Available in PDF, EPUB and Kindle. Book excerpt: Complex disorders of the carbohydrate metabolism and associated complications cause many abnormalities detectable by radiography in the bones and joints. Mild clinical symptoms associated with very severe radiological changes were first rec ognized in relation to the gastroenterologic complications of diabetes. This phenomenon is more frequent in the skeletal system. For example, mild and painless swelling of the foot joints may often mask extremely severe bone destruction. Several other bone changes associated with diabetes are only detectable by radiography. Thus, the radiologist plays an important role in confirming these diabetic complications, furthermore he is involved in the therapeutic management of the patient. Although many details on this subject have been published, however no summarizing monograph has yet appeared. Manuals discussing diabetes include only short reviews on complications of the osseous system. The fact that the incidence of diabetes is very high, at present 1 %-2 % of the population is affected and their number is gradually increasing - dis plays the timeliness of this subject. Fifty years of experience with insulin therapy indicates that several important problems still remain to be solved. Insulin and modern oral antidia betic drugs proved extremely efficient in the management of hyperglycemia and ketosis, but the incidence of other complications has not decreased. Moreover, as the number of diabetics and their life expectancy increase, late complications become likewise more fre quent. Diabetic osteoarthropathy is one of these complications.

Available in PDF, EPUB and Kindle. Book excerpt: In the past decade there has been a major sea change in the way disease is diagnosed and investigated due to the advent of high throughput technologies, such as microarrays, lab on a chip, proteomics, genomics, lipomics, metabolomics etc. These advances have enabled the discovery of new and novel markers of disease relating to autoimmune disorders, cancers, endocrine diseases, genetic disorders, sensory damage, intestinal diseases etc. In many instances these developments have gone hand in hand with the discovery of biomarkers elucidated via traditional or conventional methods, such as histopathology or clinical biochemistry. Together with microprocessor-based data analysis, advanced statistics and bioinformatics these markers have been used to identify individuals with active disease or pathology as well as those who are refractory or have distinguishing pathologies. New analytical methods that have been used to identify markers of disease and is suggested that there may be as many as 40 different platforms. Unfortunately techniques and methods have not been readily transferable to other disease states and sometimes diagnosis still relies on single analytes rather than a cohort of markers. There is thus a demand for a comprehensive and focused evidenced-based text and scientific literature that addresses these issues. Hence the formulation of Biomarkers in Disease. The series covers a wide number of areas including for example, nutrition, cancer, endocrinology, cardiology, addictions, immunology, birth defects, genetics, and so on. The chapters are written by national or international experts and specialists.

Available in PDF, EPUB and Kindle. Book excerpt: Public and political concern about the increasing prevalence of diabetes has prompted major concern about treatment of patients with the condition. Foot complications are some of the commonest causes of hospitalisation of people with diabetes and if not treated well often lead to amputation. There is evidence that 85% of these amputations can be prevented by better understanding of the problem and by multi-disciplinary teams working more effectively together. This has been recognised and NICE have recently published guidelines on diabetic foot complications as have Diabetes UK and NHS Diabetes. These have been successful in raising awareness of the problem but the local multi-disciplinary teams need clear practical advice on how to manage the foot in diabetes and deliver high quality care. With the current interest in improving outcomes for patients with foot complications this is an ideal time to make a practical evidence-based handbook available. This book will provide clear practical guidelines on how to manage all aspects of the foot in diabetes as well as an in-depth analysis of the most recent evidence. The book will be based on care pathways with algorithms for each section so it would be of practical value in any clinic in primary or secondary care. It will appeal to a wide range of health care professionals treating people with diabetes: vascular surgeons and trainees, orthopaedic surgeons, diabetes specialist nurses, podiatrists and tissue viability nurses.

Available in PDF, EPUB and Kindle. Book excerpt: The aim of this book is to review the latest findings on musculoskeletal disease associated with diabetes. It has been increasingly recognized that maintaining skeletal health is an important factor in achieving longevity in healthy subjects. Diabetes has been established as a disease independently associated with sarcopenia and increased risk of bone fracture resulting from osteoporosis; therefore, it is reasonable to maintain that musculoskeletal health is important in preserving good health. As either bone or muscle is intimately involved in the regulation of metabolic status, keeping the musculoskeletal system healthy is important for improving abnormal glucose metabolism toward normal levels and is vital, as well, for maintaining normal activities of daily living. Muscle is a target of insulin for enhancing the entry of glucose; thus it is conceivable that sarcopenia and muscle containing fat streaks causes insulin resistance in diabetic patients. Furthermore, bone is an organ that regulates Ca and Pi levels in serum by releasing or resorbing Ca and Pi to and from bone tissue. It is now known that bone is a definitive endocrine organ for regulating glucose metabolism and Pi metabolism by secreting osteocalcin and FGF-23 from osteocytes/osteoblasts. Readers will learn of the recent findings in this area, and this book will benefit physicians who deal with diabetes, particularly orthopedists and bone specialists, as well as all physical therapists.

Available in PDF, EPUB and Kindle. Book excerpt: Physical movement has a positive effect on physical fitness, morbidity, and mortality in individuals with diabetes. Although exercise has long been considered a cornerstone of diabetes management, many health care providers fail to prescribe it. In addition, many fitness professionals may be unaware of the complexities of including physical activity in the management of diabetes. Giving patients or clients a full exercise prescription that take other chronic conditions commonly accompanying diabetes into account may be too time-consuming for or beyond the expertise of many health care and fitness professionals. The purpose of this book is to cover the recommended types and quantities of physical activities that can and should be undertaken by all individuals with any type of diabetes, along with precautions related to medication use and diabetes-related health complications. Medications used to control diabetes should augment lifestyle improvements like increased daily physical activity rather than replace them. Up until now, professional books with exercise information and prescriptions were not timely or interactive enough to easily provide busy professionals with access to the latest recommendations for each unique patient. However, simply instructing patients to “exercise more” is frequently not motivating or informative enough to get them regularly or safely active. This book is changing all that with its up-to-date and easy-to-prescribe exercise and physical activity recommendations and relevant case studies. Read and learn to quickly prescribe effective and appropriate exercise to everyone.
