All sectors of the country are attempting to push forward initiatives that will improve the health care system as well as the health of the American population in general. Despite the economical dedication to health care, about 1/5, the system remains uneven and fragmented, patient harm is quite common, care is often uncoordinated, and many more mishaps occur. There exists many obstacles to improve the nation's health care system; these include the capacity to reliably and consistently measure progress. In 2006 the Institute of Medicine (IOM) established the Roundtable on Value & Science-Driven Health Care which has since accelerated the development of a learning health system- one in which science, informatics, incentives, and culture are aligned to create a continuous learning loop. This learning loop would thus help make the health care system better. In response, the IOM organized a 2-day workshop to explore in depth the core measurement needs for population health, health care quality, and health care costs. The workshop hoped to gain a full understanding of how to improve the nation's measurement capacity to track progress in the health care system. Having this knowledge would help the nation get one step closer to the creation of an efficient learning loop. The workshop was divided into a series of sessions that focused on different aspects of measurement. Core Measurement Needs for Better Care, Better Health, and Lower Costs: Counting What Counts: Workshop Summary includes explanations and key details for these sessions: Vision, Current Measurement Capabilities, Specifying the Shape of a Core Metric Set, and Implementation. A hospital that excels at capacity management is easy to spot: no lines of people waiting and no patients in hallways or sitting around in chairs. These hospitals don’t divert incoming ambulances to other hospitals; they have excellent patient safety records and efficiently move patients through their organization. They exist but are sadly in the minority of American hospitals. The vast majority are instead forced to constantly react to their own poor performance. This often results in the building of bigger and bigger institutions, which, instead of managing capacity, simply create more space in which to mismanage it. These institutions are failing to resolve the true stumbling blocks to excellent patient care, many of which you may have experienced firsthand in your own visit to your hospital. Or, "We need to build capacity in schools so that students can achieve." In district offices, statehouses, and elsewhere, the sentiment is repeated in various forms, but the term "capacity" is almost always used. What do education leaders mean when they use the word capacity? Some people use the word to mean the intangible behaviors or characteristics that are needed for a school to improve, while others use capacity as a synonym for ability or knowledge. Meanwhile, economists often talk about capacity in terms of quantities or outcomes that an organization is able to produce. Given the nearly ubiquitous use of the term in education policy discourse, the authors offer a common framework for analyzing capacity that educators, policymakers, and researchers alike can apply and understand with consistency. Their goal is not to provide an easy, new, one-sentence definition, but rather to create a shared language that can be applied to research and improvement efforts in schools. In this Policy Brief, they break capacity down into component parts, explaining how each one builds off the next and contributes to the overall concept. Their hope is that the four research-based components they suggest--human capital, social capital, program coherence, and resources (building on Hatch, 2009; Elmore, 2000; Fullan, 2000; Spillane & Thompson, 1997; Corcoran & Goertz, 1995)--will stimulate discussion of a widely accepted meaning of capacity in both research and practice.
