Available in PDF, EPUB and Kindle. Book excerpt: Why did President Clinton's efforts to reform the financing of American health care fail? For years to come, politicians and scholars of public policy will revisit the debate over Clinton's health care plan. What did planners do right? And what did they do wrong? How can the mistakes of that experience be avoided in the future? What steps can now be taken to achieve some measure of reform in smaller pieces? In The Problem That Won't Go Away, economists, political scientists, sociologists, public opinion experts, and government staff offer answers to these and other crucial questions. They recount the history of the Clinton health care plan, present several alternative strategies the administration might have pursued, and conclude that none was likely to achieve the administration's goals of universal coverage and cost containment. Many support the view that the administration, Congress, and the nation lacked the political consensus and the information to credibly describe the effects of any single bill to reform the U.S. health care system. In that case, was the only option available to the administration to reach for goals far more modest than those it sought? Health care financing as a national political issue will not go away. Pressure to cut public spending to balance the budget means that medicare and medicaid will stay in the legislative spotlight; the retirement of the baby-boom generation in the beginning of the next century promises large increases in the cost of medicare; and a flood of new and costly medical technologies will continue to put financial pressure on everyone responsible for paying for health insurance. But, as this book illustrates, the nature of the debate in the years after the demise of the Clinton plan will be altogether different from that of the past several decades.

Available in PDF, EPUB and Kindle. Book excerpt: During the 1992 presidential campaign, health care reform became a hot issue, paving the way for one of the most important yet ill-fated social policy initiatives in American history: Bill Clinton's 1993 proposal for comprehensive coverage under "managed competition." Here Jacob Hacker not only investigates for the first time how managed competition became the president's reform framework, but also illuminates how issues and policies emerge. He follows Clinton's policy ideas from their initial formulation by policy experts through their endorsement by medical industry leaders and politicians to their inclusion--in a new and unexpected form--in the proposal itself. Throughout he explores key questions: Why did health reform become a national issue in the 1990s? Why did Clinton choose managed competition over more familiar options during the 1992 presidential campaign? What effect did this have on the fate of his proposal? Drawing on records of the President's task force, interviews with a wide range of key policy players, and many other sources, Hacker locates his analysis within the context of current political theories on agenda setting. He concludes that Clinton chose managed competition partly because advocates inside and outside the campaign convinced him that it represented a unique middle road to health care reform. This conviction, Hacker maintains, blinded the president and his allies to the political risks of the approach and hindered the development of an effective strategy for enacting it.

Available in PDF, EPUB and Kindle. Book excerpt: This report was prepared to accompany President Clinton's first address to a Joint Session of Congress. It describes in detail the comprehensive economic plan being proposed by the new administration for the nation. The plan has three key elements: economic stimulus to create jobs now while laying the foundation for long-term economic growth; long term public investments to increase the productivity of people and businesses; and a balanced deficit-reduction plan to prevent the drain of private investments that generate jobs and increase incomes. The text is organized into four sections: (1) "A New Direction" (a brief 3-page preamble); (2) "A Legacy of Failure" (a 16-page statement of the problem, under subheadings such as "Skyrocketing Health Care Costs"); (3) "What We Must Now Do" (a 92-page statement of the solution, under subheadings such as "Investing in the Future: Reducing the Deficit To Increase Private Investment" and "Restoring Fairness"); and (4) "The Task Remaining" (a brief 6-page wrap-up). A closing Appendix contains 25 pages of statistical tables outlining various discretionary program savings, proposed changes to mandatory programs, stimulus proposals, investment proposals, and revenue and receipts proposals. Most tables provide figures for each year for the 6-year period 1993-1998. The field of education is touched upon in the report at six locations: (1) "Relative Earnings by Education for 25-34 Year Olds" (Chart 2-9, p. 18); (2) Chapter 1 Compensatory Education (p.31); (3) Pell Grants (p. 32); (4) "Lifelong Learning," covering full funding of Head Start and related child care funding and Medicaid, National Service, Dislocated Workers Program, Job Corps Expansion, Summer Youth Employment and Training Program, Youth Apprenticeship, and various Department of Education reforms and initiatives (p. 57-59); (5) Impact Aid "b" Projects (p. 87); and (6) Reform of Student Loan Programs (p. 92). Available in PDF, EPUB and Kindle. Book excerpt: In no other country has health care served as such a volatile flashpoint of ideological conflict. America has endured a century of rancorous debate on health insurance, and despite the passage of legislation in 2010, the battle is not yet over. This book is a history of how and why the United States became so stubbornly different in health care, presented by an expert with unsurpassed knowledge of the issues. Tracing health-care reform from its beginnings to its current uncertain prospects, Paul Starr argues that the United States ensnared itself in a trap through policies that satisfied enough of the public and so enriched the health-care industry as to make the system difficult to change. He reveals the inside story of the rise and fall of the Clinton health plan in the early 1990sùand of the Gingrich counterrevolution that followed. And he explains the curious tale of how Mitt RomneyÆs reforms in Massachusetts became a model for Democrats and then follows both the passage of those reforms under Obama and the explosive reaction they elicited from conservatives. Writing concisely and with an even hand, the author offers exactly what is needed as the debate continuesùa penetrating account of how health care became such treacherous terrain in American politics.
