Available in PDF, EPUB and Kindle. Book excerpt: Heart disease, despite recent improvements, continues to be the single most im portant cause of death and disability in the United States. It is estimated that the direct cost of medical care for cardiovascular disease is $6 billion dollars per year. Data compiled by the National Center for Health Statistics reveal a dramatic de cline in deaths from cardiovascular disease in the United States (greater than 20% since 1968). This phenomenon has been the subject of in-depth study. It is clear that the decline is real and not a statistical artifact. The decrease in mortality has been noted in all sections of the country, though the onset and rate of decline varies in different regions of the country. Both primary prevention, through changes in risk factors, and basic and applied research leading to earlier recognition and im proved treatment have contributed to the decline. They do not fully explain the decline. Further research is needed to clarify this issue. Clinical cardiologists have been exposed to a veritable explosion of new knowl edge of mechanisms of cardiovascular disease, development of new improved non-invasive diagnostic techniques, and the pharmacodynamics of agents affect ing the cardiovascular system. This new knowledge results from contributions made by individuals from diverse disciplines including cellular and molecular bi ologists, geneticists, hematologists, cardiologists, and cardiovascular surgeons.

Book excerpt: The natriuretic peptides (NPs) family includes a class of hormones and their receptors needed for the physiological control of cardiovascular functions. The discovery of NPs provided a fundamental contribution into our understanding of the physiological regulation of blood pressure, and of heart and kidney functions. NPs have also been implicated in the pathogenesis of several cardiovascular diseases (CVDs), including hypertension, atherosclerosis, heart failure, and stroke. A fine comprehension of the molecular mechanisms dependent from NPs and underlying the promotion of cardiovascular damage has contributed to improve our understanding of the molecular basis of all major CVDs. Finally, the opportunity to target NPs in order to develop new therapeutic tools for a better treatment of CVDs has been developed over the years. The current Special Issue of the Journal covers all major aspects of the molecular implications of NPs in physiology and pathology of the cardiovascular system, including NP-based therapeutic approaches.

Book excerpt: This report considers the biological and behavioral mechanisms that may underlie the pathogenicity of tobacco smoke. Many Surgeon General's reports have considered research findings on mechanisms in assessing the biological plausibility of associations observed in epidemiologic studies. Mechanisms of disease are important because they may provide plausibility, which is one of the guideline criteria for assessing evidence on causation. This report specifically reviews the evidence on the potential mechanisms by which smoking causes diseases and considers whether a mechanism is likely to be operative in the production of human disease by tobacco smoke. This evidence is relevant to understanding how smoking causes disease, to identifying those who may be particularly susceptible, and to assessing the potential risks of tobacco products.
