Nearly all adults who have ever smoked daily first tried a cigarette before 26 years of age. Current cigarette use among adults is highest among persons aged 21 to 25 years. The parts of the brain most responsible for cognitive and psychosocial maturity continue to develop and change through young adulthood, and adolescent brains are uniquely vulnerable to the effects of nicotine. At the request of the U.S. Food and Drug Administration, Public Health Implications of Raising the Minimum Age of Legal Access to Tobacco Products considers the likely public health impact of raising the minimum age for purchasing tobacco products. The report reviews the existing literature on tobacco use patterns, developmental biology and psychology, health effects of tobacco use, and the current landscape regarding youth access laws, including minimum age laws and their enforcement. Based on this literature, the report makes conclusions about the likely effect of raising the minimum age to 19, 21, and 25 years on tobacco use initiation. The report also quantifies the accompanying public health outcomes based on findings from two tobacco use simulation models. According to the report, raising the minimum age of legal access to tobacco products, particularly to ages 21 and 25, will lead to substantial reductions in tobacco use, improve the health of Americans across the lifespan, and save lives. Public Health Implications of Raising the Minimum Age of Legal Access to Tobacco Products will be a valuable reference for federal policy makers and state and local health departments and legislators.

Available in PDF, EPUB and Kindle. Book excerpt: States have banned smoking in workplaces, restaurants, and bars. They have increased tobacco tax rates, extended "clean air" laws, and mounted dramatic antismoking campaigns. Yet tobacco use remains high among Americans, prompting many health professionals to seek bolder measures to reduce smoking rates, which has raised concerns about the social and economic consequences of these measures. Retail and hospitality businesses worry smoking bans and excise taxes will reduce profit, and with tobacco farming and cigarette manufacturing concentrated in southeastern states, policymakers fear the decline of regional economies. Such concerns are not necessarily unfounded, though until now, no comprehensive survey has responded to these beliefs by capturing the impact of tobacco control across the nation. This book, the result of research commissioned by Legacy and Columbia University's Institute for Social and Economic Research and Policy, considers the economic impact of reducing smoking rates on tobacco farmers, cigarette-factory workers, the southeastern regional economy, state governments, tobacco retailers, the hospitality industry, and nonprofit organizations that might benefit from the industry's philanthropy. It also measures the effect of smoking reduction on mortality rates, medical costs, and Social Security. Concluding essays consider the implications of more vigorous tobacco control policy for law enforcement, smokers who face social stigma, the mentally ill who may cope through tobacco, and disparities in health by race, social class, and gender.

Available in PDF, EPUB and Kindle. Book excerpt: Despite heightened partisanship in the U.S. Congress and constituencies split along ideological lines, congressional representatives frequently buck their parties and seldom do precisely what voters ask. In Personal Roots of Representation, Barry Burden challenges standard explanations of legislative preferences to emphasize the important role that personal influences play in representatives' voting behavior. This timely book is the first to examine the extent to which the very same values, experiences, and interests that shape congressional members as individuals and guide their own life choices similarly shape their policymaking decisions. Burden takes a close look at legislative decision making in the areas of tobacco regulation, vouchers and school choice, and religion and bioethics. He finds that personal factors become more significant when legislators are acting proactively rather than reactively, grappling with specific policy issues, and defending rather than challenging the status quo. Marshaling both qualitative and quantitative evidence, Burden reveals that the personal roots of representatives' actions can be as influential as the usual suspects of partisanship and constituency--and that personal factors quite often have the greatest impact when the policymaking stakes are at their highest. Personal Roots of Representation is a provocative book that raises pressing new questions about legislative discretion and the accountability of our elected officials.

Book excerpt: Drug overdose, driven largely by overdose related to the use of opioids, is now the leading cause of unintentional injury death in the United States. The ongoing opioid crisis lies at the intersection of two public health challenges: reducing the burden of suffering from pain and containing the rising toll of the harms that can arise from the use of opioid medications. Chronic pain and opioid use disorder both represent complex human conditions affecting millions of Americans and causing untold disability and loss of function. In the context of the growing opioid problem, the U.S. Food and Drug Administration (FDA) launched an Opioids Action Plan in early 2016. As part of this plan, the FDA asked the National Academies of Sciences, Engineering, and Medicine to convene a committee to update the state of the science on pain research, care, and education and to identify actions the FDA and others can take to respond to the opioid epidemic, with a particular focus on informing FDA's development of a formal method for incorporating individual and societal considerations into its risk-benefit framework for opioid approval and monitoring.

Book excerpt: The anthrax incidents following the 9/11 terrorist attacks put the spotlight on the nation's public health agencies, placing it under an unprecedented scrutiny that added new dimensions to the complex issues considered in this report. The Future of the Public's Health in the 21st Century reaffirms the vision of Healthy People 2010, and outlines a systems approach to assuring the nation's health in practice, research, and policy. This approach focuses on joining the unique resources and perspectives of diverse sectors and entities and challenges these groups to work in a concerted, strategic way to promote and protect the public's health. Focusing on diverse partnerships as the framework for public health, the book discusses: The need for a shift from an individual to a population-based approach in practice, research, policy, and community engagement. The status of the governmental public health infrastructure and what needs to be improved, including its interface with the health care delivery system. The roles nongovernment actors, such as academia, business, local communities and the media can play in creating a healthy nation. Providing an accessible analysis, this book will be important to public health policy-makers and practitioners, business and community leaders, health advocates, educators and journalists.

Available in PDF, EPUB and Kindle. Book excerpt: Focusing on areas of public health practice in which the systematic application of epidemiologic methods can have a large and positive impact, this text applies traditional epidemiologic methods for determining disease etiology to the "real-life" applications of public health and health services research. Brownson and Petitti--true to their belief that epidemiology is the diagnostic discipline of public health--provide a much-needed book that bridges the gap between theoretical epidemiology and public health practice. Their uniquely comprehensive coverage includes a number of topics not traditionally addressed by other epidemiology texts. Twelve chapters offer a thorough, in-depth analysis of the important issues, methods, problems, and limitations within applied epidemiology. Following an introduction, three overview chapters detail study design and interpretation, outbreak and cluster investigations, and public health surveillance. The remaining chapters highlight key issues and provide practical recommendations on eight contemporary topics, such as community intervention trials, outcomes research, risk assessment, screening, and cost-benefit analysis. The editors--both very well-known researchers in the field--have compiled reviews with direct relevance to or a strong basis in epidemiology. All chapters have been authored by recognized authorities in the field of epidemiology and public health. Case studies of actual programs at the end of each chapter illustrate major points and provide a foundation for more detailed discussions. Applied Epidemiology is intended for practitioners in epidemiology, and students in epidemiology and related disciplines that rely heavily on epidemiologic methods and reasoning. It will be a practical and informative tool in academic institutions, federal agencies with significant educational missions, state and local public health agencies, and health care organizations.
