Book excerpt: With the beginning of the 1980's it was becoming increasingly evident that the lack of approval of new cardiovascular agents for use by clinicians in the United States for the treatment of cardiovascular disorders was becoming a problem. Patients requiring medical therapy for hypertension, angina pectoris, arrhythmias, congestive heart failure, and vasospastic disorders of the coronary arteries could receive in the United States only a small number of the drugs available to physicians in the rest of the world. In fact, as the 1980's began, there was only one available beta blocking agent released by The Food and Drug Administration; and even as of this writing, no oral calcium antagonist agent. This lag, in part, has been due to the confusion of proper and expeditious methods to define safety and efficacy of such agents so that the United States regulatory agency (Food and Drug Administration) could approve the use of such agents by clinicians. The vast number of new beta blocker and calcium antagonist agents being developed, as well as the long-term use abroad of many new drugs, has raised important questions as to how relative safety and efficacy of such agents can be determined to facilitate availability in the United States.

Progress has been particularly rapid in the past few years when M-mode echocardiography, and more recently 2-dimen sional echocardiography have been applied to the study ofHCM. In addition to new insights as to what the disease is, there has been enormous progress concerning its treatment, with the application of beta-adrenergic blocking agents and surgical relief of left ventricular outflow tract obstruction. Although these approaches have led to great strides in the symptomatic control of the disease, many patients' symptoms have remained refactory to medical and surgical therapy. Most discouragingly, sudden death still occurs, even in patients on large doses of beta-blocking agents and in patients who have had surgical relief of left ventricular outflow tract obstruction.

The patient who survives his first myocardial infarction carries approximately a tenfold risk of recurrence and sudden death when compared to the normal population. The concept of secondary prevention, therefore, has emerged as an active strategy aimed at the reduction of fatal and non-fatal recurrences of myocardial infarction. Apart from risk factors of relevance in primary prevention, secondary prevention is dependent on the extent of the disease itself; in other words the number of vessels involved, the extent of myocar dial damage and the degree of electrical instability. Nonmedical aspects such as the level of education, the degree of social support and the attitude towards stress also appear to influence the prognosis. The aim of this volume is to bring together all those factors relevant to achieving the maximal life span in patients afflicted with a disease that by its very nature is a lifelong process. However, as the late Paul Dudley White stated appropriately more than 40 years ago, it is not sufficient merely to add years to a life, one should also add life to the years. The scientific importance, therapeutic benefit and marketing potential of these compounds have caused an explosion of scientific literature describing their effects in many preclinical and clinical settings. The definitional characteristics of these compounds suggest a certain predictability of their biological profile but their therapeutic usefulness varies widely dependent upon their physical properties, net hemodynamic effects, duration of action and incidence of side effects. CEBs appear uniquely suited to the treatment of the underlying complexity of cardiovascular disease. The CEBs of the future may live up to the expectations of pathophysiologically based therapeutics and allow the heart and blood vessels to outlive the cells which they support. The development of CEBs is an evolving story of epic proportions and represents the cooperative efforts of individuals in all areas of science.
