Depuis 2008, la question du rapport bénéfices/risques du DOCS se pose, à l'origine d'une controverse mise en avant dans le rapport du comité d'orientation s'appuyant sur une concertation citoyenne. La décision partagée, plus respectueuse de l'individu, implique un échange bilatéral d'informations, puis une délibération du professionnel et du patient autour des options émergeant et enfin une décision qui résulte d'un accord commun entre les deux parties. L'objectif de ce travail était de déterminer les attentes et représentations des femmes concernant la création d'un outil d'aide à la PDP, dans le cadre du projet DEcision partagée dans le cadre du DépIstage du CAnCEr du Sein (DEDICACES) visant la création d'un outil de PDP utilisable en consultation. Méthodologie : étude qualitative par entretiens semi-dirigés menés auprès de femmes de 40 à 75 ans jusqu'à suffisance des données. Analyse thématique selon une approche phénoménologique avec triangulation par 2 binômes de chercheurs. Résultats : treize femmes ont été interviewées permettant de dégager six thèmes : les représentations des femmes du cancer du sein, les représentations des femmes vis-à-vis du dépistage du cancer du sein, les profils des femmes vis-à-vis du dépistage, les représentations des professionnels de santé selon les femmes, les attentes des femmes vis-à-vis de l'information, les moyens d'information. Discussion : les femmes apparaissent ambivalentes entre modèle paternaliste et informatif. Cela fait écho aux représentations qu'elles ont des profils de pratique des professionnels de santé oscillant entre attitude proactive et passive en matière de dépistage du cancer du sein. La création et l'utilisation d'un outil de PDP dans le DO doit donc se faire conjointement avec une formation des professionnels de santé à l'approche centrée patient et à la PDP.

Face à cette balance bénéfice risque incertaine, la décision médicale partagée (DMP) semble essentielle pour une prise en charge centrée sur le patient. Le projet DEDICACES (DEcision partagée dans le cadre du DépIstage du CanCEr du Sein) a pour but de créer un outil d'aide à la décision (OAD) utilisable en consultation. Objectif : explorer les attentes et les représentations des différents professionnels de santé intervenant dans le DO du cancer du sein vis-à-vis d'un OAD. Méthode : étude qualitative par entretiens semi-dirigés menés auprès de sages-femmes, gynécologues, radiologues et coordinateurs de structure de gestion. Recrutement par convenance en visant la saturation théorique des données. Analyse par théorisation ancrée. Codage manuel des données. Triangulation en groupe de plus d'un tiers des entretiens (5/13). Résultats : 13 entretiens ont été menés dont 3 auprès de sages-femmes, 4 gynécologues, 3 radiologues et 3 coordinateurs de structure de gestion. Des postures communes se sont dessinées : le sachant, le robot et le conciliateur. Malgré cela on a observé un important clivage entre les représentations des différents professionnels interviewés. La DMP était peu connue et interpellait certains professionnels qui remettaient en question l'intérêt d'un OAD. Les attentes des autres professionnels s'orientaient plutôt vers un outil numérique, simple, visuel, réduisant les inégalités sociales de santé. Discussion : il existe une ambivalence chez les professionnels de santé entre la volonté d'informer les femmes et celle de favoriser leur participation au dépistage. En ce sens, l'intérêt des professionnels vis-à-vis d'un OAD est mitigé. Le rôle des sages-femmes est trop méconnu et sa communication au grand public permettrait de favoriser l'accès à la prévention de beaucoup de femmes. Enfin une étude en cours s'intéresse à une nouvelle stratégie de dépistage basée sur le risque individuel.

Il s'est intéressé au principal sujet de polémique : les divergences autour des taux de surdiagnostics. Méthodes. Deux revues systématiques de la littérature ont été réalisées : la première a analysé les différents outils d'information dans le monde et la deuxième les différences méthodologiques conduisant aux divergences de taux de surdiagnostics. Les sites de l'INCa et de Cancer rose ont été analysés avec le prisme des critères internationaux d'outils d'aide à la décision. La concertation citoyenne a été analysée de façon mixte et séquentielle (qualitative/netnographie et quantitative/analyse de correspondance multiple) afin de comprendre les décisions de participer ou non. Résultats-conclusions. L'information appropriée s'éloigne du « pinkwashing » et prend en compte les émotions des femmes. Pour les professionnels, les surdiagnostics conduisent à se réapproprier le doute scientifique et à le gérer.

