Book excerpt: Contexte : les symptômes, révélateurs de la maladie, sont souvent découverts à un stade de la maladie trop avancé. Un retard dans l'évaluation des symptômes serait présent chez 60 % des patients atteints de cancer qui consultent tardivement (Ristvedt, 2005). Certains déterminants au retard ont été étudiés. Leurs mécanismes et interactions psycho-socio-comportementaux amenant à la consultation sont peu clairs. Matériau et méthode : notre étude est de type qualitatif par récits de vie. Elle aborde dans une chronologie les expériences du sujet. Les données sont recueillies lors d'entretiens individuels puis comparées. Nous essayerons de modéliser la dynamique des interactions entre les déterminants de la consultation tardive. Résultats : quatre axes sont ressortis : les temporalités (patient, médecine), le suivi médical, les représentations des patients et ses dimensions personnelles. La temporalité du patient est marquée par une minimisation du symptôme et une faible vigilance corporelle. La gravité perçue est retardée. Des mécanismes cognitifs d'adaptions et de défense sont à l'œuvre. Cette temporalité est sous l'influence des représentations du patient et de la qualité du suivi médical. Les dimensions personnelles agissent directement sur les perceptions de temporalité et les représentations. Conclusion : un modèle psycho-socio-comportemental a pu être établi. Le retard à la consultation est la conséquence de retard sur deux niveaux de basculement : social-environnemental et psychique-interne. L'identité à la maladie est retardée. En jouant sur les déterminants, la survenue plus précoce des points de basculement permettrait de prévenir le retard à la consultation.

Book excerpt: La réforme de l’Assurance Maladie est en application depuis juillet 2005 et encourage le parcours de soins coordonnés au sein duquel le médecin traitant joue un rôle pivot. Tous les bénéficiaires de l’Assurance Maladie sont invités, à partir de l’âge de 16 ans, à déclarer un médecin traitant. En septembre 2007, 20% de la population n’avait pas encore adhéré au système. Quels sont les facteurs influençant l’absence de déclaration du médecin traitant, et quel est le profil social et médical des patients n’ayant pas effectué leur déclaration ? Nous avons réalisé une étude prospective, observationnelle, transversale et mono observateur, auprès de 269 patients représentatifs de la population d’Ile-de-France, consultant en médecine générale de ville et hospitalière entre février et avril 2008. Deux profils de patients se distinguent nettement. Premièrement, les moins de 35 ans. Il s’agit surtout de patient de sexe masculin, étudiant ou ayant un emploi, sans pathologie chronique ni ALD 30, ayant un niveau d’éducation supérieur, et qui n’ont pas effectué la déclaration soit parce qu’ils n’en ressentent pas la nécessité sur le plan médical, soit par manque d’opportunité, soit par refus du système. Deuxièmement, les patients vivant dans la précarité, qui n’ont effectué leur déclaration parce qu’ils ne connaissent pas l’existence du système du médecin traitant. En conclusion, nous pouvons dire que si l’Assurance Maladie souhaite une meilleure adhésion des assurés au parcours de soins coordonnés, il est nécessaire qu’elle effectue une politique d’information auprès de ces 2 populations cibles, qui risquent, sinon, d’être pénalisées.

Book excerpt: Much orthodox economic theory is based on assumptions which are treated as self-evident: supply and demand are regarded as independent entities, the individual is assumed to be a rational agent who knows his interests and how to make decisions corresponding to them, and so on. But one has only to examine an economic transaction closely, as Pierre Bourdieu does here for the buying and selling of houses, to see that these abstract assumptions cannot explain what happens in reality. As Bourdieu shows, the market is constructed by the state, which can decide, for example, whether to promote private housing or collective provision. And the individuals involved in the transaction are immersed in symbolic constructions which constitute, in a strong sense, the value of houses, neighbourhoods and towns. The abstract and illusory nature of the assumptions of orthodox economic theory has been criticised by some economists, but Bourdieu argues that we must go further. Supply, demand, the market and even the buyer and seller are products of a process of social construction, and so-called ‘economic' processes can be adequately described only by calling on sociological methods. Instead of seeing the two disciplines in antagonistic terms, it is time to recognize that sociology and economics are in fact part of a single discipline, the object of which is the analysis of social facts, of which economic transactions are in the end merely one aspect. This brilliant study by the most original sociologist of post-war France will be essential reading for students and scholars of sociology, economics, anthropology and related disciplines.
