Specifically, the laser’s unique physical properties and precise bio-tissue interactions render this versatile source of biologic energy an attractive tool for multiple therapeutic purposes in cardiovascular medicine. Over the course of the last 2 decades the utilization of laser technology has become an important component for the management of patients with complex cardiovascular diseases. During this time period, cutting edge laser technology including a variety of wave length generators, newly designed catheters, and a selection of advanced optic fibers have been introduced and applied in the cardiovascular circulation. Improved lasing techniques in the cardiac catheterization suites and operation rooms have been implemented for treatment of ischemic coronary syndromes, peripheral arterial occlusive disease and other atherosclerotic thrombotic conditions. Moreover, during this 20 year time frame, several multicenter and single center clinical studies have been published focusing on the role and utilization of lasers in coronary and peripheral revascularization. And within the rapidly expanding field of interventional cardiac electrophysiology, laser technology has recently revolutionized the management of fractured, abandoned and malfunctioning leads of cardiac pacemakers and automatic defibrillators. Consequently, replacing a notoriously cumbersome and high risk open heart surgery with safe and markedly efficient percutaneous laser based extraction. This textbook will provide the most authoritative, comprehensive and contemporary information covering technological progress, clinical experience and pertinent aspects of laser applications in cardiovascular medicine. It will be of interest to cardiologists, vascular surgeons and interventional radiologist as well as medical students, scientists, biomedical engineering students and graduates.

Book excerpt: As noted in the Foreword, this report is one of several volumes resulting from this study of future health care technology. The purpose of the study, as formulated by the STG, was to analyze future health care technology. Part of the task was to develop an 'early warning system' for health care technology. The primary goal of the project was to develop a list or description of a number of possible and probable future health care technologies, as well as information on their importance. Within the limits of time and money, this has been done. However, given the vast number of possible future health care technologies, complete information on the importance of each area could not be developed in any depth for all technology. Therefore, four specific technologies were chosen and were prospectively assessed. These future technologies were examined in more depth, looking particularly at their future health and policy implications. Subsequently, the project was extended to September 1986, and two additional technologies will be assessed.

Available in PDF, EPUB and Kindle. Book excerpt: Today, nearly 60 years after the invention of the first medical laser, multiple laser and light systems exist and are applied in various medical specialties such as dermatology, ophthalmology, and urology. This volume - the first in the series Aesthetic Dermatology - focuses on the laser treatment of cutaneous lesions with a vascular target. Each chapter describes a particular laser or light modality and its specific application to a variety of both vascular and nonvascular lesions. Renowned specialists in laser medicine have contributed their expertise, incorporating current evidence-based literature and their own personal treatment recommendations, as well as pearls and perils. The purpose of this book is to explore the options and parameters available to treat cutaneous lesions traditionally responsive to vascular laser therapy and to expand the application to further lesion treatments. Readers who wish to broaden their knowledge and further hone their skills in treating cutaneous vascular lesions with lasers will find this publication a valuable and comprehensive review.

Book excerpt: With the growing age of the population, the proportion of elderly patients with diffuse coronary artery disease and severe comorbidities is increasing. The characteristics of these patients render them unsuitable for currently recognized revascularization procedures (CABG, PTCA). Therefore, there is an urgent need for new treatment modalities which could alleviate their symptoms and increase blood flow to the ischemic myocardium, while being less invasive for their fragile condition. The aim of this book is to provide an anatomical and physiological background as well as a complete review of the alternative methods of revascularization which are under development, namely transmyocardial laser revascularization, therapeutic angiogenesis and neurostimulation.

Available in PDF, EPUB and Kindle. Book excerpt: Since the development of the first laser generator by Maiman in 1960, laser energy has gained increased interest and has subsequently been used in various fields of applica tion. Due to its high energy density, combined with a sharply defined irradiation field, laser has aroused great expectations, especially since laser beams have been used very successfully in various fields especially technology and medicine. Medical and surgical applications of the laser were among the first and have been of undisputed benefit in, for example, ophthalmologic interventions such as coagulation of the retina, or for endoscopic tumorectomy in the bronchial, gastrointestinal and urogenital tracts. Consequently, considerable research effort was made to also apply laser techniques in interventional cardiology for the treatment of atherosclerosis, aimed at removing plaque material from stenosed tissue or occluded ateries and recanalizing tough and calcified segments unsuitable for the well-extablished technique of balloon dilatation. Ablating atheroma rather than pushing it aside was thought to achieve better results. However, it was found that the laser may create rough-edged cuts partly with substantial thermal or mechanical damage to the surrounding tissue, consequently provoking preconditions for unwanted restenosis. Different types of laser generators and probes have therefore been investigated in arterial vessels, mostly focusing on indirect laser effects. Intravasal probes with laser heated metal tips were used, as well as balloon catheters with an incorporated laser fiber designed to heat the surrounding tissue outside the balloon and to, ineffect, "iron out the plaque".

Book excerpt: Nd:YAG laser has recently been widely used for the various vascular applications. This book presents basic vascular and tissue interactions in the Nd:YAG laser treatment of vascular lesions and prepares a guide for clinical use. Lesion diameter, colour, depth and pressure make the basis of the choice of laser parameters. Based on the research presented in this book, it is evident that lasers have been used increasingly as an alternative to surgical methods in the treatment of congenital and acquired vascular lesions. Clinical use of Nd:YAG laser presents a wide range of options to the practising physician in the treatment of different vascular lesions from easy to difficult.

Available in PDF, EPUB and Kindle. Book excerpt: Lasers have a wide and growing range of applications in medicine. Lasers for Medical Applications summarises the wealth of recent research on the principles, technologies and application of lasers in diagnostics, therapy and surgery. Part one gives an overview of the use of lasers in medicine, key principles of lasers and radiation interactions with tissue. To understand the wide diversity and therefore the large possible choice of these devices for a specific diagnosis or treatment, the respective types of the laser (solid state, gas, dye, and semiconductor) are reviewed in part two. Part three describes diagnostic laser methods, for example optical coherence tomography, spectroscopy, optical biopsy, and time-resolved fluorescence polarization spectroscopy. Those methods help doctors to refine the scope of involvement of the particular body part or, for example, to specify the extent of a tumor. Part four concentrates on the therapeutic applications of laser radiation in particular branches of medicine, including ophthalmology, dermatology, cardiology, urology, gynecology, otorhinolaryngology (ORL), neurology, dentistry, orthopaedic surgery and cancer therapy, as well as laser coatings of implants. The final chapter includes the safety precautions with which the staff working with laser instruments must be familiar. With its distinguished editor and international team of contributors, this important book summarizes international achievements in the field of laser applications in medicine in the past 50 years. It provides a valuable contribution to laser medicine by outstanding experts in medicine and engineering. Describes the interaction of laser light with tissue Reviews every type of laser used in medicine: solid state, gas, dye and semiconductor Describes the use of lasers for diagnostics
