Available in PDF, EPUB and Kindle. Book excerpt: Health systems should function in such a way that the amount of inappropriate care is minimized, while at the same time stinting as little as possible on appropriate and necessary care. The ability to determine and identify which care is overused and which is underused is essential to this functioning. To this end, the "RAND/UCLA Appropriateness Method" was developed in the 1980s. It has been further developed and refined in North America and, increasingly, in Europe. The rationale behind the method is that randomized clinical trials--the "gold standard" for evidence-based medicine--are generally either not available or cannot provide evidence at a level of detail sufficient to apply to the wide range of patients seen in everyday clinical practice. Although robust scientific evidence about the benefits of many procedures is lacking, physicians must nonetheless make decisions every day about when to use them. Consequently, a method was developed that combined the best available scientific evidence with the collective judgment of experts to yield a statement regarding the appropriateness of performing a procedure at the level of patient-specific symptoms, medical history, and test results. This manual presents step-by-step guidelines for conceptualising, designing, and carrying out a study of the appropriateness of medical or surgical procedures (for either diagnosis or treatment) using the RAND/UCLA Appropriateness Method. The manual distills the experience of many researchers in North America and Europe and presents current (as of the year 2000) thinking on the subject. Although the manual is self-contained and complete, the authors do not recommend that those unfamiliar with the RAND/UCLA Appropriateness Method independently conduct an appropriateness study; instead, they suggest "seeing one" before "doing one." To this end, contact information is provided to assist potential users of the method.

Available in PDF, EPUB and Kindle. Book excerpt: In Insulin Resistance: The Metabolic Syndrome X, outstanding investigators thoughtfully summarize our current understanding of how insulin resistance and its compensating hyperinsulinemia (Syndrome X) play a major role in the pathogenesis and clinical course of high blood pressure and cardiovascular disease-the so-called diseases of Western civilization-as well as polycystic ovary disease. Under the aegis of Gerald Reaven, the discoverer of Syndrome X, the distinguished authorities writing here detail for the first time the pathophysiological consequences and the clinical syndromes, excluding Type 2 diabetes, related to insulin resistance. They also examine the genetic and lifestyle factors that contribute to the wide differences in insulin action that exist in the population at large. Each author has been encouraged to present a point of view that reflects their unique insights. The first authoritative book on the subject, Insulin Resistance: The Metabolic Syndrome X illuminates the special importance of insulin resistance as a major cause of hypertension, heart disease, and polycystic ovary syndrome. Its thoughtful and detailed approach will make it an essential reference for basic and clinical researchers seeking to understand these critical phenomena.

This book was released on 2015-01-08 with total page 164 pages. Available in PDF, EPUB and Kindle. Book excerpt: This book disentangles the issues in connection with the advancement of Health Technology Assessment (HTA) and its interface with health policy. It highlights the factors that should shape its progress in the near future. Interdisciplinary and critical views from a number of professionals are put together in a prescient order to cast some light and make recommendations as to the next steps HTA should take to be fit for purpose. A wealth of documents dealing with HTA have been published over the last three decades. HTA allegedly is one of the bedrocks of regulation and medical decision making. However, counter vailing visions contend that geographical variations in the role that HTA is actually playing within countries pinpoints specific room for improvement. Given our social preferences, cherry-picking HTA’s features and successes over the last decades moves it away from its possibility frontier. Some of the most noteworthy hindrances that HTA faces, in several countries, to making headway towards its consolidation as an efficient tool for regulation and decision making are as follows: insufficient resources, delays in assessment, inadequate priority setting, regulatory capture, public distrust, actual influence on regulatory decisions, the need for strengthening international cooperation and harmony, the lack of sound and consistent assessments of diagnostic tests, medical devices and surgical innovations and limited dissemination. Time has come for HTA to take a renewed stand. There is a pressing need to submit HTA to in-depth critical scrutiny.

Written by three pioneers in this growing subspecialty, Onco-Nephrology provides authoritative, definitive coverage of the mechanism and management of these two life-threatening diseases. This unique, single-volume resource covers current protocols and recommends management therapies to arrest kidney failure and allow oncologic treatments to continue and succeed. - Addresses acute and chronic kidney diseases that develop from a variety of cancers. This includes direct kidney injury from the malignancy, paraneoplastic effects of the cancer, and various cancer agents used to treat the malignancy. - Discusses key issues regarding kidney disease in patients with cancer, including conventional chemotherapeutic regimens and new novel therapies (targeted agents and immunotherapies) or the malignancies themselves that may promote kidney injury; patients with chronic kidney disease who acquire cancer unrelated to renal failure; and kidney transplantation, which has been shown to carry an increased risk of cancer. - Contains dedicated chapters for each class of the conventional chemotherapeutic agents, targeted cancer agents, and cancer immunotherapies including the basic science, pathogenic mechanisms of injury, clinical manifestations, and treatment. - Includes special chapters devoted to the individual classes of chemotherapies that relate to kidney disease for quick reference. Discusses increasingly complex problems due to more numerous and specialized anti-cancer drugs, as well as increased survival rates for both cancer and renal failure requiring long-term patient care. - Covers anti-VEGF (antivascular endothelial growth factor) agents and cancer immunotherapies – treatments that are being recognized for adverse kidney effects. - Utilizes a clear, logical format based on the ASN Core Curriculum for Onco-Nephrology, making this reference an excellent tool for board review, as well as a practical resource in daily practice. - Enhanced eBook version included with purchase. Your enhanced eBook allows you to access all of the text, figures, and references from the book on a variety of devices.
