6 month time point indicating that the therapists rated their relationships with youth under 12 years old more favorably than youth over 13 years old. Finally, the therapist2s ratings of the alliance were not correlated with psychotherapy outcome. These findings indicate that associations between therapeutic alliance and psychotherapy outcomes may be less pronounced in youth treatment than in adult treatment.

Kendall (2000) identified three types of parent involvement: consultants, co-clients, and collaborators. Research examining the potential benefits of incorporating parents as co-clients and collaborators in individual and group cognitive behavioral therapy (CBT) for anxious children has found mixed results, likely due to methodological issues (e.g., not specifically measuring the type nor the amount of parental collaboration over the course of treatment), as well as child/family factors. Furthermore, these studies have often ignored how parental involvement impacts the treatment of social functioning, and less is known related to the impact of parental collaboration on group treatment outcomes in clinical service settings. This study attempted to address these research limitations by examining the influence of type and amount of parental collaboration on the outcomes of a resilience-based group CBT intervention that targets social functioning known as the Resilience Builder Program℗ʼ (RBP; Alvord, Zucker, & Grados, 2011) for anxious children in a private practice setting. Participants consisted of 99 children (Mage= 9.73; 72.7% male; 82.8% Caucasian; 63.6% on medication) enrolled in RBP at a large private psychotherapy practice. Parents of boys, children with co-morbid Attention Deficit Hyperactivity Disorder (ADHD), and those who participated in RBP for the first time reported higher amounts of assistance on the success journals over the course of therapy. Greater use of therapy terms at home was related to younger child age and greater parental psychopathology. Greater amount of collaboration over the course of therapy was related to higher pre-therapy ratings of anxiety, whereas higher ratings of the combination of specific types of collaboration at post-treatment (i.e., assisting with homework and the success journal) was related to higher family income, greater number of children in the household, and better pre-therapy ratings of family communication. Similarly, higher ratings of assistance on the success journals at post-treatment were significantly related to higher family income and greater number of children in the household. Additionally, higher ratings of assistance of homework at post-treatment were significantly related to pre-therapy ratings of family communication. Results found significant improvements in children0́9s emotional, social, and behavioral functioning following treatment with RBP; however, parental collaboration was not predictive of improvements in parent reports of children0́9s emotional or social/behavioral functioning. These findings provide preliminary clarification of the nature of parental collaboration in RBP and what clinical and demographic variables are associated with differences in parental collaboration for anxious children.

Book excerpt: Anxious youth who are school reluctant find school to be an aversive environment, yet do not refuse school. These children experience higher levels of loneliness, negative affectivity, and anxiety severity compared to their non-school reluctant counterparts with anxiety disorders. While factors influencing treatment outcome, such as the therapeutic alliance, have been examined extensively in children with anxiety disorders and in school refusing youth, no literature to date has examined these factors in school reluctant youth with anxiety disorders. Included in the present study were 91 youth with an anxiety disorder, of which 41 were school reluctant. This study sought to examine the impact of the therapeutic alliance on the relationship between school reluctance status and various indicators of treatment outcome. While school reluctance status was not an overall significant predictor of treatment outcome, it did significantly predict clinician-reported treatment outcome when examined in the context of middle- and late-treatment alliance. Therapeutic alliance did not significantly moderate the relationship between school reluctance status and treatment outcome as reported by child, parent, teacher, or clinician. A significant decrease was observed in the number of school reluctant youth at pre- compared to post-treatment. Results of this study suggest that cognitive behavior therapy is effective in treating child anxiety, regardless of school reluctance status.
