It details the review and disposition of aeromedically disqualified aviation training program applicants, aircrew members, and air traffic controllers, both civilian and military. The traditional core of Army aviation medicine is the delivery of clinical and operational preventive medicine services. This administration guide establishes a public health protocol as it pertains to the conduct of U.S. Army FDMEs. The FDME protocol adopts the current standard of preventive health care promoted by the U.S. Public Health Service, but is modified to meet the needs and challenges of the operational Army aviation environment. The protocol is the result of 6 years of literature review, AEDR research, and staffing of proposed aviation medicine public health policy through the U.S. Army Aeromedical Center, U.S. Army Aeromedical Research Laboratory, Army Surgeon General Aviation Medicine Consultant, and the Aeromedical Consultant Advisory Panel.

The components are administratively linked by social security number (SSN). A new component is an upgrade of the medical document microfiche archive to laser optic CD-ROM. The U.S. Army Aeromedical Activity requested assistance in creating an indexing scheme for the new system. They wanted to use a single SSN digit, the disks to fill at a uniform rate, and to keep the same aircrew member on the same disk even if multiple entries were made for the same patient. The SSN Of each case in the microfiche archive was extracted as a reference population. Analysis Of the frequency distribution of decimal values for selected SSN digits showed the last SSN digit would meet the stated requirements. Any of the first five SSN digits should not be used for indexing.

Book excerpt: The U.S. Army Aviation Epidemiology Data Register (AEDR is an automated database which allows electronic storage, analysis, and retrieval of information of the Flying duty Medical Examination (FDME). The FDME consists of a completed Report of Medical History (standard form (SF) 93), Report of Physical Examination (SF 88), Report of Electrocardiogram SF 520) with the electrocardiogram tracing, and for certain classes of FDME, additional information on lifestyle factors and family history. Demographic data, patient history, physician history, physical findings, screening tests, and diagnoses information are included on these forms, each of which has a unique value in health care, administration, and research, and each of which must be handled differently in the AEDR. Demographic and screening test results are entered directly from the FDME to the AEDR. History, physical findings, and diagnoses are translated into a standardized alphanumeric code, a modification of the International Classification of Diseases (ICD). Because of the unique characteristics of the military aviation environment, the ICD is inadequate to support all the clinical, administrative, and research functions of the AEDR. The ICD has been supplemented with additional diagnostic codes to provide extra specificity, codes for physical findings and electrocardiograms, and codes to automate the administrative process. These codes are present and discussed.

Book excerpt: An epidemiological analysis of the coronary angiography outcomes of Fort Rucker civilian aviators could not be completed without first conducting an analysis of attrition from aviation service due to medical and nonmedical causes. Longitudinal data on flying duty medical examinations and aeromedical boards conducted on Fort Rucker area civilian aviators was reviewed using the U.S. Army Aviation Epidemiology Data Register (AEDR). The AEDR records of 847 Fort Rucker civilian aviators were reviewed over nine calendar years from 1985 to 1993. By the end of 1992, 528 (62.3 percent) remained in aviation service, 251 (29.6 percent) were lost to nonmedical attrition, and 68 (8.1 percent) were lost to medical attrition. Of the 847, 196 (23.14 percent) had operational military affiliation (dual-status service as civilians and reserve component aviators), while 651 (76.86 percent) did not. Book excerpt: This was a descriptive study to meet the short-notice requirements of the U.S. Army Aero-medical Center in support of the upcoming Women in Army Aviation Symposium in February 1996. Analysis of available data in the Aviation Epidemiology Data Register and school records at the U.S. Army School of Aviation Medicine provided a better understanding of the age distribution, numbers, retention, and aeromedical disposition outcomes of female Army flight surgeons and aeromedical physician assistants for the period 1986 to 1995. Seven data tables are provided to summarize the findings. Among a cohort of 141 applicants, 107 attended the Army Flight Surgeon Primary Course, Aeromedical physician assistant applicants were just as likely as flight surgeons to attend the U.S. Army Fight Surgeon Primary Course (Relative risk (Katz)=l.06, CI 0.95= 0.81,1.39) Many attending the course required a waiver for their medical disqualifications (42.1%) . Flight surgeons were more likely than aeromedical physician assistants to require a waiver, but not significantly (Relative risk (Katz) =2.75, CI 0.95=0.76,9.91).
