Detailed reference tables were published separately. This report summarizes the findings extracted from the reference tables and subjects data to statistical analysis. There has been a significant aging of the Army aviator population from 1986 through 1992, whatever the component of service. Most of the aging effect is seen in the cohort of aviators age 40 and older. This trend is due likely to a marching cohort of Vietnam-era aviators who are approaching military retirement age. Female aviators are significantly younger than their male peers. This finding is due likely to the recent recruitment of women into Army aviation during the 1980's. A bimodal curve exists in the age distribution of Army aviators. This finding is due likely to trends in Army force structure during the Vietnam War build up, followed by post-Vietnam force reduction, and followed by 1980's cold war build up. Now the Army is undertaking another force reduction in this decade.

Book excerpt: An epidemiological analysis of the coronary angiography outcomes of Fort Rucker civilian aviators could not be completed without first conducting an analysis of attrition from aviation service due to medical and nonmedical causes. Longitudinal data on flying duty medical examinations and aeromedical boards conducted on Fort Rucker area civilian aviators was reviewed using the U.S. Army Aviation Epidemiology Data Register (AEDR). The AEDR records of 847 Fort Rucker civilian aviators were reviewed over nine calendar years from 1985 to 1993. By the end of 1992, 528 (62.3 percent) remained in aviation service, 251 (29.6 percent) were lost to nonmedical attrition, and 68 (8.1 percent) were lost to medical attrition. Of the 847, 196 (23.14 percent) had operational military affiliation (dual-status service as civilians and reserve component aviators), while 651 (76.86 percent) did not. Book excerpt: The U.S. Army Aviation Epidemiology Data Register (AEDR) was queried for Army aviators with the finding of glycosuria, hyperglycemia, impaired glucose tolerance (IGT), diabetes mellitus (DM), use of oral hypoglycemic agents, or use of insulin for the period 1988 to 1992. The paper reviewed the literature pertaining to aircrew with diabetes, listed the aeromedical concerns related to flying duties and diabetes, and listed factors affecting the incidence and prevalence of diabetes mellitus in Army aviators. The study tabulated the incidence and age-specific annual rates of diabetes mellitus and impaired glucose tolerance, and tabulated the distribution and reasons for aeromedical dispositions of aircrew with these conditions. U.S. Army aeromedical planners can expect about 21 new cases of DM and IGT each year. Aviators over 35 years of age were at the greatest risk. About 78.4 percent of the aviators with DM and about 10.9 percent of the aviators with IGT will not be fit for flying duties. This was primarily due to an inability to gain dietary control of their condition or the discovery of other significant medical conditions, such as coronary artery disease. A cost-benefit and case-finding analysis of trained aviators stratified by age showed that screening for DM and IGT would be more effective if started at 35 years of age.

Book excerpt: Graduated aviators from the U.S. Army student aviator cohort of the Class of 1987 were followed in the Aviation Epidemiology Data Register for 6 years to determine if there was a gender-specific difference in attrition from aviation service. Overall, 30 percent attrition was found in 6 years of observation following initial aviator training. There was no significant gender-specific difference in attrition (p>0.05, life table analysis). Pregnancy was the most common identified cause of female aviator attrition. However, there was no significant increase risk for attrition among all pregnant aviators after delivery (relative risk=0.545, CI0.95=0.144,2.06). Among male aviators, flying evaluation boards with nonmedical termination of aviation service, death due to aircraft mishaps, and alcohol abuse were common identified causes of attrition. These three conditions accounted for 47 percent of known causes for male aviator attrition.

The U.S. Army aviation medicine community can expect an annual incidence rate about 1 case of HNP per 1,000 aviators years. However, the incidence rate is increasing. Aviators about age 40 were at the greatest risk. About 7.4 percent of the aviators with HNP were removed permanently from Army flying duties due to HNP complications.

Book excerpt: This was a descriptive study to meet the short-notice requirements of the U.S. Army Aero-medical Center in support of the upcoming Women in Army Aviation Symposium in February 1996. Analysis of available data in the Aviation Epidemiology Data Register and school records at the U.S. Army School of Aviation Medicine provided a better understanding of the age distribution, numbers, retention, and aeromedical disposition outcomes of female Army flight surgeons and aeromedical physician assistants for the period 1986 to 1995. Seven data tables are provided to summarize the findings. Among a cohort of 141 applicants, 107 attended the Army Flight Surgeon Primary Course, Aeromedical physician assistant applicants were just as likely as flight surgeons to attend the U.S. Army Fight Surgeon Primary Course (Relative risk (Katz)=l.06, CI 0.95= 0.81,1.39) Many attending the course required a waiver for their medical disqualifications (42.1%) . Flight surgeons were more likely than aeromedical physician assistants to require a waiver, but not significantly (Relative risk (Katz) =2.75, CI 0.95=0.76,9.91).
