Book excerpt: Nurse to nurse handoff has been known as a sacred time in nursing communication and has had a long standing in performing this task without the patient. Recent research has shown an increase in patient safety, nurse satisfaction and patient satisfaction when nurse-to-nurse handoff occurs at bedside and includes the patient. Bedside report has been shown to increase the perception of safety in the patient, identify potential medication errors before they occur, and influence a sense of satisfaction among nurses. The nurses that practice this style of reporting are facilitating a connection between the patient and the nursing staff. A review of the literature seeking studies reflective of a change to bedside report has been conducted. With the use of Lewin09́s Change Theory an implementation, evaluation and dissemination plan has been established to support an Adult Medical Unit09́s change to bedside shift report. Each phase of the plan utilized evidence based best practice to support the change to bedside report. In implementing a standardized report process, this unit09́s goal was to improve patient safety, patient satisfaction, and nurse satisfaction. Surveys were conducted before and after implementation of the change along with real time audits to provide feedback to staff. Additional tools were provided to staff to further support their transition to bedside report. Through the literature review conducted, other near miss situations were noted that further supported the implementation of bedside shift report. These included medication errors, infiltrated intravenous sites, and incorrect laterality of scheduled surgeries. It is also noted that bedside report is supported by the Institute of Medicine and The Joint Commission who aim to support health care to be provided in the safest manner.

It is a time to review the plan of care, reconcile medications and infusions and answer questions. Traditionally, this has occurred in a central area such as a conference room or the nurses' station, away from the patient and family. Moving shift report to the bedside has been proven to improve communication between caregivers and the patient. Critical information can be exchanged and the bedside location allows for direct visualization of the patient along with verification of infusions, equipment settings and patient acuity. When this communication occurs at the bedside, patients and families are brought into the conversation and become active participants in the plan of care. Research has shown that conducting shift report at the bedside can significantly improve both patient and nurse satisfaction, leading to a more patient centered care approach, which in turn, may translate into better patient outcomes (Anderson and Mangiano, 2006). Bedside shift report (BSR), is a nursing intervention that may also assist with the Joint Commission directive to' involve both the patient and his or her family to develop a treatment plan that is tailored to the patient's unique needs. Staff should encourage the patient and family to ask questions throughout the course of care and provide opportunities for them to participate in care discussions' (The Joint Commission, 2010 pg. 18). To examine the effect of implementing BSR, a pilot study will be conducted in a 25-bed unit within an acute care hospital in the northeast United States. Presently, handoff report is conducted at the nurses' station; it is not uncommon for there to be questions later when the nurse actually visualizes the patient and the previous caregiver is gone. The process can also leave the patient unattended for up to an hour while report is taking place. This may result in delay of care or potentially, missed opportunities to avert crises. Based on the results of this pilot study, a decision will be made regarding hospital wide implementation.

The purpose of this study is to show bedside shift report increases patient safety as well as staff satisfaction. This study found bedside report lead to less falls and medication errors. Communication breakdowns are the cause of a significant number of sentinel events. Staff was satisfied with shorter report times and improved communication between shifts. Nurses were better prepared and were able to better assess patient needs after implementation of bedside shift report. Nurses were held more accountable and patients became more involved in their care. This new process gave the oncoming nurse the opportunity to visualize the patient and ask questions. Finding of this report confirmed how important having a standardized report system is in reducing errors. Accurate and consistent communication was improved after the implementation of bedside reporting. Effective communication at shift change is a fundamental component of nursing and the culture of safety. This study was conducted within a large hospital on medical and surgical units. A survey was obtained before the implementation of bedside report and several times over the first year post. Along with the use of surveys visual observations were done as well as in person interviews. Results of this study show that bedside report increased nurse's satisfaction, helped in the prioritization of patients which increased work flow, and decreased the amount of time for handoff.

A bedside shift reporting pilot will be implemented using Lewin's Change Theory as the foundation. A hand tool will be created using evidence-based research on bedside shift reporting will be created to implement bedside shift report so that an effective report can be given to allow patients to become more proactive in their health care management. Bedside shift reporting increases the communication level between the patient and nurse; thus increasing patient safety and patient satisfaction. Nurses are more accountable to the patient at the bedside.

Some of these errors are accounted for through miscommunication during the change of the nurse shift report. One of the Joint Commission’s 2019 national patient safety goals for hospital programs was to improve communication effectiveness among caregivers since mistakes in a caregiver’s communication accounted for 37% of severe patient injuries. Safety is threatened by patients not being included. Therefore, adopting the bedside shift report allows patients to be actively engaged, reinforcing nurse-patient communication. The bedside shift report enables nurses to provide safe, high-quality, patient-centered care at the bedside. At the time of this project, a 24- cardiac bed unit at a community hospital in Louisiana lacked a bedside shift report. Surveillance of the nurse shift report at the nurses’ station unveiled countless communication gaps. The Hospital Consumer Assessment of Healthcare Providers and Systems survey measures patient perception of their hospital experience. The scores from April 2019 revealed a need for improvement in nurse-patient communication at the unit level. The data were compared for the communication with nurse dimension of the Hospital Consumer Assessment of Healthcare Providers and Systems scores preimplementation of the bedside shift report from April 2020 (July 2018–June 2019 discharges), with survey scores postimplementation of the bedside shift report from April 2021 (July 2019–June 2020 discharges), using the longitudinal data analysis. The four specific questions in the Hospital Consumer Assessment of Healthcare Providers and Systems survey related to communication with the nurse and comparing data preimplementation and postimplementation of the bedside shift report were examined. This quality improvement research project aimed to assess the bedside shift report on patient satisfaction in care delivery for the communication with nurse dimension in the Hospital Consumer Assessment of Healthcare Providers and Systems survey. An oncology nursing unit at Banner Gateway Medical Center is currently giving shift report away from the patient, causing a decrease in satisfaction scores. The purpose of this project proposal is to examine bedside reporting compared to traditional reporting at the nurses' station and the impact on patient satisfaction. This project will provide a description of the problem, proposed solution, and a detailed implementation, evaluation, and dissemination plan. Evidence suggests that bedside reporting is more patientcentered and comprehensive ensuring that quality care is delivered from shift to shift. Furthermore, this reporting method improves the patient's experience of care, promotes nurse and patient engagement, increases time spent providing direct care, improves communication, and enhances nursing practice. Furthermore, research shows that bedside shift report allows the oncoming nurse to visualize the patient, perform a quick assessment, answer patient questions, confirms the plan of care, and establishes patient goals for the shift. This new method of reporting will improve nursing practice, outcomes, and patient satisfaction.

