The first part describes the differences between hypoxia and ischemia, animal models, the effects of ischemia on myocardial function and metabolism, and the electrophysiological consequences of ischemia. The second part deals with the mechanisms of cardiomyocyte death in ischemia, structural aspects of irreversible ischemic injury, necrosis and apoptosis of cardiac cells, the role of calcium, and the concept of calcium antagonism. The third chapter is a brief description of reperfusion injury, its clinical relevance, and possible prevention. The fourth part summarizes changes in myocardial vasculature during ischemia and reperfusion. The fifth part is the survey of two main possibilities for increasing cardiac resistance to ischemia and hypoxia, i.e. long-lasting adaptation to chronic hypoxia and short-lasting preconditioning. The last part of the book deals with comparative and ontogenetic aspects of cardiac sensitivity to oxygen deprivation; this chapter also summarizes the ontogenetic differences and limitations in endogenous and exogenous protection of the ischemic/hypoxic heart.

Available in PDF, EPUB and Kindle. Book excerpt: Myocardial ischemia and subsequent reperfusion of the ischemic myocardium represent complex phenomena encompassing numerous physiological processes. This book aims at enhancing our understanding of these processes and stresses recent important developments in this very active area of research. The concise, state-of-the-art reviews cover recent advances in many fields important to the area of myocardial ischemia and reperfusion including physiology, pathology, pharmacology, biochemistry and molecular biology with reference to clinical relevance and applicability of these findings. Major areas which are highlighted include vascular mechanisms resulting in myocardial ischemia, cellular events in the ischemic, postinfarcted and reperfused myocardium as well as new exciting developments in cardiac protection that involve both novel pharmacological approaches as well as endogenous cardioprotective mechanisms such as preconditioning. Aimed at both the basic and clinical cardiovascular investigator, the book comprehensively reviews the rapid progress made in recent years in understanding the etiology of myocardial ischemia and reperfusion. It will further serve as an authoritative reference for all those interested in learning about the important developments in the treatment of myocardial ischemic and reperfusion disorders.

Book excerpt: Over the past three decades, impressive progress in the field of pathogenesis, prevention and therapy of ischemic heart disease has resulted in a marked decline in mortality in the Western World. However, the incidence of this devastating disease is on the rise in developing countries. The Ischemic Heart is based upon a recent symposium in Tokyo on the subject. This volume is organized into two sections: (i) Pathophysiologic Mechanisms of Ischemia-Reperfusion Injury and (ii) Preconditioning and Protection of Ischemia-Reperfusion Injury, and contains up-to-date information concerning the current concepts of ischemia-reperfusion injury, the sequence of events resulting in the loss of contractile dysfunction, and mechanisms of cardioprotection by several drugs as well as the role of ischemic preconditioning in attenuating problems associated with ischemia-reperfusion injury.

Book excerpt: Current evidence suggests that the ischemic preconditioning response is a multi-factorial process consisting of an initial early trigger, an intermediate mediator, and an end effector. Each of these steps in is now its own intense area of investigation. The need to render the heart ischemic for a brief period to invoke the preconditioning response is currently the major factor limiting clinical application of this powerful cardioprotective strategy. Recent research efforts have utilized brief exposures to pharmacological agents, in lieu of a brief preconditioning ischemia, to trigger/mimic the ischemic preconditioning-induced response. The World Heart Congress held in Winnipeg in July 2001 provided a forum for the presentation of new insights into the basic mechanisms of ischemia and reperfusion injury, as well as novel strategies to protect the heart from cell death, ventricular arrhythmias, and contractile dysfunction. Many pioneers in the fields of ischemia-reperfusion injury and preconditioning-induced protection presented there and the chapters in this book represent selected papers from these symposia.

This book was released on 2012-12-06 with total page 417 pages. Available in PDF, EPUB and Kindle. Book excerpt: The cardiac system represents one of the most exciting challenges to human ingenuity. Critical to our survival, it consists of a tantalizing array of interacting phenomena, from ionic transport, membrane channels and receptors through cellular metabolism, energy production to fiber mechanics, microcirculation, electrical activation to the global, clinically observed, function, which is measured by pressure, volume, coronary flow, heart rate, shape changes and responds to imposed loads and pharmaceutical challenges. It is a complex interdisciplinary system requiring the joint efforts of the life sciences, the exact sciences, engineering and technology to understand and control the pathologies involved. The Henry Goldberg Workshops were set up to address these multivariable, multidisciplinary challenges. Briefly, our goals are: To encourage international cooperation and foster interdisciplinary interaction between scientists from the different areas of cardiology; to relate microscale cellular phenomena to the global, clinically manifested cardiac function; to relate conceptual modeling and quantitative analysis to experimental and clinical data; to gain an integrated view of the various interacting parameters, identify missing links, catalyze new questions, and lead to better understanding of the cardiac system. The outstanding success of past workshops has encouraged their continuation. The first Henry Goldberg Workshop, held in Haifa in 1984, introduced the concept of interaction between mechanics, electrical activation, perfusion and metabolism, emphasizing imaging in the clinical environment. The second Workshop, in 1985, discussed the same parameters with a slant towards the control aspects.

Book excerpt: Coronary heart disease (CHD) is the leading cause of death worldwide with 3.8 million men and 3.4 million women dying each year. Cardioprotection refers to the prevention of CHD and the clinical improvement in patients suffering from cardiovascular problems. This book focuses on the role of cardioprotection in surgery and the use of pharmacological therapies such as ACE-inhibitors, statins and beta-blockers in order to reduce the myocardial injury sustained by the patient and the significant risk of morbidity and mortality. It includes new cardioprotective strategies aimed at improving the clinical outcomes of patients in these settings, as well as current well-established methods for reducing myocardial injury in acute coronary syndrome patients.

During the last decades, major improvements have been done in reducing both ischemic and reperfusion injury and therefore improving the quality of the operation. This book summarizes the newest developments in this field. In addition, specific comments are made concerning the surgical application of the fields in all kinds of operation, including routine cases as well as complex procedures. The reader will gain a detailed insight regarding how to protect patients best during routine cardiac procedures (coronary artery revascularization and valve replacement) as well as prolonged, technically demanding operations

This book was released on 2012-07-20 with total page 311 pages. Available in PDF, EPUB and Kindle. Book excerpt: Much has been written about reperfusion injury in the past decade but unfortunately the information has been generally presented in the form of original specialist papers and little if any integral publication exists on the topic, summarising and analysing the clinical impact of the condition and its management. The pathophysiology and molecular mechanisms of reperfusion injury are complex and, regarding diagnosis, individual diagnostic techniques have been proposed but without a proper assessment of the relative values of these methods. A publication dealing with integral diagnostic strategies would be welcome by the managing physician. Management of the condition is also problematic, as strategies that appear to work in the experimental models do not translate into beneficial interventions in patients. There is a need for these issues to be addressed and discussed in a monographic fashion. Management of Myocardial Reperfusion Injury will tackle these issues in a modern and systematic way and the information will be delivered in a fashion that will be appealing to the reader.
