Book excerpt: Introduction : de la grossesse subie à la grossesse choisie, voilà tout l'enjeu de la contraception. Malgré la multitude de moyens contraceptifs dont on dispose, les échecs demeurent fréquents. La première consultation pour demande de contraception est une étape clé dans le suivi gynécologique, c'est ce moment qui a été retenu pour notre travail. L'objectif principal de cette étude était d'évaluer les attentes et connaissances des femmes dans le but d'améliorer l'observance thérapeutique. Matériel et méthodes : une étude quantitative transversale pluricentrique a été réalisée d'avril à octobre 2018. Résultats : les 160 questionnaires recueillis ont permis de mettre en évidence plusieurs faits. Le recours au médecin était trop tardif. Au moment de la consultation, plus de la moitié des patientes avaient déjà eu des rapports sexuels. Parmi celles-ci, 90% avaient utilisé une contraception. Les femmes attendaient de cette consultation des informations et un accompagnement dans le choix de la méthode contraceptive. La pilule apparaissait comme la contraception de choix des patientes notamment parce qu'elle leur était familière. 97,5% des femmes avaient déjà reçu des informations pourtant 1 femme sur 3 souhaitant la pilule en attendait une prévention des IST. Les effets secondaires sont largement évoqués et représentent un frein au recours et à l'adhésion de la jeune femme à la contraception. Conclusion : pour que la contraception soit réussie, elle doit être adaptée à chacun. Développer l'information et favoriser le recours au médecin apparaissent comme des éléments clés.

Peu d'études ont été réalisées sur leurs attentes quant à cette première consultation de contraception. Méthode : Une étude basée sur un questionnaire en ligne a été menée de septembre 2021 à mars 2022 à l'aide d'une affiche placée en salle d'attente de médecins généralistes, au SUMPPS, au CPEF et dans des pharmacies du Calvados et de l'Orne. Elle concernait les femmes de 15 à 20 ans ayant consulté un médecin généraliste pour leur première demande de contraception. Résultats : Quatre principales attentes de cette consultation ont été retrouvées et étaient des demandes d'informations sur les différents moyens de contraception possibles, la conduite à tenir en cas d'oubli de pilule, les douleurs menstruelles et les infections sexuellement transmissibles. Les attentes ne différaient pas en fonction de l'âge des patientes. Une crainte importante est celle d'un examen gynécologique. Les patientes se sentaient plus à l'aise en présence d'une femme médecin et posaient plus facilement des questions en l'absence d'un accompagnant. La contraception majoritairement choisie était la pilule (91,5 % des cas). 86,4 % des patientes interrogées se disent satisfaites de cette consultation. Conclusion : Le médecin généraliste est un acteur majeur concernant la contraception féminine. L'attente principale des patientes est de discuter des différents moyens de contraception possibles afin de la choisir avec le médecin. Il est important de rassurer les patientes concernant cette consultation et de les rencontrer seules afin de pouvoir aborder les sujets importants pour elles.

Le médecin généraliste n'est pas la première source d'information en matière de contraception. L'expérience personnelle ou d'un tiers prime sur le discours médical. Le gynécologue est la source médicale de référence pour la contraception malgré son manque parfois de disponibilité. Les difficultés de communication entre médecins et patientes sont souvent dues à un discours qui n'est pas assez vulgarisé pour la femme qui est en face. Les patientes sont très attachées à la relation de confiance qu'elles ont avec leur médecin et au temps consacré à la consultation. Elles sont aussi en demande d'une prescription plus individualisée. Une consultation obligatoire de prévention sur la contraception peut être un moyen d'informer les femmes tôt dans leur vie sexuelle pour éviter la survenue d'une grossesse non désirée. Conclusion : l'étude qualitative a permis de mettre en évidence des témoignages concrets sur les femmes et leur vision de la contraception. C'est au médecin de prêter attention aux attentes et aux connaissances des patientes pour en améliorer l'information.

