Hearing loss affects about 7% of the population in developed countries, and the proportion is increasing as the average age of the population increases. The audiogram is the most widely used diagnostic tool in audiology clinics around the world. The audiogram involves measuring the threshold for detecting sounds of different frequencies. Sometimes the audiogram is the only diagnostic tool that is used. However, hearing problems are not completely characterized by the audiogram. Two individuals with similar audiograms may show very different abilities in the detection and discrimination of sounds at above-threshold levels. Also, a person may have hearing difficulties despite having an audiogram that is within the range conventionally considered as ‘normal’. One factor that may influence the discrimination of sounds, especially the ability to understand speech in background sounds, is sensitivity to temporal fine structure (TFS).This monograph reviews the role played by TFS in masking, pitch perception, speech perception, and spatial hearing, and concludes that cues derived from TFS play an important role in all of these. Evidence is reviewed suggesting that cochlear hearing loss reduces the ability to use TFS cues. Also, the ability to use TFS declines with increasing age even when the audiogram remains normal. This provides a new dimension to the changes in hearing associated with aging, a topic that is currently of great interest in view of the increasing proportion of older people in the population.The study of the role of TFS in auditory processing has been a hot topic in recent years. While there have been many research papers on this topic in specialized journals, there has been no overall review that pulls together the different research findings and presents and interprets them within a coherent framework. This monograph fills this gap.

Book excerpt: Purpose: This study was designed to investigate (1) the effects of age and hearing loss on auditory neural coding, (2) interrelationships between auditory evoked potentials (AEP) taking place at early and later stage of auditory processing, and (3) electrophysiological correlates of age-related declines in speech-in-noise perception. Methods: 30 young normal-hearing adults (YNH, M=21 years), 26 older adults with near-normal hearing (ONH, M=63.9 years) and 26 older adults with hearing loss (OHL, M=72.8 years) participated in the study. AMLR Pa and N1, P2 and N2 of ALLRs were recorded using two-channel electrode system. 500 Hz tone burst and syllable /ba/ were presented in quiet and in babble at the level of 90 Leq (dBA), accompanied by babble at the level of 65 Leq (dBA). Revised Speech Perception in Noise (R-SPIN) test was conducted to measure speech-in-noise perception. Results: ONH listeners demonstrated significantly enhanced Pa and N1 amplitudes and significantly prolonged Pa, P2 and N2 latencies compared with YNH listeners, indicating the effects of aging. OHI group demonstrated significantly prolonged N2 latencies compared with ONH group, indicating the effects of hearing loss. OHI listeners demonstrated significantly enhanced amplitudes and significantly prolonged latencies across all AEP components compared with YNH listeners, indicating that the combined effects may have stronger impacts on age-related changes in AEP morphology. Significant correlations between the amplitudes of Pa and each component of ALLRs were found in ONH and OHI groups, indicating that enhanced Pa amplitudes correspond with enhanced amplitudes of cortical responses in older listeners. Significant correlations between latencies of Pa and each component of ALLRs were found in all groups, indicating that prolonged Pa latencies correspond with prolonged latencies of cortical responses in all listeners. In both ONH and OHI groups, lower R-SPIN-LP scores are significantly correlated with enhanced Pa, P2, and N2 amplitudes and prolonged N2 latencies, particularly for syllable /ba/. Discussion and Conclusions: Interplay between effects of aging and hearing loss may have stronger impacts on morphological changes in the AEP waveforms. Amplitude-based interrelationships reflect age-related changes in a transfer of neural information between subcortical and cortical auditory network. Latency-based interrelationships indicate the association between neural timings at subcortical and cortical levels. Age-related enhancements of Pa, P2 and N2 amplitudes and age-related prolongations of N2 latencies may serve as electrophysiological correlates of age-related declines in SIN perception.

A survey of the existing auditory-space perception literature suggests that three main types of factors may account for this variability: - physical factors, e.g., acoustical characteristics related to sound-localization cues, - perceptual factors, e.g., sensory/cognitive processing, perceptual learning, multisensory interactions, - and methodological factors, e.g., differences in stimulus presentation methods across studies. However, the extent to which these–and perhaps other, still unidentified—factors actually contribute to the observed variability in spatial hearing across individuals with normal hearing or within special populations (e.g., hearing-impaired listeners) remains largely unknown. Likewise, the role of perceptual learning and multisensory interactions in the emergence of a multimodal but unified representation of “auditory space,” is still an active topic of research. A better characterization and understanding of the determinants of inter-individual variability in spatial hearing, and of its relationship with perceptual learning and multisensory interactions, would have numerous benefits. In particular, it would enhance the design of rehabilitative devices and of human-machine interfaces involving auditory, or multimodal space perception, such as virtual auditory/multimodal displays in aeronautics, or navigational aids for the visually impaired. For this Research Topic, we have considered manuscripts that: - present new methods, or review existing methods, for the study of inter-individual differences; - present new data (or review existing) data, concerning acoustical features relevant for explaining inter-individual differences in sound-localization performance; - present new (or review existing) psychophysical or neurophysiological findings concerning spatial hearing and/or auditory perceptual learning, and/or multisensory interactions in humans (normal or impaired, young or older listeners) or other species; - discuss the influence of inter-individual differences on the design and use of assistive listening devices (rehabilitation) or human-machine interfaces involving spatial hearing or multimodal perception of space (ergonomy).

Impaired perception, such as that resulting from hearing loss or reduced visual or tactile resolution, increases demands on ‘higher-level’ cognitive functions to cope or compensate. It is possible, for example, to use focused attention to overcome perceptual limitations. Unfortunately, cognitive functions also decline in old age. This can mean that perceptual impairments are exacerbated by cognitive decline, and vice versa, but also means that interventions aimed at one type of decline can lead to improvements in the other. Just as improved cognition can ameliorate perceptual deficits, improving the stimulus can help offset cognitive deficits. For example, making directions and routes easy to follow can help compensate for declines in navigation abilities. In this Topic, we bring together papers from both auditory and visual researchers that address the interaction between perception and cognition in the ageing brain. Many of the studies demonstrate that a broadening of representations or increased reliance on gist underlie perceptual and cognitive age-related declines. There is also clear evidence that impaired perception is associated with poor cognition although, encouragingly, it can also be seen that good perception is associated with better cognition. Compensatory cognitive strategies were less successful in improving perception than might be expected. We also present papers which highlight important methodological considerations that are required when studying the older brain.

The authors outline a systematic approach to the clinical detection, evaluation and amelioration of individuals who exhibit binaural interference. Suggestions are furnished on improved techniques for evaluation of the binaural advantage in general and on sensitized detection of the disorder in particular. The book ends with recommendations for future directions. Given the adverse impact of binaural interference on auditory function and its occurrence in a significant subset of the population with hearing loss, as well as in some individuals with normal-hearing sensitivity, research on binaural interference only recently has begun to flourish, and adaptation of audiologic clinical practice to identify, assess, and manage individuals with binaural interference has yet to become widespread. The authors intend for the book to provide impetus for pursuing further research and to encourage audiologists to explore the possibility of binaural interference when patient complaints suggest it and when performing audiologic evaluations. The book is intended for practicing clinical audiologists, audiology students, and hearing scientists.
