Book excerpt: Despite numerous reports of cerebral damage in cardiac surgery, the subject has not been given the attention it requires. This book, with a preface by Torkel Aberg, will remedy that situation. The causes and incidence of pre- and post-operative cerebral damage are considered in the first section. Cardiac surgery patients frequently have preoperative cerebral impairment, not suprising when one considers the impaired circulation from a damaged heart and the brain's prodigious need for blood. Moreover, several perioperative aspects of surgical procedures have been considered as possible causes of cerebral dysfunction, for example: microbubbles, toxic by-products, non-pulsatile blood flow. The second section describes how imaging techniques (CT scan, MRI, regional cerebral blood flow imaging), and functional assessment techniques. (PET scan, EEG, BEAM and evoked potentials) can be used to measure cerebral damage. In the third section, psychometric and neuropsychological techniques are used to assess impaired mental abilities (abstract thinking, language, memory, visuo-spatial ability, mental flexibility, attention and concentration). The final section explores the relationship between cerebral dysfunction and psychopathology (several types of depression, anxiety, and aspects of organic brain syndrome, delirium and dementia).

According to this article, old age represents a distinct period oflife in which the physiologic changes caused by aging are accompanied by an increasing number of pathologic changes. We now know that the organs of the body age neither at the same rate nor to the same extent and that physiologic alterations are indeed superimposed by pathologic changes; as a result of the latter phenomenon the origins and course of illnesses in the elderly can present unusual characteristics. The frequency of concurrent disorders in the elderly entails the danger of polypragmatic pharmacotherapy, i. e. , the use of various drugs to combat various disorders while neglecting the possibly adverse combined effects of these drugs. To obviate this danger, special knowledge in the field of geriatrics, the medical branch of gerontology, is necessary. Geriatrics is constantly increasing in importance owing to the near doubling of life expectancy over the past 130 years and to the improved diagnostic and therapeutic techniques made available by medical pro gress. The rapid recent development of experimental gerontology has played an essential role in enabling us to understand the special features of geriatrics. This progress has, however, been accompanied by such a vast increase in the volume of literature on the subject that specialists in the field can scarcely maintain an overall perspective of new publications.

Neglect has been replaced by a more optimistic therapeutic endeavour and countless old people have benefitted from this approach. But there is also a drawback, and this is the risk of hazardous side effects of medication which are often directly proportional to the biological potency of the drug and may be unpredictably increased due to changes in the senile organism. In fact the anatomical and biological changes in old age alter both the kinetics of most drugs and the receptor response. On account of these changes the individual tolerance of aged patients to drug therapy may be quite different from that of younger subjects. Thus for a variety of reasons elderly patients receive more drugs, but they are at a higher risk of encountering adverse reactions, which often show atypical clinical features. We can therefore speak of "geriatric iatrogenic disorders" and point out that some of these side effects are determined by a complex pathogenesis due to the particular pathophysiological condition in the elderly. It is important to encourage the habit of individ ually evaluating potential risk versus expected advantages of drugs in keeping with the same principles adopted in the evaluation of cost/benefit ratio.
