Available in PDF, EPUB and Kindle. Book excerpt: Teach us to live that we may dread Unnecessary time in bed Get people up and we may save Our patients from an early grave. A most revealing paraphrase by Asher* of a verse by Bishop Thomas Ken more than adequately summarizes the plight of the immobilized patient, who often lies dormant and de pressed for years on end. In this volume, Dr. Steinberg has offered the reader a unique opportunity to share his many years of experience in caring for the immobilized patient. His careful attempt to explore the pathophysiologic effects of immobilization on a number of organ systems, combined with a host of practical aspects with regard to patient care, is unique and refreshing. This text should command the re spect of any physician faced with the vicissitudes and frus trations of caring for the immobilized. The final chapter de tailing "The Psychological Aspects of Immobilization," by Hammer and Kenan, offers the reader considerable insight into the essentials and value of occupational and physical therapy. It should prove most valuable to physicians as well "Asher, R. A. ]. Dangers of going to bed. Br. Med. Avioli St. Louis Preface It may be a paradox that the importance of disability and immobilization has been enhanced by the very progress of medical science.

Book excerpt: Since the publication of the first edition, the U.S. Surgeon General released the first-ever report on bone health and osteoporosis in October 2004. This report focuses even more attention on the devastating impact osteoporosis has on millions of lives. According to the National Osteoporosis Foundation, 2 million American men have osteoporosis, and another 12 million are at risk for this disease. Yet despite the large number of men affected, the lack of awareness by doctors and their patients puts men at a higher risk that the condition may go undiagnosed and untreated. It is estimated that one-fifth to one-third of all hip fractures occur in men. This second edition brings on board John Bilezikian and Dirk Vanderschueren as editors with Eric Orwoll. The table of contents is more than doubling with 58 planned chapters. The format is larger – 8.5 x 11. This edition of Osteoporosis in Men brings together even more eminent investigators and clinicians to interpret developments in this growing field, and describe state-of-the-art research as well as practical approaches to diagnosis, prevention and therapy. Book excerpt: The revised and the expanded second edition of this concise and targeted handbook presents the “need-to-know” musculoskeletal conditions and injuries required for all health care providers dealing with pediatric patients, including primary care physicians, emergency medicine physicians, radiologists, residents, students, nurses, and therapists, to brush up on pediatric orthopedics and sports medicine topics with ease. The most up-to-date information is contained within themed chapters--sports injuries and pediatric trauma, for example--as well as by anatomical region--the hip, knee, foot, spine, etc.--including chapters new to this edition covering pediatric musculoskeletal image interpretation and general aspects of sports medicine. This book addresses high-yield points needed for both general day-to-day pediatric orthopedics and sports medicine practice as well as information needed for the musculoskeletal section of the General Pediatrics Certifying Examination and other board exams for other specialties, physician assistants, nurse practitioners, and physical therapists. Well-illustrated and utilizing an easy-to-follow, bullet-pointed format, Pediatric Orthopedics and Sports Medicine gives the reader the basic knowledge to be able to identify and understand the management of common pediatric orthopedic and sports medicine conditions, and indications to either treat these conditions or refer the patient to an orthopedist.

This book is organized into eight chapters that review the body of information from studies on healthy volunteers conducted in direct support of the space program. Considerable chapters are devoted to the changes occurring in the cardiovascular system, bone and muscle, metabolism and endocrine responses, psychosocial responses, and exercise tolerance. Other chapters discuss the clinical effects and clinical management of deterioration while indicating the changes that have been found in health, normal bed rested subjects. The remaining chapters explore the data on crews that have flown in since their physiological responses are qualitatively similar to those observed in bed rested subjects or immobilized patients on earth. Clinicians, nursing staff, physiologists, researchers, and students in the field will find this book invaluable.

Available in PDF, EPUB and Kindle. Book excerpt: The purpose for this 30-day bed-rest study was to investigate the effects of short-term, high-intensity isotonic and isokinetic exercise training on maintenance of aerobic work capacity (peak oxygen uptake); muscular strength and endurance; and orthostatic tolerance, equilibrium, and gait. Other data were collected on muscle atrophy, bone mineralization and density, endocrine analyses of vasoactivity and fluid-electrolyte balance, muscle intermediary metabolism, and performance and mood of the subjects. Nineteen men (32-42 yr) were allocated into three groups: no-exercise control (peak oxygen uptake and isokinetic tests once/wk, N = 5), isotonic exercise training (electronic Quinton ergometer, supine, N = 7), and isokinetic exercise training (electronic Lido ergometer, supine, N = 7). The exercise training regimens were conducted near peak levels for 30 min in the morning and 30 min in the afternoon 5 d/wk. The protocol consisted of a 7-d ambulatory control period during which the subjects equilibrated on the standardized diet, 30 d of 6 degrees head-down bed rest, and a final 4.5 d of ambulatory recovery. Their diet consisted of commonly available fresh and frozen foods; mean caloric consumption of 2,678 +/- SE 75 kcal/d (control), 2,833 +/- SE 82 kcal/day (isotonic), and 2,890 +/- SE 75 kcal/d (isokinetic) resulted in mean weight losses during bed rest of 1.01 kg, 0.85 kg, and 0.0 kg, respectively. The results indicated that: (1) The subjects maintained a relatively stable mood, high morale, and high esprit de corps throughout the study. Scores improved in nearly all performance and mood tests in almost all the subjects. Isotonic training, as opposed to isokinetic exercise training, was associated with decreasing levels of psychological tension, concentration, and motivation, and with improvement in the quality of sleep. (2) Peak oxygen uptake was maintained during bed rest with isotonic exercise training; it was not maintained as well with isokinetic ( -9.0%) or no-exercise ( -18.2%) training. If a 9% reduction in aerobic power is acceptable, isokinetic exercise training could be used for maintenance of strength, endurance, and the reduced aerobic capacity in astronauts during flight. (3) In general, there were few decreases in strength or endurance of arm or leg muscles during bed rest, in spite of reduction in size (atrophy) of some leg muscles. (4) There was no effect of isotonic or isokinetic exercise training on orthostasis, because tilt-table tolerances were reduced similarly from 42-53 min to 30-34 min in the three groups following bed rest. (5) Bed rest resulted in significant decreases of postural stability and self-selected step length, stride length, and walking velocity, which were not influenced by either exercise training regimen. Pre-bed-rest responses were restored by the fourth day of recovery.

The pathophysiological, epidemiological and clinical aspects of hypercalcemia of malignancy are presented in this issue, with a focus on the recently discovered humoral factor responsible for the development of hypercalcemia. With a better understanding of the pathophysiology of this condition and the development of new potent drugs, capable of inhibiting bone resorption, especially bisphosphonates, the clinician will be more successful in correcting hypercalcemia in the great majority of patients.