Méthode : Une revue de la littérature a été réalisée par 3 chercheurs sur la période 2006-2016. Les articles originaux inclus, en langue française ou anglaise, traitaient des documents d'informations ou d'outils d'aide à la décision dans le DOCS sans restriction sur le type d'étude. Résultats : Les 69 articles inclus montraient qu'une progression des connaissances chez les participantes n'était pas corrélée à une augmentation de l'intention de participer ni à une augmentation de la participation. La forme de l'information (brochures, vidéos...) ne semblait pas influer sur ces taux. Les participantes appréciaient les entretiens avec un interlocuteur formé. Dans un tiers des études, la population était incluse sur des critères ethniques et sociaux. Huit études abordaient le surdiagnostic. Conclusion : Cette revue rappelle l'importance des entretiens répétés support d'une décision médicale partagée. Des interventions spécifiques comme le concept de « patient navigator » pourraient lutter contre les inégalités sociales de santé. L'incertitude actuelle concernant le taux de surdiagnostics doit être communiquée aux patientes.

Book excerpt: Introduction: Shared decision-making is a decision-making model where, following an exchange of information and deliberation between patient and health professional, both parties converge toward an accepted decision. This approach values a patient-centered approach. In France and in United Kingdom, decision-making aids for cervical cancer screening are available through leaflets and websites. We collected and analyzed all of these data from the National Health Service and French Health Insurance for evaluation using the up to date scientific evidence criteria from IPDAS. Evaluation according to IPDAS criteria: Several points were missing in all analyzed aids: the level of evidence of related scientific studies, the disease prevalence in absolute value, the effectiveness of cervical cancer screening and its expected benefit of morbidity/mortality decreasing in absolute value. Cancer-related complications are mentioned only by British aid. Finally, risks of complications related to screening are incompletely mentioned by aids of the two countries. Discussion: The absence of data expressed in absolute value and explanation about low scientific evidence provides truncated information that promotes participation and influences choice for screening. The limits of cervical smear screening should be mentioned to inform patients about risks of over-diagnosis, over-treatment, and specific complications. Lack of participation should be recognized as a choice in its own right and systematically mentioned. Perspective of improvement: We propose that to systematically deliver to the patient a verified information easy to understand and to write the aids in partnership with patient associations. Conclusion: As of today, the proposed aids are not suitable for the decision-making of patients about their participation in screening. The health professional's role is central to discuss with the patient the quality of the delivered information and to support decision-making process.

Cette étude a pour but de réaliser un état des lieux des connaissances et représentations des femmes à travers les différents moyens d'information dont elles disposent. Méthode : Les bases de données Medline et Cochrane ont été consultées. Les critères d'inclusions comprenaient les articles scientifiques datant du 01/01/1990 au 01/08/2016 contenant une information donnée ou recherchée auprès de femmes entre 50 à 75 ans et en rapport avec le dépistage en soins primaires. Les critères d'exclusions comprenaient les femmes âgées de moins de 50 ans ou de plus de 75 ans ayant des antécédents personnels ou familiaux de cancer du sein. Résultat : 259 articles ont été identifiés mais seuls 5 ont été inclus. L'un abordait le sujet par le biais des gynécologues, un autre par celui des femmes, deux autres par les brochures et le dernier par internet. Dans ces cinq articles, l'information contenue était variable et biaisée en faveur du dépistage avec les notions de risques minimisés ou absents et des bénéfices surévalués et souvent présents. Conclusion : Selon cette étude, les patientes ne peuvent pas à l'heure actuelle prendre de décision objective concernant le dépistage du cancer du sein. Il pourrait être utile d'envisager un outil objectif lors de consultations orientées sur le dépistage.

