Les sujets âgés constituent un groupe hétérogène avec des caractéristiques qui nécessitent une prise en charge médicale individualisée par rapport aux sujets plus jeunes. Nos travaux ont montré d'une façon générale, l'importance de l'impact de l'âge dans le diagnostic, le traitement, la survie et la qualité de vie dans le cancer du sein. L'âge moyen de nos patientes est de 77ans et la tranche la plus touchée est celle des patientes âgées entre 72 et 74ans avec 31%.26 patientes ont subi une chimiothérapie 100% avant ou après l'intervention chirurgicale et seulement 2 patientes n'ont pas subi une chirurgie (8%).Une chimiothérapie peut être nécessaire après ou avant l'intervention chirurgicale afin d'améliorer la survie et la qualité de vie des patientes âgées. Dans ce contexte, l'évaluation oncogériatrique présente un intérêt et serait d'une importance capitale car elle permet une prise en charge individualisée de la patiente âgée atteinte d'un cancer du sein, avec de nombreux facteurs, tels que l'espérance de vie, les comorbidités, les fonctions cognitives et l'environnement social.

Book excerpt: Contexte : les patientes porteuses d'un cancer du sein sont sous représentées dans les essais cliniques alors que 60% des décès en lien avec le cancer du sein survient chez la femme âgée de 65 ans et plus. Les essais cliniques concernant les traitements adjuvants chez le sujet âgé sont rares alors que les résultats obtenus dans les essais sur la population générale ne peuvent être directement extrapolés aux patients âgés sans preuves spécifiques. De ce fait nous avons examiné les facteurs associés à la prescription d'une chimiothérapie adjuvante et l'impact de celle-ci sur la survie globale dans une large cohorte française de patiente âgée de 65 ans et plus. Patients et méthodes : les patientes ont été rétrospectivement identifiées dans une large cohorte de 23,134 patientes avec un cancer du sein localisé qui ont préalablement bénéficié d'une chirurgie première dans dix-huit centres académiques français entre 1990 et 2019. Une analyse par régression logistique binaire a été réalisée en vue d'identifier les facteurs associés à la prescription d'une chimiothérapie adjuvante chez ces patientes. L'impact de la chimiothérapie adjuvante sur la survie globale a été étudié dans ce groupe à l'aide de la réalisation d'une analyse multivariée selon le modèle de Cox avec inclusion des facteurs confondants connus tel que : l'âge ; le type histologique ; le grade tumoral ; la taille tumorale ; l'invasion lympho-vasculaire ; le statut ganglionnaire ; la présence de récepteurs hormonaux et hormonothérapie adjuvante. L'étude de la survie globale a été menée après appariement des groupes selon un score de propension. Résultats : Sur 6605 patientes âgées de 65 ans et plus, 1493 ont bénéficié d'une chimiothérapie adjuvante (22.6%). L'administration d'une chimiothérapie adjuvante était associée de manière significative à une atteinte ganglionnaire macroscopique et chez les patientes avec des récepteurs hormonaux négatifs. De même un âge inférieur à 80 ans, une histologie canalaire, une taille tumorale >20mm, un grade tumorale élevé et une hormonothérapie adjuvante était statistiquement associé à la prescription d'une chimiothérapie adjuvante. Après analyse multivariée selon le modèle de Cox incluant l'âge, l'histologie, l'invasion lympho-vasculaire, la taille tumorale, l'atteinte ganglionnaire, l'hormonothérapie adjuvante, l'expression des récepteurs hormonaux et le grade tumoral, la chimiothérapie adjuvante améliorait significativement la survie globale (HR 0.71, IC95% 0.58-0.86, p

Les objectifs de ce travail étaient d'identifier les déterminants socio-économiques et géographiques du stade de découverte du cancer du sein en fonction de l'âge, de décrire la prise en charge et la survie des patientes atteintes de cancer du sein en fonction de l'âge, et d'identifier les déterminants de la qualité de vie, en fonction de l'âge chez des survivantes à 5 ans du cancer du sein. Les résultats des travaux réalisés ont montré que les femmes âgées de 50-74 ans et qui vivaient dans des zones économiquement défavorisées étaient diagnostiquées à des stades avancés pour leur cancer du sein. Les résultats ont également permis de mettre en évidence une diminution du traitement par chirurgie conservatrice et une augmentation de la proportion de femmes ne recevant aucun traitement chez les femmes les plus âgées. La survie était également moins bonne chez ces femmes. Enfin, cinq ans après le diagnostic du cancer du sein, chez les femmes âgées, seuls les comorbidités et le score de précarité EPICES impactaient la qualité de vie. Afin d'améliorer la survie et la qualité de vie des patientes âgées, une approche pluridisciplinaire par l'évaluation oncogériatrique, présente un intérêt et serait d'une importance capitale pour permettre une prise en charge individualisée de ces patientes.

Environ 10% des femmes > 75 ans atteinte d'un cancer du sein y seront confrontées. Leur plus mauvais pronostic impose souvent le recours théorique à la chimiothérapie. L'objectif de ce travail est d'étudier les caractéristiques cliniques et biologiques des femmes âgées atteintes d'un CSTN et d'analyser leur prise en charge.Cette étude rétrospective a analysé 1692 patientes âgées de plus de 75 ans présentant un cancer du sein infiltrant au stade précoce, diagnostiqué à l'Institut Curie entre 2010 et 2015. Les paramètres étudiés étaient : caractéristiques cliniques incluant des paramètres gériatriques, caractéristiques tumorales, détail des traitements, survie globale (SG) et survie sans maladie (SSM).Un CSTN au stade précoce a été retrouvé chez 8,5% des patientes (n=134). L'âge médian était de 81 ans. Le questionnaire G8 a été réalisé chez 51 % des patientes et était anormal dans 71% des cas. Une évaluation gériatrique approfondie a été réalisée chez 53% des patientes. 90% des patientes ont été opérées et 74% ont reçu une radiothérapie adjuvante. 46% ont reçu une chimiothérapie (taxanes : 49% ; anthracyclines : 26%). La SG à 5 ans et la survie spécifique étaient respectivement de 69% (IC95% : 61-79) et 75% (IC95% : 67-84). La SSM à 5 ans était de 56% (IC95% : 46-67). L'âge
