Book excerpt: "Diabetes is a chronic disease that occurs when either the pancreas does not produce insulin or when the body cannot use the amount of insulin produced effectively. In order to avoid the plethora of complications related to diabetes, it is vital to educate the patient on self-care pertaining to this disease. The purpose of this study was to identify barriers to self-care in individuals with diabetes. This study utilized a quantitative, non-experimental, and descriptive-correlational research design. Data collection began at a region diabetes education center located in Michigan and for a six-week time period, all participants that met the inclusion criteria were given the survey packet. Eighty surveys were distributed and the researcher received a total of 20 entirely completed surveys for a response rate of 25%. The data was entered into a statistical software program known as Statistical Package for the Social Sciences (SPSS). Further data analysis was derived using descriptive statistics. The findings from this study confirm that there are areas where interventions are needed in self-care among the diabetic population. Combining the findings of this study with similar studies may help nurses develop in definitions that improve self-care behavior. By using the SUmmary of Diabetes Self-Care Activities, the investigator gained knowledge on perceived barriers to self-care behaviors in individuals with diabetes. Further research should be completed in order to affect changes in clinical practice that will improve care and patient outcomes." -- From the abstract, page vii.

Book excerpt: The overall goal of my CNL project was to increase participation of diabetic patients in diabetes self-management education (DSME) by at least 20%, by identifying and addressing barriers and obstacles hindering the achievement or maintenance of diabetes self-management behaviors in a medical-surgical unit. A total of 20 diabetic patients completed both the pre - and pot-test questionnaires; 40% of participants were Asian Americans, 30% were African Americans, 20% were Hispanics, 10% were Caucasians. A pre- test questionnaire was administered to identify the barriers susceptible to hinder the achievement of diabetes self-care behaviors. The main barriers identified include the limited accessibility and the high cost of healthy foods, the inability to read food labels, the lack of knowledge about carbohydrate counting, the lack of social support, the lack of time and lack of motivation, the lack of knowledge of self-care procedures including blood glucose monitoring, medication management, poor problem-solving skills, maladaptive coping mechanisms, and the lack of knowledge of acute complications. Following the pre-test, an intervention consisting of education of patients in diabetes self-management was implemented to address the identified barriers. The goal of the education was to improve the overall health status by empowering diabetes patients to acquire knowledge, learn skills, develop the appropriate behaviors, develop the problem-solving and coping skills to overcome the barriers and obstacles identified. Following the intervention, over 50% of participants agreed to be able to make improvements in diabetes self-management behaviors. Most of them did change their self-management behaviors; others started reflecting on it.

Few studies have been done to explore rural population's access to and attendance of diabetes self care education and support groups. The purpose of this qualitative study was to determine if residents of a rural southern Minnesota community have access to diabetes education and support groups and what barriers they face to attending the groups. The understanding gained through this study will help to develop nursing interventions to assist the diabetic patient to connect with diabetes education and support groups. The study was qualitative, utilizing the interview method. The setting was a rural southern Minnesota community with six voluntary participants over age 18 with type 2 diabetes. Interviews were conducted with four guiding questions. The study results suggest that intervening early after the type 2 diabetes diagnosis is essential to promoting self management as this is when interest is peaked and patients are most open to learning. Nurses should assess for self care deficits and intervene to promote self care with all diabetic patient interactions. Incorporating diabetic education, such as handouts, websites, education and support groups, and brief explanations of complications, at every visit is important to promote self care.

Despite the provision of comprehensive diabetes management programmes, patients are often not able to achieve the desired outcomes. The aims of the study were to investigate factors affecting self-care behaviours of patients with Type 2 DM among Hong Kong Chinese people and to explore barriers to self-care and strategies to overcome barriers from patients' and nurses' perspectives. The conceptual framework was developed from the Health Belief Model and the Theory of Planned Behaviour. A sequential explanatory mixed methods design was adopted. Structured questionnaires were used to investigate key variables for predicting self-care for 183 patients with Type 2 DM. Subsequently, 22 patients and 17 nurses were selected for focus group interviews. The quantitative findings showed that both attitudes and modifying factors (knowledge, demographics, socio-economic factors, insulin therapy) had a significant effect on self- care behaviours. They showed that older patients, those of lower socio-economic status, being treated with insulin, having better knowledge about DM, and with positive attitudes reported better adherence to self-care. The qualitative findings indicated that the patients and nurses perceived barriers to self-care were consistent including physical, psychological, socio-economic, environmental and cultural factors. The focus group interviews revealed that nurses understood most of the barriers to self- care perceived by their patients. The Diabetes Self-care Model was developed in the study to inform and guide healthcare professionals when designing diabetes management programmes by addressing key variables affecting self-care behaviours and patient outcomes. The unique position of nurses and their understanding of patients' needs are essential for improving patient outcomes. Therefore, the roles of nurses should be reinforced and further developed to address the needs of the patients through improving the effectiveness of diabetes management programmes and patient empowerment.

Book excerpt: Type 2 diabetes mellitus represents a daunting self-management challenge due to its complicated daily treatment regimen which includes adhering to diet, exercise, and medication recommendations. Little research has evaluated perceived barriers to diabetes self-care in a group that is at particular risk for diabetic complications: U.S. Latinos. This study had two aims. The first was to provide evidence that experimental and validated self-report instruments selected for this study functioned in a comparable manner in both Spanish and English in a sample of low income, predominantly Spanish-speaking diabetic patients (n = 77). The second aim was to examine hypothesized relationships of selected predictors to two criterion variables measuring health status (i.e., glycosylated hemoglobin A1c and Diabetes Impact) using a method described by Baron & Kenny (1986). Predictors included age, education, and income as covariates, and perceived environmental barriers to self-care, self-efficacy for treatment adherence, fatalistic beliefs (external health locus of control), and Latino cultural diabetes beliefs. Potential moderator and mediator variables of the relationship between perceived barriers to self care and criterion variables included self-efficacy for treatment adherence, fatalistic beliefs and Latino cultural diabetes beliefs. Scales used in this study had coefficient alpha values ranging from .82-.96 in Spanish and .74-.97 in English and also demonstrated strong factor structure integrity. Bivariate correlations indicated that higher scores on perceived barriers to self-care were significantly related to lower self-efficacy scores [r = −.34, p
