Book excerpt: The objective of the program committee of the Fifth International Symposium on Atherosclerosis was to bring together experts in many disciplines to broaden the scope of the attack on this disease and to foster interaction. Our hope was that such interaction would accelerate the eradication of the disease. The symposium achieved that objective and con tinued the tradition of the previous symposia in providing a forum for summaries of recent research developments in the study, treatment and prevention of atherosclerosis. The leading authorities and researchers in this field and in the related areas of interest have presented the newest information, concepts and ideas that have evolved in the past three years since the previous meeting in Tokyo. The most promising fields for future investigation are clearly identified, as are the nature of the controversies that persist in some highly important aspects of treatment of this disease. The appearance of these proceedings so soon after the meeting will greatly enhance the impact of the symposium on current research in atherosclerosis. The program committee is particularly indebted to the excellent response of the inves tigators for their willingness to participate in the symposium and for their successful efforts in bringing high quality to their presentations. The impetus for the study was an article in the Los Angeles Times about architectural interventions that were planned for several noise-impacted schools under the air corridor of Los Angeles Interna tional Airport. These interventions created an opportunity to study the same chil dren during noise exposure and then later after the exposure had been attenuated. The study was designed to test the generality of several noise effects that had been well established in laboratory experimental studies. It focused on three areas: the relationship between noise and personal control, noise and attention, and noise and cardiovascular response. Two years later, a second study, designed to replicate and extend findings from the first, was conducted.

Separated somewhat in time and place, each developed its distinctive socio-cultural accouterments but all shared a need to adjust to the individual, societal and environmental limitations imposed by life at high altitude. Partial oxygen pressure, temperature and humidity fall systematically as altitude rises, but there are other changes as well. Darwin, Forbes, von Humboldt, von Tschudi and other naturalists of the eighteenth and nineteenth centuries who weaved their way through South America commented repeatedly on the tolerance or apparent indifference of the indigenes to the rigors of life at altitudes above 3000 meters but its impact upon lowlanders. Von Tschudi (1847), for example, observed 'in the cordillera the effect of the diminished atmospheric pressure on the human frame shows itself in intolerable symptoms of weariness and an extreme difficulty of breathing . . . . The first symptoms are usually felt at the elevation of 12,600 feet (3800 m) above the sea. These symptoms are vertigo, dimness of sight and hearing, pains in the head and nausea . . . . Inhabitants of the coast and Europeans, who for the first time visit the lofty regions of the cordillera, are usually attacked with this disorder. ' But von Tschudi's description of acute mountain sickness was hardly the first; his Spanish predecessors had known and commented upon it too.

Book excerpt: Hypertension is a major health problem and contrary to ischemic heart disease, which occurs only in Western countries, its distribution is almost universal. It is this universality that has prompted us to gather, in this book, data on arterial blood pressure obtained in different parts of the world. Moreover, cerebrovascular mortality, which is the commonest cause of death from hypertension, is decreasing in most Western countries and in Japan, and the reasons for this are still far from clear. A major problem in comparing blood pressure values from different centers is the standardization of the measurement. Complete standardization will never be achieved if one takes into account the numerous factors that may influence blood pressure in epidemiological studies. Whether blood pressures are measured by doctors or by technicians, are recorded at home, in the working place or in a hospital, in sitting, standing or supine position, and is taken during the same examination-all of these whether a blood sample factors can influence blood pressure measurement. But meals, time of day (blood pressure being higher in the evening), heart rate, cuff size, stethoscope used, digit preference, month of year, temperature, etc., can equally influence the measurement. Home reading of arterial blood pressure at standardized times is probably the best answer to all of these problems and has been used with gratifying results in a comparative study between Belgium and Korea.
