However, little is known regarding rowing exercise and spine bone mineral density in older women. The aim of this study was to examine differences in spine bone mineral density (BMD) and back strength between premenopausal and postmenopausal competitive female masters rowers (n=28, 45.5 ± 4.7 yrs, n=28, 56.1 ± 5.7 years, respectively) and age-matched non-rowers (n=30, 43.3 ± 4.2 yrs; n=26, 56.8 ± 4.8 years). Competitive rowers were recruited from nine rowing clubs in the local area and compared to controls recruited from the same region who were normally active but not participating in rowing activity. Participating rowers had been engaged in competitive rowing for a minimum of one year. The average years spent rowing for the premenopausal and postmenopausal groups was 7.5 ± 6.6 yrs and 5.9 ± 6.9 yrs, respectively. BMD (g/cm2) of the third lumbar vertebrae (L3) was measured by dual-energy x-ray absorptiometry (DXA) in both the anterior-posterior and lateral views. Back strength was assessed using a standing cable tensiometer. Subjects also completed questionnaires to assess diet, physical activity, medical history and rowing history. Differences in BMD and back strength between groups were determined by analysis of covariance, controlling for lean mass. Compared to controls, postmenopausal rowers had 3.2% higher BMD at the anterior-posterior spine (p=.02) and 4.4% higher lateral spine BMD (p=.04). Furthermore, isometric back strength was 22.6% greater in these rowers than controls (p=.01). In contrast, controls had higher lateral BMD than rowers, with no differences in AP spine BMD or back strength between the premenopausal rowers and controls. Back strength was a significant predictor of AP spine BMD in premenopasual rowers and controls (R2=0.137, p=0.004) and of lateral spine BMD in postmenopausal rowers only (R2=0.153, p=0.04). There were no differences in calcium intake, age, menopausal status, weight, or lean mass between rowers and controls in either the premenopausal or postmenopausal samples. Since both increased BMD and back strength are associated with reductions in vertebral fracture risk, our results suggest that rowing exercise may be an important strategy to promote bone health and reduce vertebral fracture risk in postmenopausal women. However, the forces applied in rowing may not be great enough to alter bone mass before the onset of menopause. Therefore more research is needed examining rowing exercise in these older populations.

Book excerpt: Exposures during childhood that affect bone mineral density (BMD) and bone mineral content (BMC) could have long-term effects on osteoporosis development and associated fracture risk. We evaluated associations of physical activity in late childhood, adolescence, and in young adulthood with young adult BMD and BMC using data from the Dietary Intervention Study in Children (DISC) and DISC06 Follow-up Study. Moderate/Intense physical activity (MET hours/week) was assessed by questionnaire on five occasions during childhood/adolescence and again during young adulthood. BMD and BMC were measured by dual energy x-ray absorptiometry (DEXA) scans during young adulthood. Linear mixed models were used to analyze the association of physical activity with BMD and BMC during young adulthood. Generalized linear latent affects mixed models were used to evaluate the association of the latent variable "childhood physical activity" with BMD and BMC in young adulthood. Young adult BMC and BMD increased with increasing physical activity during young adulthood after adjustment for body weight, although the test for trend was significant only for BMD. Young adult BMC increased significantly with increasing physical activity during youth. Promotion of physical activity in youth and young adulthood could potentially decrease the magnitude of osteoporosis in older women as a public health problem, but more research is needed.

Book excerpt: A meta-analysis was done to determine what effect exercise has on the bone mineral density (BMD) of the lumbar spine, proximal femur, and distal radius in postmenopausal women, utilizing data from 18 studies. Treatment effects (TE) were calculated for each of the skeletal sites, aerobic and strength training, as well as methods of BMD measurement (dual energy x-ray absorptiometry and dual photon absorptiometry). The resulting TEs found exercise, in general, to significantly increase (p c 0.05) the BMD of the lumbar spine (0.73%), and the proximal femur (0.35%); however, the distal radius showed a decrease ( -0.91%) in BMD. When looking for the effect of aerobic versus strength training, significant (p c 0.05) increases in BMD were found in the spine and femur. Since positive changes were not seen in all sites, the benefits of exercise may be specific to the skeletal sites which the working muscles are attached. When the % change for exercise and control groups were analyzed separately, the exercise group increased 2.31% at the lumbar spine and 0.61% at the proximal femur. However, the exercise group decreased -2.21% at the radius. The control group decreased at the lumbar spine -1.09%, and at the femur -1.1 I%, however, increased at the radius 0.36%. The BMD % changes were significantly larger for the spine and femur, therefore, vigorous exercise along with resistance training may help slow the decrease in BMD as one ages.

Book excerpt: This study used data from the Third National Health and Exam Survey in order to examine the association between weight cycling and low bone density. A household interview and a physical examination were conducted for each survey participant. Physical examinations were conducted in specially equipped mobile examination centers (MEC); examinations were conducted in specially equipped mobile examination centers (MEC); were taken using the DXA by trained technicians. After combining the data from the exam, lab, and adult household interview surveys there were 31,808 observations available for this study. Acceptable bone mineral density (BMD) scans were performed on 14,646 men and women aged 20 and older. Only non-Hispanic white females aged 40-69 years of age were included in the current study. This left a sample size of 1348 women. From the home interview information on health practices, alcohol, tobacco, drug use, and physical activity was obtained from study participants. A family questionnaire asked questions about race and ethnicity, family income, and other demographic characteristics of the household. As part of the exam component women were asked several questions which were used to determine menopausal status. Univariate analyses and multivariate analyses were used to examine the independent and adjusted associations between risk factors and the outcome. Significant independent associations with bone mineral density (BMD) were found with weight cycling, age category, menopause, body mass index (BMI), maternal family history of hip fracture or osteoporosis, and smoking. Prior fracture, exercise, and alcohol use were not statistically significant. Weight cycling, BMI, and exercise had unadjusted T-score means positively associated with BMD; age, menopause status, maternal family history of hip fracture or osteoporosis, prior fracture, smoking, and alcohol use had unadjusted T-score means negatively associated with BMD. Because all covariates were well known risk factors for low bone density they were left in the final model. In the multivariate analysis of covariance model, weight cycling was statistically significant (p-value .0057) and had adjusted T-score means which were positively associated with BMD.
