Book excerpt: Qualitative information was collected of 64 indigenous women in highland Guatemalan and 73 mestizo women throughout El Salvador via in-depth interviews and direct observations prior to the design of education/communication programs. Data were collected on beliefs, attitudes and values related to infant feeding in the first 24-36 months and on actual breastfeeding and weaning practices during health and illness. Findings on cultural factors were related to the infant feeding decisions of interviewed women and to patterns of breastfeeding initiation, exclusive breastfeeding, introduction of liquids and weaning foods into infants' diets. In both settings, despite important differences in cultural traditions, women hold very similar and strong beliefs, attitudes and values towards the qualities of breastmilk, breastfeeding, the nutritional needs of young children, and their own capacity of produce adequate quantities of milk of acceptable quality. These cultural factors color their interpretation of infant behaviors such as crying and influence their day-to-day decisions about infant feeding. Dominant in the resulting suboptimal feeding patterns is the almost complete absence of exclusive breastfeeding beyond the second month. These findings demostrate the persistence of cultural factors in infant feeding practices in the early months (AU).

Babies who are breastfed are less likely to become overweight and obese. Many mothers in the United States want to breastfeed, and most try. And yet within only three months after giving birth, more than two-thirds of breastfeeding mothers have already begun using formula. By six months postpartum, more than half of mothers have given up on breastfeeding, and mothers who breastfeed one-yearolds or toddlers are a rarity in our society. October 2010 marked the 10th anniversary of the release of the HHS Blueprint for Action on Breastfeeding, in which former Surgeon General David Satcher, M.D., Ph. D., reiterated the commitment of previous Surgeons General to support breastfeeding as a public health goal. This was the first comprehensive framework for national action on breastfeeding. It was created through collaboration among representatives from medical, business, women's health, and advocacy groups as well as academic communities. The Blueprint provided specific action steps for the health care system, researchers, employers, and communities to better protect, promote, and support breastfeeding. I have issued this Call to Action because the time has come to set forth the important roles and responsibilities of clinicians, employers, communities, researchers, and government leaders and to urge us all to take on a commitment to enable mothers to meet their personal goals for breastfeeding. Mothers are acutely aware of and devoted to their responsibilities when it comes to feeding their children, but the responsibilities of others must be identified so that all mothers can obtain the information, help, and support they deserve when they breastfeed their infants. Identifying the support systems that are needed to help mothers meet their personal breastfeeding goals will allow them to stop feeling guilty and alone when problems with breastfeeding arise. All too often, mothers who wish to breastfeed encounter daunting challenges in moving through the health care system. Furthermore, there is often an incompatibility between employment and breastfeeding, but with help this is not impossible to overcome. Even so, because the barriers can seem insurmountable at times, many mothers stop breastfeeding. In addition, families are often unable to find the support they need in their communities to make breastfeeding work for them. From a societal perspective, many research questions related to breastfeeding remain unanswered, and for too long, breastfeeding has received insufficient national attention as a public health issue. This Call to Action describes in detail how different people and organizations can contribute to the health of mothers and their children. Rarely are we given the chance to make such a profound and lasting difference in the lives of so many. I am confident that this Call to Action will spark countless imaginative, effective, and mutually supportive endeavors that improve support for breastfeeding mothers and children in our nation."--Page v.

The Hispanic population in the United States has a high breastfeeding initiation rate; however, they also have a high formula supplementation rate by the time the infant reaches three months of age. Recently immigrated primiparas represent an especially vulnerable group since they have limited support systems, have never breastfed, and are experiencing the transitions of immigration and motherhood simultaneously. The aims of this critical ethnographic study were 1) to describe the cultural values, beliefs, and practices of recently immigrated Mexican primiparas surrounding exclusive breastfeeding during the first three months of life; 2) to explore if and how culture, gender, and issues of power and marginalization affect recently immigrated Mexican primiparas' attitudes, beliefs, and practices regarding exclusive breastfeeding; and 3) to gain an understanding of how recently immigrated Mexican primiparas respond to nurses' and healthcare providers' "authoritative knowledge" regarding exclusive breastfeeding, to determine how they sort through this information, and if and how they integrate this information to create new unique understandings of exclusive breastfeeding. The study's findings are drawn from one year of participant observations conducted in a Women, Infants, Children (WIC) program office, as well as a four-month series of three longitudinal interviews with eleven women, and partial interview sets with two women. Findings revealed that WIC has created a culture of exclusive breastfeeding and that WIC is a major source of information for women regarding breastfeeding. Exclusive breastfeeding is a natural choice for Mexican immigrant women and is considered "women's work." As a result of the immigrant transition, women's support systems were altered and they often experienced isolation during the transition to motherhood and the role of providing for their baby by exclusively breastfeeding. Women experienced additional challenges in their goal to exclusively breastfeed as they negotiated issues of modesty as well as returning to work. However, women asserted agency regarding breastfeeding decisions based on the values of providing the best for their baby and the maternal-infant bond, and they valued biomedical information in their decision-making processes."--Abstract.

Book excerpt: Despite research consistently demonstrating the benefits of breastfeeding, Native American women in North Central North Dakota have the lowest rate of breastfeeding in North Dakota with a rate of 8.24%. The reasons why these women are not breastfeeding are not known. There have been no studies regarding the attitudes and knowledge about breastfeeding of these women or their health care professionals. The purpose of this study was to assess the knowledge and attitudes about breastfeeding in this population. Thirteen pregnant women, ten women who have given birth, and twelve health care professionals who provide care to these women comprised the sample of this descriptive study. The results of the surveys demonstrated that these women knew the benefits of breastfeeding but the majority of them did not breastfeed their infants. Attitudes seem to be a more relevant factor than knowledge in influencing breastfeeding initiation in this sample of women. Health care providers should be aware that their own attitude toward breastfeeding may affect a woman's choice to breastfeed. Results also demonstrated the health care professionals had limited education about infant feeding, lactation, and breastfeeding. These health care providers may be providing conflicting and possibly incorrect knowledge about breastfeeding. Current evidence based breastfeeding recommendations and practices should be incorporated into continuing education so consistent and correct information is provided. Health systems should establish a baby friendly environment that supports and encourages breastfeeding. In addition, the unique characteristics of the Native American women in North Central North Dakota, or any community in which one lives or works, should be considered to better plan interventions that will be effective and sustainable. It is not one identifiable factor that the decision to breastfeed is dependent upon, but factors that may interact and overlap in ways to influence a women's decision to breastfeed. Understanding context is vital to designing and implementing successful interventions in breastfeeding promotion. Culturally relevant information gathered from this population may not be transferable to others in this particular tribe who may live somewhere else or to other Native American tribes, as their specific cultural attributes may be different from this particular tribe.
