Book excerpt: Liver metastases are a frequent and often fatal occurrence in cancer patients, particularly those with malignancies of the gastrointestinal (GI) tract. While recent improvements in surgical techniques and a more aggressive approach to resection of liver metastases have improved long term survival for some patients, most patients with hepatic metastases still succumb to their disease. To improve these dismal statistics, a better understanding of the biology of liver metastasis, particularly the early stages that can be targeted for prevention, is essential. Once cancer cells enter the liver, several different scenarios may occur. The cancer cells may be immediately destroyed by local defence mechanisms, they may enter a state of dormancy as solitary cells and never produce a metastasis, initiate a short-lived process of proliferation that is aborted before a metastasis is established or actively proliferate to form macrometastases. The chapters in Part I of this book provide insight into the cellular/molecular mechanisms that determine which of these scenarios prevails. Written by experts researchers in the filed of metastasis, these chapters provide state-of-the art reviews on the cellular and molecular processes that impact the early stages of the metastatic process. The unique microenvironment of the liver, its various anatomical, cellular and molecular features and the impact they have on metastasis are highlighted. This first Metastasis Congress in the FRG was organized in conjunction with the German Association of Cancer Research (SEK) of the German Cancer Society. The congress topic generated tremendous interest and attracted about 400 scientists from 22 countries. Most participants came from Europe, Israel, and the United States. Why did we organize the Metastasis Congress? Only about 50% of all people who develop some form of cancer are curable. Despite improved patient care and increasingly innovative and effective techniques for diagnosing and treating primary cancers, the development of secondary cancer colonies, i. e. , metastasis, can not be prevented and is the major cause of death. In the Federal Republic of Germany there are still as many as 160 000 cancer patients per year who succumb to their disease, often after periods of terrible suffering, and this overall figure is not improving. Partly because of the complexity of the process, basic research on metastasis has lagged behind other disciplines such as carcino genesis and cancer genetics. Metastasis formation involves the ability of malig nant cells to invade adjacent tissue and to penetrate into lymphatic or blood circulatory systems, or both, and to spread to near or distant sites to form new tumor colonies. Meanwhile, research on metastasis is receiving much attention.

Over the past decade, the widespread use of modern chemotherapeutic and biological agents, combined with laparoscopic surgical techniques, has improved the prognosis of metastatic CRC. A better understanding of the biology of the tumor, along with high efficiency of diagnostic and therapeutic methods, as well as the spread of screening programs, will improve the survival of the CRC patients in the near future.
