Book excerpt: The current study was part of a larger study where overarching purpose was to design a daily measure of everyday cognitive performance. The current investigation examined the extent to which self-report pain was related to cognitive performance in a sample of 148 community dwelling older adults (M = 73 years old, SD = 6.84). A multidimensional battery was used to assess socio-demographics, physical health (e.g. self-report pain, self-report physical health), mental health (e.g. negative affect, life-event stress, and daily stress), and cognitive functioning (e.g. short-term memory, processing speed, inductive reasoning, and working memory) at pretest and over 8 occasions. The direct relationship between self-report pain and cognition was examined as well as potential moderators and mediators of the pain-cognition relationship both at the between- and within-person levels. At the between-person level, self-reported pain was found to be significantly related to processing speed, where high pain was associated with slower reaction time. At the within-person level self-reported pain was found to be significantly related to working memory, where on days when self-reported pain was higher than an individualÃ¢â'¬â"¢s average, their working memory performance suffered. Within-person self-reported pain X age interactions were found for short-term memory, processing speed, and working memory. Additionally, negative affect and stress were found to moderate the pain-cognition relationship. The strength of the pain-cognition relationship is best understood by examining moderating factors such as age, stress, and negative affect. Furthermore, the handful of significant interactions at the within-person level suggest that the pain-cognition relationship is dynamic and should not be assessed at only one time point.

Book excerpt: Given the increasing number of older adults in the population, the fact that about 1 in 10 people over the age of 65 will develop mild cognitive impairment, and the substantial individual, familial, and financial burden associated with such disorders, the need for innovative research examining cognitive impairment in older adults is evident. The present study used a microlongitudinal design to assess cognition and contextual factors that may affect cognition for 14 consecutive days using a daily diary method in older adults with varying degrees of cognitive function. This study design enables investigation of concurrent associations between variables, as well as providing unique information not gleaned from the traditional focus on mean values of cognition. The present study had two broad aims: 1) to compare variability in cognition in older adults with varying degrees of cognitive impairment and 2) to investigate relationships between daily cognitive performance, variability in cognitive performance, and contextual factors that may influence daily cognitive performance and variability in older adults with varying degrees of cognitive impairment. Results suggest there was sufficient intraindividual variability in daily cognition to warrant investigation of within-person associations. Furthermore, the contextual factors of pain, stress, and sleep were predictive of cognitive performance, but with significance and directionality of these associations depending on level of measurement (baseline, daily, or mean values). Finally, associations between contextual factors and cognition were frequently conditional upon baseline cognitive status. The findings highlight the need for continued examination of these associations to expand our understanding of cognition in older adults and to discover potential targets for interventions to attenuate cognitive decline.

Book excerpt: Summary Introduction Chronic pain is frequently reported in older adults and the prevalence will only increase with the rising number of older adults as a result of the baby-boom generation. The aim of our studies was to clarify the association between pain and several domains of cognitive functioning in older adults, which may shed light on potentially modifiable risk factors for cognitive impairment in aging. Pain and cognition cross-sectionally We found that pain severity was associated with impaired memory, executive functioning, and general cognitive performance and pain interference (with daily activities) with impaired attention, memory, executive functioning and general cognitive performance, after adjustments for age, gender, race and education. However, many of these associations were not statistically significant anymore after additional adjustments for chronic conditions and psychiatric medications. The association between pain interference and memory and general cognitive performance in older adults persisted, even after adjusting for demographics, chronic conditions, behaviors, and psychiatric medication. We also found that the observed relationships between pain and cognitive performance were partly explained by the effect of pain on attentional resources. Therefore we focused on the association between pain and attention in the next chapters. First we found that the Test of Everyday Attention (TEA), which was only validated to measure attention in adults 18-80 years old, is also a valuable tool to measure attention in the very old (chapter 3). However, the Visual Elevator Test, which measures the domain of attentional switching, was the most challenging test for these older participants. In chapter 4 we showed that higher pain severity was associated with poorer scores in measures of selective and sustained attention. Pain interference scores were also significantly inversely associated with selective attention.

Approximately one-quarter of community-dwelling Americans aged 65 and older are considered to be socially isolated, and a significant proportion of adults in the United States report feeling lonely. People who are 50 years of age or older are more likely to experience many of the risk factors that can cause or exacerbate social isolation or loneliness, such as living alone, the loss of family or friends, chronic illness, and sensory impairments. Over a life course, social isolation and loneliness may be episodic or chronic, depending upon an individual's circumstances and perceptions. A substantial body of evidence demonstrates that social isolation presents a major risk for premature mortality, comparable to other risk factors such as high blood pressure, smoking, or obesity. As older adults are particularly high-volume and high-frequency users of the health care system, there is an opportunity for health care professionals to identify, prevent, and mitigate the adverse health impacts of social isolation and loneliness in older adults. Social Isolation and Loneliness in Older Adults summarizes the evidence base and explores how social isolation and loneliness affect health and quality of life in adults aged 50 and older, particularly among low income, underserved, and vulnerable populations. This report makes recommendations specifically for clinical settings of health care to identify those who suffer the resultant negative health impacts of social isolation and loneliness and target interventions to improve their social conditions. These impairments are established largely on reports of signs and symptoms of impairment and functional limitation.
