Osteoporosis also results in loss of bone mineral density throughout the body, including the maxilla and mandible. Successful osseointegration of dental implants is attributed to their ability to integrate well with bone. The influence of bone quality on dental implant osseointegration has been discussed in several studies, and higher rates of dental implant failure have been reported in patients with low bone quality and an inadequate bone volume. Osteoporosis represents a risk factor for osseointegration, and this relationship may be derived from the association of the disease with a deficiency in bone formation. This condition would compromise the healing capacity and the apposition of bone at the implant interface. Currently, there is no clear consensus regarding dental implant treatment in osteoporotic individuals. Studies have revealed contradictory reports regarding the success and failure of dental implants in patients with osteoporosis. Antiresorptive agents have been widely used to treat osteoporosis. Dental implant placement in patients on bisphosphonate therapy may trigger osteonecrosis of the bone. Hence, it is important to analyze factors that have to be taken into consideration prior to implant therapy in patients with osteoporosis and those undergoing treatment. This chapter outlines dental implant osseointegration under osteoporotic conditions. The possible effect of bisphosphonate therapy on dental implant survival will also be discussed based on the current literature.

Therefore we must pay attention to bone quality along with bone density and volume upon diagnosis. A major determinant of Bone quality is collagen quality that may influence the bone tissue responses and metabolism. However, the influence of bone quality in terms of collagen quality on the establishment of osseointegration has not been well understood Aim: To clarify the influences of collagen crosslink deficiency on bone response at the implant and its surrounding tissues in vivo.Materials and Methods: Four weeks old Wister male rats (n=24) were divided into test (n=12) and control groups (n=12). Animals were group housed in standard cages with free access to standard chow and water. To inhibit enzymatic cross-link formation in vivo, beta-aminopropionitrile (BAPN) was used. Test group were given water containing 0.1% BAPN and continued until the day of sacrificing. Control group were given normal water. After 4 weeks, all rats received a custom made machined surface titanium implants in both right and left tibias (n= 48). At 1,2,4 weeks after the surgery, rats were sacrificed and the specimens were prepared from retrieved tibias for decalcified histological sections and observed under a microscopy. All the animal experiments were conducted in compliance with the protocol reviewed by the Institutional Animal Care and Use Committee and approved by the President of our University. Results: Remodeling of the bone tissue adjacent to the installed implants in the cortical area seemed to be more active in the test group compared to the control yet, the quality and integrity of newly formed bone is questionable in the BAPN group. The Bone Implant Contact (BIC) was significantly higher in the control group. The total volume of newly formed bone in bone marrow area was higher in control group than test group. Interestingly, a tremendous amount of immature bone and chondritic tissue was formed on the outer surface of the bone in both groups. The number of adipocytes in bone marrow was much higher in test group through out the 3 time points. Conclusion: Collagen quality affected the bone tissues reactions in vivo and might affect the establishment of osseointegration in turn. Further studies would be required to clarify the direct evidence of these differences and to establish clinical implications.
