Es por esto que lo que aprendemos en los libros de texto y en las aulas de clases no siempre es igual a lo que vemos en la pr ctica cl nica, notando que un paciente enfermo no necesariamente padece de todos los s ntomas y signos descritos en los libros, se nos hace imprescindible dejar por escrito casos cl nicos, para que estudiantes de medicina puedan complementar el aprendizaje de los libros de texto, con pacientes de la vida diaria. Un caso cl nico es el reporte detallado de signos, s ntomas, diagn stico, tratamiento y evoluci n de un paciente individual. Tambi n puede incluir los perfiles demogr ficos y epidemiol gicos del paciente. Algunos casos cl nicos son ilustrativos de s ndromes comunes; otros pueden ilustrar tambi n alg n s ndrome de baja prevalencia pero de gran importancia, o pueden emplearse para la ense anza de alg n rea de la medicina. Los casos cl nicos tienen su propio papel en el progreso de la ciencia m dica. Ellos permiten el descubrimiento de nuevas enfermedades, de efectos inesperados (adversos o beneficiosos) y desempe an un papel importante en la educaci n m dica. Siguen siendo una de las piedras angulares del progreso m dico, ya que proporcionan nuevas ideas en medicina, y porque frecuentemente se hacen observaciones particulares importantes en el contexto del conocimiento ya existente de una enfermedad. El objetivo del reporte de un caso cl nico es hacer una contribuci n al conocimiento m dico, presentando aspectos nuevos o instructivos de una enfermedad determinada. Se debe escoger un caso cl nico siempre pensando en una necesidad mayor, es decir, si responde "s " a cualquiera de las siguientes preguntas: El mundo cient fico/m dico le gustar a saber sobre este caso? Deber a saberlo? Ser a bueno compartirlo? Tiene un enfoque nuevo y diferente? Para la elaboraci n de un caso cl nico, debe tener muy claro el mensaje did ctico que desea comunicar. La introducci n, el desarrollo y la conclusi n de cada caso deben reflejar el razonamiento que ha seguido el m dico a lo largo de todo el proceso diagn stico y terap utico, sin olvidar que su redacci n ha de ser amena para poder cumplir su funci n docente.

The collection featured many of the leaders in the field, including Paul Farmer, Arthur Kleinman, David Satcher, Nancy Scheper-Hughes, Dorothy Porter, and Leon Eisenberg. The Kaiser Family Foundation has conducted interviews with two of the authors of papers in this collection, David Satcher and Paul Farmer. In its launch issue in October 2004, PLoS Medicine signaled a strong interest in creating a journal that went beyond a biological view of health to incorporate socioeconomic, ethical, and cultural dimensions. For example, that first issue contained a policy paper on how the health community should respond to violent political conflict a debate on whether health workers should screen all women for domestic violence, and a study on the global distribution of risk factors for disease. Two years on, our October 2006 issue takes our interest even further. It contains a special collection of ten magazine articles and fi ve research papers devoted entirely to social medicine. We are delighted that the collection features many of the leaders in the fi eld, including the renowned medical anthropologists Paul Farmer and Arthur Kleinman, the former United States Surgeon General David Satcher, and the Harvard professor of social medicine and psychiatry Leon Eisenberg. Most of our readers have welcomed our inclusive view of what a medical journal should highlight. Some, however, have been critical, suggesting that we should publish "less soft stuff" and more "hard science." These critics might argue that in this era of stem cell research and the human genome project, of molecular medicine and DNA microarray technology, the notion of social medicine seems irrelevant and outmoded. But the ultimate role of a medical journal is surely to contribute to health improvement, and that means looking not just at molecules but at the social structures that contribute to illness. The stark fact is that most disease on the planet is attributable to the social conditions in which people live and work. The socially disadvantaged have less access to health services, and get sicker and die earlier than the privileged. Despite impressive technological advances in medicine, global health inequalities are worsening.