Author: Gilbert Young, RN, BSN Setting: Unit 1 North is a 32-bed cardiovascular specialty unit that specializes in the care of post-cardiac surgery and complex telemetry patients. The typical length of stay is between 5-7 days. Patients are admitted to the unit from the ICU, ED, or other medical facility for cardiac surgery. Rationale: The quality gaps are nurse communication HCAHPS scores not being consistent with performance goals, the unit had 16 documented patient falls in 2016, and patient surveys and staff handoff observations supported the need for this project. The project aim is to (1) improve nurse communication, (2) increase the nurses' consistency and satisfaction with handoff (3) implement the clinical report tool, and (4) reduce patient falls by 25% on cardiovascular specialty unit by August 2017. Return on Investment: Reducing four patient falls or 25% of last year's total will be an annual cost avoidance of $120,000 once this performance improvement plan is implemented. The initial annual savings is estimated at $110,300 Literature Review: The studies assert the importance of bedside report as a means to include the patient, eliminate information gaps, visualize the patient and surroundings for patient safety, and improve team communication. Additional studies examined the use of a standardized nursing handoff tool and saw improvement in patient satisfaction and a reduction in nursing errors. Methodology: Institute of Healthcare Improvement model for improvement and Lewin's change management model. Collaborated with unit council to develop clinical handoff tool. Educational plan includes PowerPoint presentation and simulation sessions. Project implemented in February 2017 and guided by PDSA (plan-do-study-act) cycle. Nurse leaders to observe use of clinical handoff tool during shift handoff. Results: Since implementation in February, average daily census of 32 patients and averaged 66 survey responses each month. Nurse communication scores have been consistently 4 Stars since implementation of this QI project. Current HCAHPS scores up to May 2017. There was a 12.5% reduction in patient falls compared to the same time period last year. Nurse leader observation of NKE using the handoff competency checklist demonstrated successful use of the handoff tool by all nurses, including remediation for some staff. Patient and staff surveys revealed positive improvements in seeking patient input and staff experiencing more satisfaction with the quality if information they receive at shift handoff.

To decrease the frequency of medical errors, the implementation of a standardized communication tool is required. The use of bedside shift report is essential to nursing practice. Bedside repot is a method of nurse to nurse handoff communication performed at the patient's bedside. Nurses are able to assess the patient in person while obtaining vital information. A nurse's accountability can be higher while including the patient in their plan of care and patient satisfaction improved. Any issues and concerns can be addressed at the moment, increasing patient safety. As concerns arise with the traditional method of nursing handoff tool, the need for bedside shift report is needed to increase patient safety, therefore decreasing the number of medical errors and sentinel events. Bedside shift report is implemented to a medical surgical unit with the main goal of increasing patient safety using an improved method of nurse-to-nurse handoff communication. This paper will describe the problem, solution to the process of the issue, implementation plan, evaluation, and dissemination plan. It will also describe studies before and after the implementation with nurse and patient's perception of the process. Results from the review of studies shown reveal how other clinical settings have benefited from bedside shift report, improving patient's safety.

Book excerpt: As of 2006, the Joint Commission has made standardization of report one of its top National Patient Safety Goals, due to its role in patient wellbeing. Research over the last two decades has indicated that the way medical professionals give and receive handoff communication still needs improvement and continues to be a leading cause of medical miscommunication and errors. Sources of error include misidentification of the patient, mistaken medication administration, and lack of proper patient education. Improving report will increase staff preparedness in dealing with the complexities of care and the challenges of complicated diagnosis. Incorporation of bedside report into handoff communication has been found to improve ownership and accountability amongst nurses, improve the nurse/patient relationship and increase patient safety. Implementation will take place beginning on one unit in a busy regional medical center with plans for wide spread hospital application at a later date. Implementing standardized SBAR forms to act as a template during bedside report and allowing the patient to become an active participant and decision maker in care will increase both staff and patient accountability in terms of health. Gaining the support of administration and education of staff through a 4 week training process will be critical in incorporating bedside report safely into practice. Building staff confidence on their abilities to do bedside report will improve compliance. Evaluation will take place after a 6-week implementation phase through detailed comparison of before and after surveys, questionnaires, and interviews from the perspective of nursing staff as well as patients. This new form of report allows for better communication, respect and dignity, information sharing, participation and collaboration between nursing staff and patients.
