Peak bone development is thought to be achieved in the young adult years of an individual's life and can be indicative of risk of osteoporosis later in life. This study aims to explore the relationship between bone mineral density (BMD) and body composition measurements from a college aged non-student athlete population (n = 61) to that of a student-athlete population (n = 39). Furthermore, sex specific differences between and within the subgroups were evaluated. No statistical difference was found for BMD, weight, lean mass, and bone mineral content between subgroups. However, sex-specific differences were present for all variables of interest. Pearson's correlation coefficient results indicate that there was a moderate to strong relationship between BMD and lean mass for all groups.

Book excerpt: We studied the role of body composition on C-reactive protein (CRP) and interleukin-6 (IL-6); determine the differences in CRP and IL-6 among HT users and non-users; determine the effect of 12-months of resistance-training and resulting body composition changes on IL-6 and CRP; determine the relationship between BMD and IL-6, CRP and creatine kinase (CK). Sedentary women (N=208, age 44-66, postmenopausal) taking HT (N=106) or not taking HT (N=102) were randomly assigned to resistance-training: HT-exercise (N=55), HT-no exercise (N=45), no HT-exercise (N=53), and no HT-no exercise (N=49). The program included three weekly 60-75 minute sessions of 8 exercises performed in 2 sets of 6-8 repetitions at 70-80% of 1RM. Total fat mass (TFM), lean soft tissue mass (LSTM) and BMD were assessed by dual-energy X-ray absorptiometry (DXA). For each subject, baseline and 1-year IL-6, CRP and CK were measured. High TFM, high IL-6 and HT use were independently associated with high CRP levels. A stronger relationship between IL-6 and AFM compared to TFM was found in HT users. High TFM and LSTM were significantly related to higher IL-6 levels. A stronger relationship between CRP and AFM compared to TFM was found in HT non-users. CRP was higher in HT users (5.47ł5.40 mg/L) compared to non-users (2.70ł3.05 mg/L) and was higher in oral (5.76ł5.29 mg/L opposed; 6.14ł5.97 mg/L unopposed) compared to transdermal HT users (2.65ł4.44 mg/L). CRP increased slightly (p=0.077) in exercisers (0.54 ł 0.34 mg/L) not taking HT compared to controls ( -0.39 ł 0.35 mg/L). Reductions in TFM were associated with reductions in IL-6 and CRP in HT users. CK was positively associated to all BMD sites at baseline. IL-6 change was positively associated with change in femur neck BMD. CRP change was inversely correlated with change in lumbar spine BMD. CK change was directly related to change in total body and femur trochanter BMD. Reductions in TFM were accompanied by reductions in IL-6 and CRP; AFM was more strongly correlated with inflammation than TFM; 12-months of resistance-training did not decrease IL-6 or CRP; IL-6 and CK were positively related to BMD, and CRP was inversely associated with BMD.

The overall purpose of this study was to examine the relationship between body composition, including bone mineral density (BMD) and weight/body mass index (BMI) with functionality performance in 37 overweight (BMI of 31.3"4.8 kg/m2) postmenopausal Caucasian women, aged 54.7"3.8 y. Participants completed functionality tests including: 8 meter normal and brisk walks, 8 foot get up-and-go, handgrip strength assessment, one leg stance, and the chair-stand test. Dual X-ray absorptiometry technique with a Lunar iDXA instrument (GE Medical Systems, Madison, WI) was used to measure BMD of the total body, lumbar spine, dual femur (neck, Ward's triangle, trochanter, shaft, and total) and forearm (ulna and radius at ultradistal and 1/3 distance). BMI was less closely correlated with various functionality measures than age but was most correlated with ability to balance on one leg (r=-.465, P=.004) and normal step length (r=.354, P=.032). Physical activity (PA) is an important step in preventing osteoporosis and is one of the major determinants of peak bone mass during adolescence. However, type of PA plays an important role when examining the effects of PA on bone mineral density (BMD). PURPOSE: 1) To determine the differences in BMD among adolescent female tennis players (TP) and non-tennis players (NTP); 2) to determine the differences in body composition (BC) between adolescent female TP and NTP; 3) to determine whether BC variables (weight, fat mass, and lean mass) were predictive of differences in BMD; and 4) to assess osteoporosis knowledge among female high school adolescents. METHODS: Nineteen female TP and 19 female NTP, aged 14 to 18 years, participated in this study. Lumbar spine, total hip, femoral neck, and left and right forearm BMD, and BC were assessed using dual-energy x-ray absorptiometry. Total osteoporosis knowledge (TOK) was assessed by the Osteoporosis Knowledge Test. One-way ANOVA was used to assess differences between TP and NTP for BMD, BC, and OKT . Multiple regression analysis was used to identify significant predictors of BMD. RESULTS: TP had significantly greater femoral neck BMD than NTP (p = 0.017). In addition, when data were analyzed with an outlier excluded, TP also had significantly greater total hip BMD than NTP (p = 0.02). Although TP had greater BMD for lumbar spine, and dominant arm measurements, these differences were not significantly greater than NTP. There were no significant differences for total body percent fat (p = 0.72), total body lean mass (p = 0.07) or total body fat mass (p = 0.59) between the groups. Collectively, body composition variables (body weight, total body lean mass and total body fat mass) significantly predicted BMD at the femoral neck and hip with lean mass being the best predictor among the three independent variables. A moderate level of knowledge related to osteoporosis was found among female adolescents. In addition, there were no significant differences between TP and NTP for TOK. CONCLUSION: This study suggests that adolescent female TP have greater femoral neck and total hip BMD than NTP. This difference might play an important role in preventing osteoporosis and decreasing the risk of fractures at the hip later in life.

These tools are limited in that they may not include all risk factors, or may lack details of some risk factors. Tools are dependent on the accuracy of the epidemiological data used to derive them and tools validated in other populations may not apply to the UK. Two tools, FRAX and QFracture, are available for use in the UK. It is not clear whether these tools are equally accurate and whether choice of tool should depend on circumstances. This short clinical guideline aims to provide guidance on the selection and use of risk assessment tools in the care of people who may be at risk of fragility fractures in all settings in which NHS care is received.
