Available in PDF, EPUB and Kindle. Book excerpt: Over the past 50 years many in vitro and in vivo drug response assay systems have been developed to determine the potential - tivity of chemotherapy agents. The idea was to eliminate ineffective agents and unnecessary toxic treatment while selecting drugs active in vitro or in the mouse model that might increase the probability of response in the patient. None of these test models, however, achieved routine clinical application in the past. This might be at least in part - lated to large discrepancies that were described between the s- cess rate of the assay systems and the clinical benefit in cancer - tients. The heterogeneity of chemosensitivity that exists between different tumors as well as between individual tumor lesions may be one explanation for these findings. Furthermore, different assay end points such as proliferation, metabolism, and vitality were - veloped to evaluate the effects of cytostatic drugs on tumor cells, and these might be related to the differing results. However, knowledge about procedures for assay-assisted treatment selection has increased rapidly within the past few years, and several studies suggest that test-directed chemotherapy selection now may - prove response rates and survival in various types of tumors. The International Society for Chemosensitivity Testing in - cology (ISCO) was founded to promote, coordinate, and improve clinical and laboratory research in the field of predictive drug te- ing in human tumor cells.

Available in PDF, EPUB and Kindle. Book excerpt: Internationally acclaimed integrative medical oncologist James W. Forsythe, M.D., H.M.D., has praised Diana Warren for courage and tenacity in her arduous battle against Stage II breast cancer. Doctor Forsythe insists that "Diana deserves kudos from people around the world, for refusing to subject her body to unnecessary standard chemo and radiation that unscrupulous allopathic oncologists kept trying to require her to endure." Featured in many hot-selling books written by others including media personality Suzanne Somers, and also the author of many successful books himself, Doctor Forsythe has hailed Diana for her dogged determination amid a non-stop quest to find effective natural, non-toxic treatments as her health deteriorated, while also hunting for an experienced doctor licensed to avoid high-dose poisonous options. "I strongly recommend that everyone who suffers from cancer read Diana's compelling tale," Doctor Forsythe said. "Today, Diana is playing a formidable role in helping to lead the way as a true champion in a quest to inform the public of the urgent need to avoid toxic treatments when feasible in certain circumstances, and how to find acceptable natural remedies-plus the importance of avoiding 'standard-of-care' chemo and radiation when under the care and guidance of a qualified, licensed medical professional." After reading many hundreds of books on cancer, Doctor Forsythe hails Diana's publication as being "by far among the best from the viewpoint of a person who never attended medical school. Readers undoubtedly will benefit when learning of the exceptional example Diana has set for us all."

Book excerpt: The results of randomized trials evaluating the use of early or adjuvant systemic treatment for patients with resectable breast cancer provide an eloquent rebuttal to those who would argue that we have made no progress in the treatment of cancer. Many of the tumors that we have been most successful in curing with chemotherapy and other newer forms of treatment are relatively uncommon. In contrast, breast cancer continues to be the single most common malignancy among women in the western world, is increasingly a cause of death throughout Asia and Third-World countries, and remains one of the most substantial causes of cancer mortality world wide. The use of mammography as a means of early detection has been shown to reduce breast cancer mortality by 25-35% among those popu lations in which it is utilized. The use of adjuvant systemic treatment in appropriate patients provides a similar (and additional) reduction in breast cancer mortality. Few subjects have been so systematically studied in the history of medicine, and it seems fair to conclude that the value to adjuvant systemic therapy in prolonging the lives of women with breast cancer is more firmly supported by empirical evidence than even the more conventional or primary treatments using various combinations ofsurgery and radiotherapy.

Book excerpt: A state-of-the art collection of readily reproducible laboratory methods for assessing chemosensitivity in vitro and in vivo, and for assessing the parameters that modulate chemosensitivity in individual tumors. Chemosensitivity, Volume 1: In Vitro Assays provides a panel of 16 in vitro measures of chemosensitivity in adherent and non-adherent cells for single agents and combinations of agents. In addition to immunohistochemical and imaging approaches, these assays include clonogenic, colorimetric, fluorometric, and physiological assays. Highlights include image analysis to assess drug sensitivity, high throughput approaches using green fluorescent protein, DIMSCAN (a microcomputer fluorescence-based assay), and the ChemoFx assay used in biotechnology. A companion volume, Volume 2: In Vivo Models, Imaging, and Molecular Regulators, provides protocols for classifying tumors into response categories and customizing chemotherapy regimens to individual patients.

Book excerpt: From the book jacket: Chemo Brain. Fatigue. Chronic Pain. Insomnia. Depression. These are just a few of the ongoing, debilitating symptoms that plague some breast cancer survivors long after their treatments have officially ended. After The Cure is a compelling read filled with fascinating portraits of women who are living with the aftermath of breast cancer. Having heard repeatedly that the problems are all in your head, many don't know where to turn for help. The doctors who now refuse to validate their symptoms are often the very ones they depended on to provide life-saving treatments. Sometimes family members, who provided essential support through months of chemotherapy and radiation, don't believe them. Their work lives, already disrupted by both cancer and its treatment, are further undermined by the lingering symptoms. And every symptom is a constant reminder of the trauma of diagnosis, the ordeal of treatment, and the specter of recurrence. Most narratives about surviving breast cancer end with the conclusion of chemotherapy and radiation, painting stereotypical portraits of triumphantly healthy survivors, women who not only survive but emerge better and stronger than before. After The Cure allows us to hear the voices of those who are silenced by the optimistic breast cancer culture, women who live with a broad array of health problems long after therapy ends. Here, at last, survivors step out of the shadows and speak compellingly about their real stories, giving voice to the complicated, often bittersweet realities of life after the cure.

Available in PDF, EPUB and Kindle. Book excerpt: In the last two decades, there has been a major increase in the actual numbers of women diagnosed with breast cancer (from 65000 new cases 20 years ago in the United States to 183000 in 1993). Concomitant with this has been a shift toward making the diagnosis earlier and toward diagnosing an earlier stage ofthe disease (more than 70% of diagnoses were of advanced-stage disease two decades ago, whereas more than 70% of diagnoses are now earlier-stage disease in 1992). These changes are clearly related to the woman's greater sensitivity to abnormalities in the breast and her seeking medical care, to the physician's greater awareness of the potential diagnosis of cancer, and to the major and important impact of mammographic screening. These changes have had a major impact on the questions of appropriate treatment, thereby contributing to the controversies in the disease. What is the role of surgery? What are the respective advantages of radical mastectomy, modified radical mastectomy, and conservation surgery with and without supplementary treatment? What is the role of radiation therapy? When should radiation therapy be performed after mastectomy, and what volumes should be irradiated post mastectomy? Where is radiation therapy indicated after conservation surgery, and when is radiation therapy an effective palliative treatment measure? When should chemotherapy and/or hormonal therapy be used as an adjuvant to primary treatment and when as an effective palliative treatment? This book by Levitt and Fletcher addresses these critical and important issues.