Nous avons cherché à savoir quels sont les déterminants de l'attitude des patients en situation de non-prescription médicamenteuse. Méthode : Etude qualitative sur la base des entretiens semi-directifs de 12 patients recrutés par des médecins généralistes. Résultats : Le ressenti des patients face à la non-prescription médicamenteuse était plutôt positif ou neutre, rarement source de mécontentement. La prescription médicamenteuse n'était pas l'attente prioritaire, mais une attente parmi d'autres au sein d'une relation médecin - patient de qualité. Les représentations personnelles et sociétales de la maladie, du médicament et de l'ordonnance modulaient son acceptation. La vision d'une gestion naturelle de la santé favorisait la non-prescription, au contraire du système économique actuel (consumériste, obligation de résultat ; remboursements) qui la freinait. Les demandes explicites de médicaments concernaient surtout des produits de premiers secours remboursables ; les patients semblaient plutôt réticents à réclamer d'autres médicaments ; d'ailleurs peu avaient un a priori sur le traitement nécessaire. Conclusion : La non-prescription est acceptée par bon nombre de patients, sous réserve de satisfaire d'autres attentes, que le médecin peut appréhender en améliorant ses techniques de communication. Les représentations du médicament et de certaines maladies peuvent être modifiées par la formation des médecins, par l'information et l'éducation des patients afin de tendre vers des décisions partagées. La politique de déremboursement actuelle apporte une certaine aide à la non-prescription.

Les objectifs de ce travail étaient de définir les attentes et les besoins des patientes en matière d'information, de concevoir un outil d'aide à la décision en ligne et d'évaluer sa validité apparente et son influence sur le choix éclairé des patientes.Méthodologie : Trois méthodes de recherche ont été menées pour répondre aux hypothèses : un focus group de cinq patientes pour l'étude qualitative, l'élaboration d'un outil d'aide à la décision selon la méthode d'Ottawa et une étude randomisée monocentrique. Cette dernière a été menée auprès de patientes âgées de 18 à 40 ans, adressées pour préservation de la fertilité dans un contexte de cancer du sein, randomisées au sein du groupe IRIS (information standard) ou du groupe DECISIF (avec outil d'aide à la décision en ligne). La mesure du choix éclairé était évaluée selon trois critères, à l'aide d'un questionnaire : les connaissances, l'attitude, et la mise en place ou non de techniques de préservation de la fertilité.Résultats : L'étude qualitative a permis de mettre en évidence que l'information transmise aux femmes et l'annonce de l'infertilité potentielle était à parfaire. L'outil d'aide à la décision en ligne a été élaboré suivant les recommandations des patientes et des professionnels de santé impliqués qui ont confirmé sa validité apparente. En ce qui concerne l'étude quantitative, au total 125 patientes ont été inclues et randomisées dans le groupe IRIS (n=65) ou DECISIF (n=60). Il a été mis en évidence une amélioration des connaissances dans le groupe DECISIF (8.6/10 (±1.34)) par rapport au groupe IRIS (6.49/10 (±1.89)). Dans les deux groupes, les patientes ont une attitude favorable envers la préservation de la fertilité (96 %). La décision finale n'est pas influencée par l'appartenance à un groupe ou un autre. Ainsi, à l'issue de la consultation de préservation de la fertilité, 73.6 % (92/125), soit 69.2 % (45/65) des patientes du groupe IRIS et 78.3 % (47/60) des patientes du groupe DECISIF vont choisir de conserver leurs ovocytes, embryons et/ou tissu ovarien. Book excerpt: These guidelines have been developed to support countries to relieve pain in their paediatric populations. These guidelines address persisting pain in children caused by conditions such as cancer, HIV/AIDS, sickle-cell disease, burns, trauma, and phantom limb pain. Recommendations -- developed following a careful and transparent appraisal of available evidence -- are presented for the pharmacological treatment of mild, moderate and severe pain. The guidelines include chapters on the various systems used to classify pain and on the evaluation of pain, which reviews the available tools for routine pain measurement in children. Provision of sustainable pain relief within health care systems is covered in a separate chapter. Since morphine and other opioid analgesics -- required for the relief of moderate-to-severe pain in children -- are listed under the 1961 UN Single Convention on Narcotic Drugs, the new guidelines include an annex explaining the Convention's requirements for the handling and procurement of opioid analgesics for the relief of pain.
