Book excerpt: Excerpt from Medical Education: An Address Delivered Before the Harvard Medical Alumni Association, June 26, 1894 Your President was kind enough, in his note ask ing me to be present on this happy occasion, to pro pose thati should speak on the subject of Medical Education. It is possibly a well-worn theme, espe cially before you, who have such elaborate reports, and I am glad to say such encouraging reports, fromyear to year of the progress of this great School; but there are still some points of value, it seems to me, which we can consider here. I remember very well indeed, in the days of the elder Gross, hearing da' nansemn of medical education and the progress that we ought to have, bushels of talk and thimblefuls of action; but, after all, when you consider it, these discussions, though they led at that time to very meagre action, were not without their results, and great results, too. They were slowly leavening the whole lump of the profession. They gradually made the profession the support of all the progress that we have seen; and I am sure that the medical schools, even, I believe, Harvard University itself, would never have taken the remarkable steps in advance which have been taken in the last few years, were it not for that very constant talk, that very constant working of the leaven throughout the profession. [applause] I trust the profession. I trust them profoundly. They have ever been better in that re spect than the schools till of late. [applause] There has been certainly a remarkable wave of progress passing over this country in the matter of medical education in the last few years. It has been demonstrated, first of all, by the creation of State Boards of Health, and especially by the noble Illinois State Board of Health, a body which has done more for medical education than any other, I believe, in this country [applause], because it fixed an advancedstandard. These boards now have been established in almost all the States; and they have been followed by a still more notable advance, namely, the estab lishment of State Boards of Medical Examiners, wholly independent, as they ought to be, of the med ical schools themselves. Again, another very remark able indication is that our universities and colleges all over the land are establishing distinct courses leading up to those of the various professional schools, medi cine among them. And what does this mean but that the medical schools want better men, and that the colleges are going to furnish them? In addition to this, another important indication in the same di rection, which Dr. Langmaid has just alluded to, is the establishment for the first time of a section of Medical Pedagogics in connection with the Pan American Medical Congress. I hailed with great delight another similar indication in the programme of the American Surgical Association last month in Washington, on seeing that one of the leading papers by the distinguished gentleman who will address you later, our friend Dr. Billings, of Washington, was enti tled Methods of Teaching Surgery. It developed what to my mind was one of the most fruitful, and to me personally one of the most useful, debates that was held in that body. To this end the Committee on Surgical Research has organized an investigation, in order to determine, so far as the opportunities in this community will allow, the extent to which blood-pressure observations in surgical cases may be of value from a clinical point of view. The committee will be glad to receive the suggestions of all surgeons who may be interested in the subject and they cordially invite your criticism of the following plan: (1) It is proposed that a representative of the committee be appointed for each of the hospitals here concerned, the Massachusetts General, Boston City and Children's. This representative of the committee will provide the apparatus necessary for the estimation of blood pressure in clinical cases, and will give such instruction to house officers and nurses as may be needed to obtain a general use of the apparatus. lie will, when desired, make observations himself and record results. Full information concerning these examinations can be pro cured upon application to the Surgeon General, U. S. Army, Washington, D. C. The essential requirements to securing an invitation are that the applicant shall be a citizen of the United States, shall be between 22 and 30 years of age, a graduate of a medical school legally authorized to confer the degree of doctor of medicine, shall be of good moral character and habits, and shall have had at least one year's hospital training, after gradu ation. The examinations will be held concurrently throughout the country at points where boards can be convened. Due con sideration will be given to localities from which applications are received, in order to lessen the traveling expenses of applicants as much as possible. The examination in subjects of general education (mathe metics, geography, history, general literature, and Latin) may be omitted in the case of applicants holding diplomas from rep ntable literary or scientific colleges, normal schools or high schools, or graduates of medical schools which require an en trance examination satisfactory to the faculty of the Army Medical School. Fourteen district supervisors' offices was created, and these offices with their sub-agencies covering the entire United States, reach practically into every county, so that prompt contact may be made with ex-service men and women everywhere. This organization has rendered inestimable service in reaching ex-service men and women, and giving them prompt care and attention. These offices were begun in a small and tentative manner, each with less than feet of floor space, one doctor and a small force, a little over a year ago. In March, 1921, most of these offices occupied from to feet of oor space, with a large medical and clerical personnel in the headquarters, and well organized sub-agencies throughout the districts. In carrying out its work for ex-service men and women the Service has assembled a large personnel. The medical personnel numbers about medical officers, exclusive of designated examiners on a fee basis. A dental corps has been created and numbers about 190 dental officers; a corps of female nurses has been created and numbers about a reconstruction service has been formed and numbers about 500 reconstruction aides; a dietetic service has been organized and numbers about 150 trained dietitians. Since the inception of this work, there have been cared for in hospitals by this Service. About patients of the \var Risk Insurance Bureau, who were furnished a total of about hospital relief days. Also. About out-patient treatments have been furnished, and a total of nearly medical examinations have been made. Special services of various kinds have been arranged. For example, about patients have been given dental treatment. By May, 1991, over patients were being given occupational therapy and over physio-therapy each week.