Book excerpt: Introduction : Le taux d'IVG a diminué juste après la loi de 1975 sur la légalisation de la contraception, mais depuis plusieurs décennies son taux stagne malgré plusieurs actions des pouvoirs publiques pour y remédier. Dans ce contexte nous avons souhaité cibler les attentes des jeunes filles lors de le leur première consultation pour une demande de contraception afin peut-être trouver des explications à ce problème. Matériels et Méthodes : Nous avons réalisé une enquête qualitative dans le Jura, dans des cabinets de médecins généraliste, au moyen d'entretiens semi-dirigés, auprès de jeunes filles venant ou étant venues récemment chez leur médecin généraliste pour une première demande de contraception. Nous avons ensuite retranscrit les entretiens et avons effectué une double analyse du verbatim à l'aide du logiciel Nvivo. Nous avons obtenu une saturation des données au bout de treize entretiens. Résultats : Nous avons constaté que les jeunes filles avaient de nombreuses connaissances sur le sujet de la contraception mais qu'elles étaient superficielles. Elles souhaitaient donc des informations précises, notamment sur les effets secondaires des différentes méthodes contraceptives, sur leur fonctionnement et bien sûr savoir quoi faire en cas d'oubli. Elles souhaitaient que la consultation se fasse dans un climat de confiance et de rassurance par leur médecin. D'ailleurs, elles attendaient de lui qu'il soit pédagogue, à leur écoute afin de pouvoir discuter librement, qu'il ne les juge pas et qu'il respecte le secret médical. Par rapport à la place des parents lors de cette consultation, même si elles étaient plusieurs à ne pas souhaiter leur présence, celles qui y étaient favorables avaient l'espoir que leur médecin de famille soit un médiateur auprès de leurs parents et qu'il permette de briser le tabou de ce sujet au sein de leur famille en répondant aux questions et inquiétudes de leurs parents. Conclusion : Notre étude a mis en évidence l'importance du médecin généraliste dans la prévention et l'information des jeunes filles au sujet de la contraception et de l'éducation sexuelle. Néanmoins il intervient sans doute trop tard dans la vie sexuelle des jeunes filles qui pour la moitié ont déjà eu un rapport sexuel avant leur première consultation de contraception.

L’objectif de l’étude était d’explorer le vécu et l’expérience des femmes dans leur parcours contraceptif pour mieux comprendre ce paradoxe. Il s’agit d’une étude qualitative par entretiens semi directifs de femmes consultants en médecine générale en Ile-de-France, avec analyse thématique. Treize entretiens d’une durée moyenne de 35 minutes ont été réalisés avec des femmes âgées de 22 à 49 ans. Nous avons pu explorer les représentations des méthodes contraceptives et les facteurs influençant leur choix. Nous avons mis en évidence de nouvelles notions qui entrent en jeu dans le choix d’une méthode contraceptive : la gestion active et le contrôle par la femme du moyen contraceptif (pilule, anneau, patch) ou la délégation au corps médical (DIU, Implant) et l’importance de la localisation ou du mode d’administration du moyen contraceptif qui modifie la perception de son efficacité et de sa fiabilité. Au terme de ce travail, il semble important d’aborder lors de la consultation, la place de la femme dans la gestion active ou non de la contraception, puis la localisation qui semble lui convenir le mieux. Il semble utile d’aborder précocement la sexualité en consultation, avec l’adolescente seule, pour anticiper le besoin d’une première contraception et l’informer des moyens disponibles, y compris DIU et implant. Ainsi le médecin généraliste semble être l’interlocuteur privilégié pour informer, accompagner et suivre les femmes pour la contraception.

It would create offspring who have genetic material from two women, something never sanctioned in humans, and would create mitochondrial changes that could be heritable (in female offspring), and therefore passed on in perpetuity. The manipulation would be performed on eggs or embryos, would affect every cell of the resulting individual, and once carried out this genetic manipulation is not reversible. Mitochondrial Replacement Techniques considers the implications of manipulating mitochondrial content both in children born to women as a result of participating in these studies and in descendants of any female offspring. Nelly Oudshoorn explains why it is that, although the technical feasibility of male contraceptives was demonstrated as early as the 1970s, there is, to date, no male pill. Ever since the idea of hormonal contraceptives for men was introduced, scientists, feminists, journalists, and pharmaceutical entrepreneurs have questioned whether men and women would accept a new male contraceptive if one were available. Providing a richly detailed examination of the cultural, scientific, and policy work around the male pill from the 1960s through the 1990s, Oudshoorn advances work at the intersection of gender studies and the sociology of technology. Oudshoorn emphasizes that the introduction of contraceptives for men depends to a great extent on changing ideas about reproductive responsibility. Initial interest in the male pill, she shows, came from outside the scientific community: from the governments of China and India, which were interested in population control, and from Western feminists, who wanted the responsibilities and health risks associated with contraception shared more equally between the sexes. She documents how in the 1970s, the World Health Organization took the lead in investigating male contraceptives by coordinating an unprecedented, worldwide research network. She chronicles how the search for a male pill required significant reorganization of drug-testing standards and protocols and of the family-planning infrastructure—including founding special clinics for men, creating separate spaces for men within existing clinics, enrolling new professionals, and defining new categories of patients. The Male Pill is ultimately a story as much about the design of masculinities in the last decades of the twentieth century as it is about the development of safe and effective technologies.
