Available in PDF, EPUB and Kindle. Book excerpt: The theme of this International Symposium, co-sponsered by EFMI (the European Federation for Medical Informatics) and WHO-Eur (the European Office of the World Health Organisation) is made of four components: 1) An ethical principle: equity in health care, that aims to social justice; 2) Measurements of equity that should be case-based/population related; 3) Communication of this information at a wide scale, using telematics, e.g. integrated computer systems at distance; 4) actions to improve health care and equity in its delivery by using resource management tools and developing quality of care. The combination of these four elements is recommended as a coherent scheme for future actions of public authorities in health care. They were therefore associated in the somewhat complex title of the symposium. A special attention is given to the present degree of availability of a large number of case based/population related data sources in Europe, as well as on the need to develop generic models in telematics, rather than too strict standards at the user level. These information systems can be made easily accessible to clinicians and epidemiologists as an aid to their decision making in the European region.

Book excerpt: Electronic communications are already revolutionizing the delivery of health care in homes and communities. The medium will have increasing impact on the delivery of health care in response to the pressure to use limited resources cost-effectively. To date much of telematics in health care has focused on hospital-based bio-medicine. In this book a case is made for reorientation of telematic activity towards the use of "low tech" (social) technologies supported by "Health for All" philosophies. This book uses several case studies of pilot schemes to illustrate how the models can be applied in a variety of settings. Experience of telematics in America, Canada, Germany, Italy, The Netherlands, Portugal and Wales is shared for an international readership of innovators in health care management.

Available in PDF, EPUB and Kindle. Book excerpt: In the last few years, the protection of computerised medical records, and of other personal health information, has become the subject of both technical research and political dispute in a number of countries. In Britain, the issue arose initially as an argument between the British Me dical Association and the Department of Health over whether encryption should be used in a new medical network. In Germany, the focus was the issue to all patients of a smartcard to hold insurance details and facilitate payment; while in the USA, the debate has been whether federal law should preempt state re gulation of computerised medical records, and if so, what technical and legal protection should be afforded the patient. Whatever the origin and evolution of this debate in specific countries, it has become clear that policy and technical matters are closely intertwined. What does 'computer security' mean in the medical context? What are we trying to do? What are the threats that we are trying to forestall? What costs might reasonably be incurred? To what extent is the existing technology - largely developed to meet military and banking requirements - of use? And perhaps hardest of all, what is the right balance between technical and legal controls? As the debate spread, it became clear that there was little serious contact between the people who could state the requirements - clinical professionals, medical ethicists and patients - and the people who could explore how to meet

Book excerpt: The case mix concept has been introduced in the USA more than twenty years ago in order to measure hospital productivity and to promote quality of care. The DRGs were selected by HCFA in 1983 as the case mix classification system for the MEDICARE prospective payment system (PPS). Many other countries have adopted the case mix concept after long periods of testing and accepting, but with large variations in data collection, information standards, grouping tools, financing methods and quality of care developments all over the world. Each country has developed a local clinical and political culture about case mix tools. The present book is intended to update the case mix situation, country by country.

Available in PDF, EPUB and Kindle. Book excerpt: The provision of health care services for children is central to improving the nation's health and remains a key feature of every government's policy. This concept has been recognised in the United Kingdom since nineteenth century visionaries prompted increasing interest in the welfare of the country's school children. Successive generations have built upon these foundations and have been diligent in promoting effective service development. It is right that we follow their example, for the young have only one chance of a healthy upbringing. They are totally dependent on others to provide the right services, which are sensitive enough to address individual needs, yet sufficiently comprehensive to enable as many children as possible to reach adulthood with their potential uncom promised by illness. Our objective must be to enable today's children to enjoy a healthy childhood and to equip them to maximise the benefits of a healthy lifestyle in the years to come. We are making an essential investment in the future and must appreciate the challenges which come with that commitment. We need a multi-professional approach, positive management skills and the adoption of good management practice. I therefore welcome this book on managing child health services and commend the initiative of its editors in bringing together such an impressive team of contributors from different disciplines.

Book excerpt: This book contains examples of "convincing cases" that were selected from all over Europe and presented by experienced users and healthcare decision-makers. These examples are based on successful cases of implementation of health telematics applications and services such as electronic healthcare records and hospital information systems, specialised departmental systems, emergency telemedicine, and regional healthcare networks. Examples of experiences relating to medical informatics education are also included since the education is a crucial factor in the area of user acceptance. Cases of successful promotion of good quality applications and other issues related to users and technology are included in the last section. The readers will find qualitative and quantitative descriptions of many useful experiences of implementation and use of health telematics systems and services. The text of many of the papers is very rich in quantitative analysis and statistics providing good source of reference.
